1. SCOPE OF THE REPORT
The report analyzes the global app publishing activities of the 12 leading Pharma companies on Android
and iOS devices. The research provides a detailed picture of which app categories Pharma companies
concentrate on, how many apps they have published, which user groups they target, how they organize
their app business and how successful they are.
The report analyzes app publishing activities on three levels:
Level 1 - Pharma app publisher industry view: The first chapter provides an overview of how Pharma
companies are making use of the app channel today and how this has changed over the last 12 months.
Level 2 - Comparison of Pharma app activities: In the second part of the report, the companies are being
analyzed on a company level and compared against each other.
Level 3 - Pharma app publisher profiles: The third part provides detailed profiles of the companies’ app
activities. It also highlights the most successful apps for professional and private users (based on
downloads).
Companies in scope: Abbvie/Abbott, Astra Zeneca, Bayer HealthCare, Bristol-Myer Squibb,

3. MANAGEMENT SUMMARY
Since Apple has created the new app ecosystem, the mHealth app market segment has attracted much
attention and created hope among the mHealth app developer community.
mHealth apps promise to improve the medical adherence, let doctors get into direct contact with
patients, help hospital reduce hospital re-admissions and length of stay or reduce costs of medical trials to
name a few.
Pharma companies have taken on these opportunities and actively published apps since the beginning of
the market in 2008/2009. This benchmarking study examines how far the 12 largest Pharma companies
got after five years of app publishing and whether or not they have reached a comparable market position
in the app economy as they have in the healthcare industry.
Pharma companies are active app publishers: The leading 12 Pharma companies have published 497
unique apps. A unique app is an app which might have been published in different countries or as
platform versions, but its functionality, design and usability are same. Altogether these 497 unique apps
represent 736 app titles available for iOS and Android.
The app portfolio is diverse: 32% of apps by the leading 12 Pharma companies are medical reference
apps that mainly provide information about symptoms, medications, treatments, devices, location of
hospitals or doctors’ offices. The second largest group (20%) of apps published by Pharma companies are
medical condition management apps. These apps are supposed to support patients or just health
conscious people to manage their disease or health e.g. diabetics, asthmatics or fitness enthusiasts. The
rest of the app portfolio of Pharma companies includes a broad variety of other app categories including
recipes, games, diagnosis tools, pill reminders, eyesight testing, doctor’s appointment management and
many more.
The companies use neither common design standards nor internal app promotions to increase visibility
and download numbers.
The majority (57%) of apps target private users and are published on iOS (64%).
The reach of the app portfolios is limited: The number of all-time downloads Pharma companies have
been able to generate with their app portfolios is 6.6m1. This is a very low reach compared to mHealth
app market leaders like MyFitnessPal which alone has more than 70m downloads (on iOS and Android)
and more than 6m active users from just one app. The share of downloads which come from the leading
Pharma companies top 3 apps varies between 22% (Novartis) and 99% (Merck). 9 out of 12 Pharma
companies have generated more than 50% of downloads with just three apps. The 12 leading Pharma
companies have altogether less than 1m active users. Only four out of 12, boast more than 100,000 active
users (Sanofi, Merck, Johnson&Johnson and GSK).
Pharma companies use a decentralized app publishing approach: Most of the Pharma companies publish
their apps under multiple publishing units. The spectrum ranges from 1 (Bristol-Myers Squibb) to 17
(Novartis).
The app portfolio is not globally available: Almost half of Pharma company’s app publishing entities
target only local markets. 49% of the total 65 publishers that stand behind the Pharma app portfolios
publish their apps in 3 or less countries.

The performance benchmarking of the leading Pharma companies shows significant differences in
downloads, apps users, app categories, country focus, governance models and target groups. None of the
app portfolios has a significant impact in the mHealth app market. There are only two apps which have
generated more than a million downloads since they have been launched (GoMeal and MerckPTE).
In order to have a bigger impact on the mHealth app market Pharma companies must thoroughly improve
their app publishing approach in 7 main areas:
1. Apply design guidelines and internal app referencing 2. Concentrate app activities on where the market demand is 3. Focus on mHealth data collection 4. Open up APIs2 to build a connected app portfolio 5. Strengthen Android app presence and make use of cross platform app development tools 6. Become truly global 7. Align app publishing governance and competencies
It is also questionable whether or not Pharma companies will ever become good enough app publishers to
be able to compete against the 100% dedicated start-up companies or big tech giants like Apple or Google
which have recently shown much interest in the mHealth app market.
Each Pharma company should challenge and question their current performance in the app market. The
companies should start a discussion around whether or not it will be a good investment of time and
money to develop and publish the next 700 apps the next five years. Otherwise, Pharma might be better
skilled to undertake a role of an investor, incubator, app aggregator or mHealth data provider?
What this all means is that before going forward in the same way as the last five years, Pharma companies
should rethink their mHealth app strategy. Otherwise they will remain an industry with low impact on the
mHealth app market. This is not a serious threat to their core business today, but in five years it might be.

