공기소실폐 ( 무기폐 )

공기소실폐 ( 무기폐 ) 란 ?

• 공기가 빠져 풍선처럼 폐가 찌부러지는 현상• 폐 전체 혹은 부분폐 (lobe) 에 발생 가능

• 부분폐 일부분 (subsegmental)• 부분폐 (lobe, 소엽 )• 폐 전체 (whole lung)

• Subsegmental atelectasis 가 가장 흔함

원인 Causes of Lobar/Whole Lung Atelectasis

• 기관지 종양 (bronchogenic carcinoma)• 천식성 점액 (mucus plugging)• 이물질 삼킴• 수술 후 분비물 축적• 호흡관 ( 기관 ) 의 깊은 삽입

가슴 방사선 사진 소견

• 구조물 이동• 호흡길 ( 기도 ) 치우침• 종격동 치우침• 정상 폐의 치우침• 가슴배구분막 ( 횡격막 ) 의 상승• 부분폐 경계 ( 엽간열 ) 의 전이 • 폐혈관의 집중현상

• 폐의 백색화 ( 폐음영의 증가 , 회백화 , opacification)• 폐알채움형 ( 고형질화 ) 에서도 나타나나 공기소실폐 ( 무기폐 )

에서는 폐가 쭈그러드는 것이 다름

좌상부폐 공기소실증의 전형적 특징

1) 직접소견 : 좌상부폐의 백색화 (Hazy opacity in left upper lung)

2) 간접소견 : Left tracheal shift (indirect sign)

3) 간접소견 : 좌측 심장 윤곽선 소실증 (Loss of left cardiac silhouette)

• The left lung has 2 lobes (upper, lower), and the right lung 3 lobes (upper, middle, and lower)

• The collapse of each lobe has specific appearances on a CXR

• 폐암과 결핵 등을 반드시 고려• 모든 환자들에 대하여 원인 검사 필요• 공기소실폐의 방사선 사진의 모습은 매우 다양하다는 사실을
