******************************************************************************** To: All HKCC Students From: Hong Kong Community College Date: 19 March 2016 Subject: Mobile Student Feedback Questionnaire (mSFQ) Exercise, Semester Two 2015/16 ******************************************************************************** Dear Students Classes of Semester Two will soon come to the end. It is time for us to collect your feedback concerning the learning outcomes, subject syllabus, assessment method, teaching, etc., which is valuable to the ongoing review of our programmes and will help enhance teaching and learning performance synergistically. The mobile Student Feedback Questionnaire (mSFQ) Exercise, which is a formal feedback channel deployed by the College to let you fully express your opinions on the subject, will be carried out next week. As in the past, for the subjects where the lecture and tutorial sessions are taught by the same lecturer, the mSFQ Exercise will be carried out during a lecture with the SFQ form “SFQ A” containing items related to both the lecture and tutorial sessions; otherwise, the mSFQ Exercise will be conducted in separate lecture/tutorial session with respective SFQ forms “SFQ L” or “SFQ T”. The mSFQ forms are attached herewith for your reference. We encourage you to read the “Procedures for conducting in-class Mobile SFQ” at https://ssoportal.cpce- polyu.edu.hk/mobileSFQ/ to familiarize yourself with the process. The following points concerning the mSFQ Exercise are highlighted for your attention: 1. The College will process the information you provided in the Questionnaire in strictest confidence. The survey results will only be delivered to the subject lecturer/tutor in June 2016, after the release of your assessment results. Therefore, please be assured that your opinions will absolutely not affect your results. 2. The Questionnaire is conducted online, you are thus reminded to bring your own mobile device / notebook and make sure they are more than 40% battery charged. Kindly share you mobile device / notebook with your classmate who does not have the device after your own submission. 3. You need a valid CPCE AD account (i.e. Account to login Student email / campus computer / Moodle) to access the mSFQ system. 4. Please get a QR code scanner for your mobile phone, it helps you to access the hyperlink of mSFQ quickly. You may download the suggested QR code scanner app from https://ssoportal.cpce-polyu.edu.hk/QRcode/ or your preferred QR code scanner app from Apple Store or Google Play. 5. Please be reminded to answer all sections and click the “Submit” button at the bottom of the questionnaire when you finish. 6. The College wishes to collect your personal opinions. There is no need to discuss your views with your classmates. 7. You should complete only one Questionnaire for each class. Please do not take away any forms, we will collect them at the end of the session. The procedures of carrying out the mSFQ Exercise are as follows:

Dear Students

Classes of Semester Two will soon come to the end. It is time for us to collect your feedback

concerning the learning outcomes, subject syllabus, assessment method, teaching, etc., which is

The mobile Student Feedback Questionnaire (mSFQ) Exercise, which is a formal feedback channel

1. One or more HKCC appointed staff members (carrying the title of Investigating Officer) will

visit your class during the teaching weeks 10 – 12 (in teaching week 9 for some classes).

2. The subject lecturer/tutor will be requested to leave the classroom during the time you fill in the

mSFQ form. Meanwhile, only students who are officially registered in the class will be allowed

to remain in the classroom.

3. The Investigating Officer will show you the PowerPoint slides with audio instructions, and start

the mSFQ system. You will be given approximately 10 minutes to login the system and then

complete and submit the form.

4. You may raise your hand to request a blank form for providing feedback of open-ended

questions in written format.

5. After the above-mentioned 10 minutes, your feedback collected will be sent for analysis in

confidence.