4. PHARMA APP MARKET: HOW THE PHARMA INDUSTRY USES THE MOBILE APP CHANNEL
TODAY
The mHealth app market has been experiencing new dynamics since the market of connected wearables
(e.g. activity trackers, connected watches and blood pressure sensors) became a mass market. Only after
these devices have become available to the masses, has the mHealth data potential been seriously
perceived as the new oil of the healthcare industry.
Companies like Validic and Human API have built their business models around apps
that capture health data from an increasing number of self-tracking and monitoring
app and device users. The introduction of the HealthKit platform from Apple and the
expected similar moves from its competitors like Google and Samsung gives the
mHealth app developer community much hope. Namely, these movements will
potentially lead to thousands of apps which collect, store, aggregate, analyze, display
and share health and fitness data.
The resulting explosion of health data stored on many different silos will create
inexhaustible opportunities for the healthcare systems to improve treatment, drugs
and reduce cost.
This mega trend is creating an enormous threat and also a great challenge for the
Pharma industry. Already today millions of patients synchronizing their e.g. blood
pressure, temperature, glucose levels, mood, skin pictures, activities, food intake and
many more metrics in order to manage their health condition and potentially in the future better
understand the impact of a therapy or a drug. This direct channel (apps) which patients use on a
continuous basis is potentially so valuable that one does not understand why Pharma companies do not
play a much more visible role in the mHealth app economy.
4.1. THE PHARMA APP PORTFOLIO SIZE AND PERFORMANCE
Since last year the app portfolio of the leading 12 Pharma companies has grown by 61% to 497 unique
apps3 released on Apple App Store and Google Play. Altogether the number of apps available in both
stores is 736.
Around 45% of these apps have been published on both platforms (iOS and Android), an increase from
18% in 2013.

Most of the Pharma companies have a decentralized app publishing strategy.
The 12 Pharma companies’ apps have been published by 65 different publisher
entities. Novartis and Johnson&Johnson have the most decentralised publishing
strategy with 17 different registered entities each. Bristol Myers Squibb on the
other hand, has hired only 1 publisher for their app portfolio.
The impact of the app portfolio expansion on downloads
The enlarged app portfolio and additional year in market has had its impact on the number of all-time
downloads. All-time downloads are the accumulated downloads of each app since the app’s launch.
Within a year, the total all-time download number has increased by 197% (yoy4), reaching 6.62m. The
average lifetime download number per unique app has increased from 7,300 to 13,300.

The number of monthly active users has risen to 972,000. This means that 15% of iOS and Android
smartphone or tablet users who have downloaded a Pharma app use it at least once a month. This is in
line with the market average.
Only a few apps get all the attention and downloads
Within the 497 unique apps there is only a very small number of apps which have generated a notable
number of downloads. The most downloaded 5 apps alone represent 57% of the total downloads. These
apps are “GoMeal” by Sanofi Aventis, the periodic system education app “PTE” by
Merck, “Pill Reminder” by Bayer Healthcare, “Aquafresh Brush Time” by GSK and
“Similac Baby Journal” by Abbott.
Only two apps (“GoMeal” and “Merck PTE”) are in the “one million download league”.
The top 20 apps account for 3 out of 4 downloads of all Pharma apps. The “long tail” of
Pharma apps is substantial.
Only the two most downloaded apps have more than 100,000 active users. Another ten have more than
10,000 active users. These figures include users from iOS and Android, tablet and smartphone and all
country versions of these apps.
The vast majority of Pharma apps are not used by a notable number of users.

The majority of mHealth apps that are available in the app stores target private users. Still Pharma
companies have a much higher focus on providing mobile app services for healthcare
professionals than the rest of the market.
43% of their unique apps target doctors, nurses, clinicians and business partners. These
apps cover a broad range of use cases including e.g. apps that help doctors educate their
patients about vascular health (Abbott), application helping clinicians estimate a patient's
baseline risk of in-hospital major bleeding (Astra Zeneca), apps for the diagnosis and
treatment of cardiovascular diseases (Bayer Healthcare) and other.
Most of these apps do not have any access restriction (e.g. doctors ID, or passcodes provided by the
Pharma sales representative) and, if they have an educational purpose, can be used by private users as
well.
Download and usage share is not higher for these apps than of apps which target private users directly.

4.2. THE 10 MAIN APP CATEGORIES OF PHARMA
The app portfolio of Pharma companies is as diverse as the rest of the mHealth app market. The apps
from Pharma companies can be categorized into 10 main categories. Please note that there is an overlap
between these main categories as most of the apps have a relatively broader value proposition. To
categorize an app for the purpose of this report, apps have assigned to one of the main categories based
on their main use case. The 10 app categories Pharma companies concentrate on are:
Medical reference apps: Pharma companies publish apps which provide reference to symptoms, diseases, drugs or medical treatments. Quite often they also help users find the nearest doctor’s office or a hospital. These medical reference apps account for 33% of all unique apps available on Apple App Store and Google Play published by one of the 12 leading Pharma companies. An example is Medinfo by Novartis that allows health care professionals (HCPs) to search for the latest medical information, submit inquiries and report adverse events for Novartis products.

Medical condition management is the second largest group of apps (20%). A typical example in this category is the “Astma” app by GSK. The app targets asthma patients and lets them manage their disease better. The app includes a diary, graphics which show historical test results, pollen, fine dust, UV, ozone levels, helpful tips and a reminder function for regular testing. Other supported medical conditions are multiple sclerosis, diabetes, acromegaly, headache, pain or addiction.
Tools accounts for 8% of all apps. Tools mainly target HCPs and support a diagnosis or calculate a dose of a drug. An example is the ”Index Hemato” app by Roche. The app provides a selection of classification and prognostic index of hematological malignancies. A typical calculator is the “Clearance - GFR Calculator” app by Abbott. This calculator estimates the creatinine clearance based on serum concentration and age.
Kids: 6% of apps published by Pharma companies provide services for child health and baby care. These apps provide advice on proper teeth-brushing (e.g. “Aquafresh Brush Time” by GSK), track the overall development of a baby including sharing features and photo album (e.g. “Similac Baby Journal” by Abbott) or manage vaccination (e.g. “Vaximate” by Pfizer).
Health improvement apps represent 6% of apps released by Pharma companies. They cover a broad spectrum of mobile supported use cases. This includes general health tracking apps which track water and food consumption as well as sport and brain activities. They also support the user to stay in a better mood e.g. “Smidge 2.0” by Pfizer. This category also lists health games e.g. “Happy Nurse” by J&J, a simple jump and run game and step counters like “WalkAlong” by GSK and motivation apps e.g. “Rewards” by Novartis.
Compliance apps (3%) are mainly medical reminders like “Pill Reminder+” by Bayer Healthcare. Some simply support the regular taking of the birth control pills e.g. “PillReminder - Denk an mich” by Sanofi.
Medical education (3%) apps make use of videos, graphics, tests and tutorials to educate the HCPs, students, teachers or non-professional users about the periodic system (e.g. “Merck PTE” by Merck), human anatomy (e.g. “Vascular Videos: Private Education” by Abbott) or symptoms (e.g. “Understanding Acute Coronary Syndrome (ACS)” by Astra Zeneca).
Woman: 2% of apps target women exclusively. Such apps give advice on pregnancy, sexuality or fertility (e.g. “Aşkı Şansa Bırakma” by Bayer Healthcare).

Nutrition and Wellness: Nutrition and wellness apps each represent only 1% of the total app portfolio of Pharma companies. There are apps that provide tips for a better sleep (e.g. “SleepHelp” by Pfizer), recipe apps (e.g. “Recipes2Go” by Pfizer) or beauty tips (e.g. “ACUVUE® DEFINE” by J&J).
There are many more mobile use cases which are addressed by Pharma companies and which have been summarized in the other category section (17%). These apps include e.g. PHRs, eyesight training, pet care or doctor’s appointment to name a few.
Although the app portfolio of Pharma companies covers all app subcategories which exist in the mHealth
app market, they put importance on other use cases. The main difference is the quasi non-presence in the
most popular mHealth app markets’ categories: fitness, wellness and nutrition/weight loss apps (not
shown in the graph).
This suggests that Pharma companies’ app portfolio is closely related to their core products and not
necessarily to the main market needs.

4.3. PLATFORM PREFERENCE AND GEOGRAPHICAL REACH OF PHARMA APPS
Pharma companies have started to develop Android versions of their apps, but iOS is still the number one
platform in terms of the number of available apps, downloads and active users. 64% of the Pharma apps
are available on iOS compared to 36% for Android. All-time downloads and user numbers of iOS apps are
in the same relation to Android as the app count. The emphasis on iOS is in contrast to the Android’s
global dominance6. One reason for this loyalty to iOS might be that Pharma companies have originally
started publishing their apps on iOS and have just not had the resources or, as a result of unsatisfactory
app results, motivation to develop and maintain an Android version of their iOS apps. Another reason
might be that Pharma companies also tend to concentrate on the USA market which has a higher iOS
share than the global average (see next chapter).
The iOS focus means that only 1/3 of Pharma’s app portfolio targets the largest and fastest growing app
market (Android).

Almost half of Pharma companies’ app publishing entities target only local markets. 49% of the 65
publishers who stand behind the leading Pharma companies’ app portfolios release apps in 3 or less
countries. In fact, 59% of these local publishers focus on just one country.

5. APP PORTFOLIO COMPARISON
The app activities of the leading Pharma companies show significant differences in number of apps
published, total downloads, active user base and app category focus.
5.1. COMPARISON OF APP PORTFOLIO PERFORMANCE
All Pharma companies have a preference for iOS. Most, if not all, launched their first apps on the iOS
platform and kept Apple users as their main target group since then, despite the fact that the addressable
market for Android phones and tablets is 3-5 times bigger, depending on the region.
There are four Pharma companies which have published around 100 apps for iOS and Android. Sanofi is
the leading Pharma app publisher with its 107 available apps on iOS and Android. The iOS share varies
between 55% (GSK) and 98% (Roche).
The more apps in app portfolio, the higher the share of apps developed for Android. Small publishers stick
to iOS.

Despite the large app portfolios, there are only 3 companies that have been able to generate more than
one million downloads with their apps.
Download numbers are not the only valid performance indicator, but if a publisher is not able to gain a
significant reach within the target group via downloads, any app-related business model is likely to fail.
Sanofi is the only company in the panel that has generated more than 2 million all-time downloads for
their 107 apps. On the other hand, GSK which boasts the second largest app portfolio has attracted only
700,000 downloads since the beginning of their app activities.

Sanofi, Merck and Johnson&Johnson are clearly the app publishers with the highest reach
within the Pharma company benchmarking. Nevertheless, compared to mHealth app
market leaders like MyFitnessPal (70m downloads), Azumio or WebMD (both around

market.
AstraZeneca, Bristol-Myer Squibb, Novartis, Pfizer and Roche do not show any significant
uptake of their app portfolio on any of the platforms.

This picture even more disappointing if the share of the three most successful apps of an app portfolio is
taken into consideration. The top 3 apps’ share is for 9 out of 12 companies’ higher than 50% of all
downloads. The app portfolios of Abbott/Abbvie, GSK, Merck, Roche and Sanofi rely to
more than 80% on the performance of their top 3 apps. Merck’s app portfolio’s
performance is backed up to 97% by the Merck PTE app which is an educational app that
provides detailed information and visual aids of the periodic table of the elements. Sanofi
has generated most of its downloads with the GoMeal app (58%), an app that mainly
targets diabetics, provides food recommendation and tracking as well as activity tracking.
The download numbers of the top 3 apps per company include various country versions
as well as a secondary platform copy. This clearly shows that the Pharma company app
portfolio performance just depends on a very few apps.
On the other hand, this also means that the vast majority of apps have none or only little impact on the
app market. Most of the apps have attracted a few hundred downloads and hardly any active users.
The large “long-tail” of apps from Pharma companies as well as the lack of apps with a wide reach in the
market shows that Pharma companies have not yet found the right approach to the mHealth app market.

It is evident that the download of an app is a prerequisite for its usage. At the same time, the most
downloaded apps are not used after a few weeks or even days.
Thanks to the success of the top apps of Sanofi (“GoMeals”), Merck (“Merck PTE”), Johnson&Johnson (“7
Minute Workout”) and GSK (“Aquafresh”), these companies have also managed to maintain the largest
active user base. All four companies have more than 100,000 active users. Sanofi again leads the
benchmarking with more than 300,000 active users. A Monthly Active User is defined as a user who opens
an app at least once a month (MAU).
The share of active users in relation to the number of downloads (MAU share) is a good
indication of how well the apps succeed in engaging with their users over time. With only
12% MSD and Roche (13%) has the lowest and Johnson&Johnson (22%) the highest MAU
share in the benchmarking. This translates to a very challenging situation for MSD and
Roche. The company has to generate almost twice as many downloads with their app
portfolio to reach the same user number as Johnson&Johnson.
All of the apps in the sample are free to download. The main objective of the Pharma’s apps
is therefore not to generate additional revenue source, but more likely to create brand
awareness, offer additional services and to get into direct contact with the persons who
take their medications. Looking at the number of apps, the number of users all Pharma
companies have and assuming significant effort that has been put into the development of
the app portfolio, it is safe to say that they have not reached their goals yet.
For example, a company like Novartis has 70 apps listed in the Apple App Store and Google Play store and
reaches out to only around 30,000 users, whereas small start-up companies like MySugr (Diabetes
management app) claims to have almost 150,000 users of only one of their apps.

5.2. COMPARISON OF TARGET GROUPS, REGIONAL FOCUS AND GOVERNANCE
MODELS
Compared to the mentioned best practice examples in the report, Pharma companies have a higher share
of apps which target professional users (e.g. doctors, nurses, and teachers). Despite this, at least 1/3 of
each of the 12 leading Pharma companies’ app portfolios still target private users. Namely, Bayer
Healthcare (58%), GSK (83%), Johnson&Johnson (86%), Novartis (56%), Pfizer (73%) and Sanofi (67%)
reach out to groups like patients with chronic diseases, health conscious and fitness oriented people,
pregnant women, parents or kids rather than doctors, hospitals or insurance companies (professional
users).
Also the “professional” part of the Pharma’s app portfolio does not target only one, very specific group,
but rather all doctors e.g. by providing reference for all kinds of symptoms, medicine or therapies.
This means that the app portfolios of Pharma companies can be compared with the rest of the mHealth
app market and that the structure of the portfolio is not an argument for a lower reach and performance.

Compared to the vast majority mHealth app publishers, Pharma companies seem to find it more
challenging to align their mHealth app know-how. One indicator of this is the number of app publishers a
company uses for their app portfolio. AstraZeneca (11), Johnson&Johnson (17) and Novartis (17) are all
supported by more than 10 different app publishing entities. This is not a hard evidence for a non-
coordinated app publishing, but a large number of publishers means that communication and re-use of
existing concepts, codes in other regions is getting more difficult.

Besides this, all Pharma companies in this benchmarking analysis are global players, but
their app activities are present in only a few countries. 10 out of 12 companies
generate more than 50% of their total app downloads in only 3 countries. The main app
market for 6 out of 12 companies is the USA followed by France, Germany and the UK.
China and other Asian app markets do not constitute a significant share of downloads
for the Pharma app portfolios.
Novartis is the company which generates the biggest share of downloads (57%) outside
of its top 3 markets. On the other hand, Abbott/Abbvie has the largest share of its
downloads generated in one, single market: 88% of all downloads come from the US.

The comparison of the Pharma companies’ app portfolios shows a significant effort in terms of the
number of apps published. To a large extent, they all target private users. In terms of performance,
neither of the leading Pharma companies has reached a footprint in the app market which would be
comparably equal to the position they have in the traditional healthcare market.
Sanofi, Johnson&Johnson and Merck have gained the highest reach in the app market with their app
portfolios boasting more than 1 million downloads and 150,000 active users. Most of the downloads and
users come from just one or two apps, making it difficult to justify the effort that has been put into the
rest of the app portfolio and this is the case not only for these, above mentioned three leaders.

6. PHARMA COMPANIES’ APP PROFILES This chapter summarizes the app activities of the Pharma companies included in the report.
6.1. ABBOTT/ABBVIE APP PROFILE
Abbott/Abbvie concentrates on apps that address professional users. 68% of published apps target HCPs.
All apps are published under Abbott and Abbvie brand. Abbot has listed 45 apps on the Apple App and
Google Play Store. Their apps have been downloaded 341,000 times. Total active user number of all the
apps is 55,000, which gives an average of a little more than 1,200 active users per app.
The perceived governance model for their app business is closed and in-house. Of all the Pharma
companies in consideration, Abbott uses one of the most centralized publisher approach to publishing,
with only two publishing entities engaged. They have started to follow a more open and partnership-
based approach. This includes first developer day events and a high profile partnership with Calico, a
Google start-up company that focuses on health and well-being, in particular the challenge of aging and
associated diseases.

AstraZeneca focuses on apps that target HCPs. Their reach and success in the app ecosystem is very
limited with only ten thousand downloads and thousands of monthly active users. Their app portfolio
includes 60 apps (iOS and Android versions of the same unique app are counted as separate apps) with a
clear preference for iOS.
AstraZeneca uses a decentralized app governance model. They have 11 different app publishers listed on
the two major app platforms. In June 2014, they have announced the first partnership with Exco in Touch
a mHealth data capture and clinical trial mobile solution provider.

Bayer Healthcare has been one of the most active (in terms of number of apps) publisher among the
leading Pharma companies. They have almost 100 apps available for iOS or Android. The majority (60%) of
their apps target private users. Despite the efforts, they have not been able to attract a significant
number of downloads (587,000) or active users (91,000).
They recently shifted their focus from in-house app publishing to a more open approach using an
incubator model (“Grants4Apps”) to attract innovative mHealth app start-ups.

6.5. GLAXOSMITHKLINE APP PROFILE
GSK has an app portfolio that consists of 57 iOS and 47 Android apps. They mainly target private users
(78%), but have been only succeeded in generating a little more than 700,000 downloads. Total active
user base for all their 104 apps is 137,000 (average 1,300 users per app). GSK is one of the few Pharma
companies with a significantly higher download share of Android rather than iOS apps.
mHealth at GSK is also engaged in various projects in developing regions. They often develop these
projects in partnership with mobile operators like Vodafone.
GSK’s smartphone apps have been published centrally (by 2 publishing entities) and mainly internally.
With their “Open Wants” platform GSK has recently started to put more emphasis on attracting app
developers and to open up their boundaries.

GSK has put much importance on apps for kids as compared to the other Pharma app publishers. Most
apps for kids are in form of the localized version of one tooth brush app “Aquafresh”, which is the most
used app published by GSK. In some countries GlaxoSmithKline has partnered with some of the biggest
brands to promote the app. In Germany, for example, the app “Aquafresh Brush Time“ is called “ODOL-
MED3 PUTZZEIT“ (Odol being a mouthwash). In the Czech Republic the app is called “Odol Čas čištění“. In
the UK/ Australia GSK has teamed up with Macleans (toothpaste company) and published the app under

Johnson&Johnson has a clear focus on private app users. They use 17 different app publisher entities for
their app business. This also includes apps published by its daughter company Wellness &Prevention
which is the publisher of the most successful Johnson&Johnson app: 7 Minutes Workout, a health
improvement app. With almost 750,000 downloads and 1,500 ratings (US Apple App Store, average 4.5) in
10-month period, it is one of the best performing apps targeting private users.
Their app portfolio consists of 42 iOS and 21 Android apps. J&J’s average number of downloads is higher
than Pharma’s average, but these mainly come from one app. The rest of the app portfolio has not
generated a notable number of downloads.

Merck’s app portfolio addresses mainly HCP users (69%). Merck is concentrating their app publishing
activities in one publisher entity. With 26 app Merck does not belong to the most active app publishers
but because of the success of the Merck PT app they belong to the more successful app publishers. The
app portfolio has reached more than 1.2 m downloads with 97% of these downloads coming from the top
app. All apps count 207,000 users. This also explains the high average number of downloads. The Merck
app portfolio is basically a one app initiative.

6.8. MSD APP PROFILE
MSD is an active app publisher with 74 apps listed on Apple App Store and Google Play. The majority
(61%) of their apps are developed for iPhones and iPads. The app portfolio equally targets professional
and private users. Within the benchmarking group, the average performance of the apps is one of the
lowest.
In addition to their own app publishing activities, MSD has been actively building up partnerships to
mobilize their content e.g. Skyscape app “The Merck Manual of Diagnosis and Therapy”. They have also
set up a digital health fund (GHI) to invest in innovative healthcare technology (not only mHealth). The
GHI fund portfolio includes companies like Welldoc and Preventive which offer medical condition
management solutions for diabetics and cardio patients.

6.9. NOVARTIS APP PROFILE
Novartis mainly concentrates on publishing iOS apps. Their app portfolio consists of 52 iOS and 18
Android apps. Despite their large number of apps and a private target share of 55%, Novartis has not been
able to generate a substantial app download number or active user base. The average all-time download
number is comparably low, ranging between 1,700 and 3,000 downloads per app. In contrary to their
competitors, they do not have an app which significantly outperforms the rest of the app portfolio.
Novartis has the most decentralized app publishing strategy of all Pharma companies in the report (17
different publisher entities). They recently started to establish partnerships with technology players.
These partnerships include: Proteus Digital Health (health technology services provider), Sense.ly
(telemedicine, Oct. 2013), TicTrac (multiple sclerosis, May, 2014) and with Google regarding its smart

Among the leading Pharma companies, Sanofi is one of the most active app publishers. Sanofi’s app
portfolio comprises of 64 iOS and 43 Android apps. Their apps mainly target private users (69%). Two
thirds of all-time downloads originate from iOS users.
The company has hired 4 publishing entities to develop their app portfolio. Sanofi has also made some
effort to establish digital (not only app publishing) competencies within the global and local organizations.
They have established digital country heads, digital classes at the Sanofi university and hired digital
experts from other industries.

The app portfolio of Sanofi Aventis is as broad as the portfolio of the other Pharma companies. The
company made the biggest impact with apps that support diabetics. In 2010 Sanofi closed an agreement
with AgaMatrix for the development, supply and commercialization of blood glucose monitoring (BGM)
solutions. In 2012 iBG Star was one of the first apps to track glucose levels with the help of a FDA cleared
sensor that can be plugged into the iPhone. The app created much market awareness at the time but lost
attention in the following years. The second app that addresses diabetics is GoMeals, an app that helps
diabetics with their diet and tracks their activity levels. Both apps account for more than 2/3 of the
Sanofi’s total app user base.

7. OPTIONS FOR A BETTER APP PORTFOLIO PERFORMANCE
The top 12 Pharma companies have published 736 apps (497 unique apps) on the Apple App Store and
Google Play. They have made a substantial effort to reaching out to both private users and, to a lesser
degree, to HCPs. Since their first apps were launched in 2008, Pharma companies have failed to have a
significant impact on the mHealth app market. Only a handful of apps have generated a substantial user
numbers among their target groups.
There are some conclusions that can be drawn from the results of the benchmarking for;
1. Why are Pharma companies not successful? and 2. What are the options Pharma has to improve its market position?
The analysis of the 736 apps showed that apart from the some improvement potential in the usability,
design and functionality, there are more systematic areas with scope for improvements. In order to
improve their app performance, Pharma companies should take measures related to the following areas.
Apply design guidelines and internal app referencing: So far Pharma companies have not used the potentials of cross-referencing between their apps. They also do not use common style guides for their app portfolio which could improve their app visibility as well as strengthen corporate identity in the app market. This is becoming even more than before, given that more than 100,000 mHealth apps are listed in the two major app stores.
Concentrate app activities on where the market demand is: The app portfolio is built around the core products of the Pharma companies and not around the actual market demand. For example, if a company is specializes in the treatment of hematological diseases, the app portfolio reflects that. Apps in this case would provide references to latest research, support diagnosis and facilitate information exchange with/between the experts. There exists an app market for such products, but there are other segments e.g. health tracking, weight loss, fitness or diabetes condition management, which attract more users. An app category strategy should concentrate on the current and expected market areas with high user demand.
Focus on mHealth data collection: The mHealth app market is still at a very early stage and successful business models might change over the next 5 years. But it is most likely that the user generated health data will become a major asset in the future. App portfolios that concentrate on the collection of e.g. the activity levels, calories burned, glucose levels or blood pressures of millions of users will have all options at hand to have a profitable app business in the future. For instance, they could transfer the knowledge to help the industry develop better medications, therapies and medical devices.
Open up APIs7 to build a connected app portfolio: There is a growing number of apps which connect to other mHealth apps to read out user generated activity and medical data. By doing so such apps have become more valuable (e.g. Loose it, Fitbit, EveryMove) for their users as well as for their creators. With iOS 8, Apple has created a platform to allow for an exchange of mHealth data between apps in a very efficient way. With only a few exceptions, Pharma apps do not generally make use of third party health and/or fitness data. For medical condition management apps (Pharma app portfolio share = 20%), this will be a must in the future.
Strengthen Android app presence and make use of cross-platform app development tools: Today only 45% of the leading Pharma companies’ apps are available both on iOS and Android. Admittedly, it might not make sense to develop any of the already existing “long-tail” apps for Android, but future app developments should target both platforms right from the beginning. Otherwise, large market segments will not be reached. A multi-platform app development

approach should always include the usage of cross-platform app development tools to limit development cost and time. See “Cross-Platform Tool Benchmarking 2014 Report- find the right tool for any app project8". Given that such tools offer significant time savings, they should constitute an important building block of any new app development project.
Become truly global: Only half (51%) of the Pharma app publishing entities launch their apps in more than 3 countries. The other half is active in just 1-3 countries. This means that a great share of apps are not available for download globally, which limits the chances of having a significant impact on the app market. One of the major advantages of the app ecosystem Apple has created in 2008 was that reach out to a global audience. Top Pharma app publishers do not make use of this opportunity.
Align app publishing governance and competencies: There might be internal or legal reasons for having up to 17 different publisher entities, but in order to become a major player in the mHealth app economy this is certainly not the best governance model. App ideation, concept writing, selection of the best agency, app development, update management and marketing have become demanding tasks which require expert skills. Pharma companies compete with 100% dedicated mHealth app publishers who align all their competencies to build and market mHealth apps best in class. This is not the case within most of the Pharma companies. This creates a significant competitive disadvantage Pharma needs to face. In order to compete in the Health app market Pharma companies must align their app competencies globally. This would be also the prerequisite for all the options mentioned above.
Besides the need for improving their current mHealth app publishing business, Pharma companies should
start a discussion around its role in the mHealth app publishing i.e. which steps suit best to their
competencies and reflects market dynamics.
The above analysis shows that since the start of the app market, Pharma companies have not yet proven
to be successful mHealth app publishers. They will be challenged to become better in selecting the right
app ideas, designing the business models, making better reuse of existing concepts and codes or in
leveraging existing marketing channels to promote their apps. However, it is not clear whether it will be
enough to compete against the hundred thousands of mHealth app developers who are 100% dedicated
or tech giants like Apple, Google or Samsung that have recently entered into the mHealth app market.
Would it be a good investment to develop and publish the next 700+ apps the next five years? Which
other role/s could a Pharma company play in the app economy? There are many potential roles and the
following ones should be used as a starting point for the discussion within a company.
1. App publisher: This is the current role and continuing to play that role would require Pharma to restructure its current mHealth business.
2. Investor: For some companies it might be more appropriate to open up a fund which invests money and other resources into promising or already successful mHealth apps. This role would require an excellent screening and evaluation capability. VCs and incubators that concentrate on apps are able to work off a pipeline of 1,000 mHealth apps per year.
3. App aggregator (app store): Lack of visibility in the congested app stores creates a need for specialized mHealth app stores. Pharma companies could pre-select mHealth apps which fit to their core products or fulfil current market demands. It does not matter which compliance app is downloaded as long it increases the adherence of the users and allows more users to find the right app for their needs. The role of a Pharma company would be to manage and promote the app store among app developers as well as to HCPs and patients.
4. Data provider /API manager: There will be hundreds of mHealth apps capturing millions of vital data metrics from their users every day. Most of it will be stored on “walled garden” servers with no link to the healthcare systems in doctor’s offices or hospitals. There are already companies

which try to bridge the gap between the app economy and the healthcare system, but the question is if a Pharma company would be able to jump into this very dynamic market with all their financial strength and the immense value it could extract for itself from the data analysis to create better products.
A prerequisite for all of the last three roles is an excellent link into the mHealth app developer community
and a good understanding of their needs and concerns. Some of the Pharma companies have started to
do occasional app developer events like Hackathons or mHealth competitions, but this is not enough to fill
the pipeline of e.g. 1,000 apps per year, to get e.g. the 20 best mHealth apps for cancer diagnosis, asthma
management or eyesight testing onto the own app store or understand what value app developers would
expect from allowing the mHealth data aggregator to access their data.
Pharma companies need to treat app developers as their customers who must to be understood,
segmented and managed the same way Pharma companies do that today with e.g. doctors.
What this all means is that before going forward in the same way as the last five years, Pharma companies
should rethink their mHealth app strategy. Otherwise they will remain an industry with low impact on the
mHealth app market. This is not a serious threat to their core business today, but in five years it might be.
