    It is a device for performing shoulder and elbowmaintenance exercises in the event of fracture outcomes,less serious impingement syndromes and rigidity causedby arthrosis. It consists of a wall-mounted metal framefeaturing a height-adjustable wheel depending onpatients size or the exercise to perform. Te instrument,derived from the Lapidaris wheel,is tted with ahandle and knob to perform a wider range of exercises.Patients can work both sitting and standing, start withfree exercise and then, by means of a clutch, graduallyapply a wider resistance. By using this instrument, thetherapist can avoid the most repetitive work while thecooperative patient performs the activity in a totally safe

    way. Te Rota comes complete with a manual and avideotape, suggesting a set of exercises. A set of accessories can be ordered (code AC0008) toincrease the number of exercises offered by this device.

    01348 ROTA MOBILE

    Tis instrument is for shoulder and elbow rehabilitation in cases of stiffness, impingementsyndrome and pre- and post-surgery treatment. It has a metal frame and a height-adjustable wto meet the patients size or depending on the exercise to perform. It can be easily moved by aoperator.It comes complete with a set of accessories, including: a handle, a knob, a forearmsupport, a ywheel mass, an elbow rest support and a front handgrip.Mainly used to perform active exercises, it can also be used to do passive/self-passivemobilization and proprioceptive exercises. Patients can start with free exercise and then, bymeans of a clutch, gradually apply a wider resistance.By using this instrument, the therapist can avoid the most repetitive work: thephysiotherapist programs the sequence and intensity of the exercises, while the patient cancheck the range of motion reached at each repetition and is therefore motivated to commitoneself.Te recovery of range of motion can be monitored with a quick and extremely reliable assessm

    1) When the handle reaches the wheeltop, the shoulder reaches its maximumabduction: therefore, the previousphase consists in the shoulderabduction (north-west quadrant).

    2) Going on with the rotating movementfrom the wheel top, the handle comesto its farthest point from the user;the shoulder bends forward and theelbow extends. Tis quarter of circlecorresponds to the elbow extension(north-east quadrant).

    3) From that distance, the handle comesto the lowest point of the wheel;the elbow starts bending while theshoulder decreases its exion; this isthe phase ofelbow exion(south-east quadrant).

    4) From the lowest point of the wheel,the handle reaches the closest point tothe user; the elbow bends further, theshoulder extends or moves in a waysimilar to the extension: this is thephase ofshoulder extension(south-west quadrant).

    External rotation and abductioncombined in several degrees. Passiveexercises: they can replace the electricmobilizer in post-surgery treatment. Active exercises: external rotatorstrengthening can be achieved duringan abduction movement with minimumstimulation of the deltoid muscle.

    Active Therapy

    The training equipment called M.R.S. (Multi-functional Rehabilitation Station for Continued IntensiveRehabilitation, or Stazione Riabilitativa Polifunzionale R.I.C., ) has been specifically devised for anactive training (muscle recruitment and strengthening, balance, coordination, proprioception...)in R.I.C. (Continued Intensive Rehabilitation) exercises. The M.R.S. not only makes the Therapists

    job more practical, easier, smoother and less taxing, but it also allows the Patient to assume variouspostures, perform exercises using weights and balance weights, and with the option of axialsuspension to help movements and achieve the widest possible joint ROM.

    Tis motor rehabilitation is intendedfor the neurological pathologies.In particular, it applies to spinalcord injuries in the post-acute andstabilised stage, whatever the degreeand level of the injury itself (...recovery after a spinal cord injury is a

    rule, not an exception... Prof. Wise Young).Te Continued IntensiveRehabilitation method originated

    from the Russian school ofrehabilitation - which is mainlybased on the mechanical aspect ofmobility - but subsequently movedaway from it. Long studies, carefulobservation and direct experiencewith Patients helped realise that

    the rehabilitation result cannot beachieved with motor training only.Te performances obtained are theresult of several factors that can be

    found in the complex physiological,pathological and emotionalmechanisms of the sufferers of suchpathology. Only by respecting thesecharacteristics and the physiologicalposture can we achieve appreciableresults that last over time.

    Tis method is developed in such away that the various rehabilitationtreatments, while still following a

    sequential protocol, never appear tobe strict and/or xed. Every therapyprogramme has to be tailored toeach person, who is seen as a whole(pathology, post-traumatic issues,motivation, expectations, type of job,inter-personal issues behavioural,

    emotional, and so on). In additionto their individual challenges,all Patients also have enormousfunctional potentials, which can

    Te Continued IntensiveRehabilitation consists of three stepsexecuted during the same trainingday:Osteo-articular Kinesytherapy: joint mobilisation, stretching (for30-60 minutes depending on thetype and site of the spinal cordinjury, tendon retraction andspasticity); Motor Rehabilitation active exercises (about 150 exercisesalways performed in an activeway, with all the variations neededto personalise the rehabilitationprogramme). Te Patient is treatedby a Terapist on a one-to-onebasis for about 3 hours Walking(between 5 and 30 minutes);Physiotherapy massage (for 30-60minutes), electrostimulations (10

    minutes for each muscle zone),electromassage (15 minutes forlower limbs), magnetotherapy (40minutes).Te R.I.C.s main objective is toincrease the Patients functionalcapabilities to the maximum, ifpossible even if they are walkingwith the aid of small splints belowthe knee and at full load (Patientsweight on lower limbs). By aimingat this objective, the Patient willalso achieve other benets such

    as: remarkable improvement intheir quality of life, prevention ofhypokinetic pathologies and ofthe most frequent complicationsrelated to the current pathology(bedsores, spontaneous fractures,orthostatic hypotension, tendonretractions, breathing problems,oedema, depressive syndromes, etc.),maintaining muscle tone and massand reduction of spasticity (in somecases, drug treatment can be reducedor stopped).

    Active Therapy

    It is advisable to plan the durationof the rehabilitation programmeaccording to a continued andprogressive format in order to try toachieve the desired objectives.

    Te training equipment (R.I.C.Method Multi-function Station)has been devised to performthe active exercises under theR.I.C. (muscle recruitment andstrengthening, balance, coordination,proprioception, and so on). Tis:1) makes the Terapists job morepractical, easier, smoother and lesstaxing; 2) allows the Patient toassume various postures, performexercises using weights and balanceweights, and with the option of axial

    EXAMPLE OF ORGANISATIONALPATTERN(used at the Centro Giusti inFlorence, RIC method referencecentre)

    - Outpatient R.I.C. therapy at theCentro Giusti for 15 weeks (thePatients own Terapist is requiredto attend during the last 3 weeksin order to learn the R.I.C. to beperformed during home care);- Home R.I.C. therapy for about3/5 months (the rehabilitationprogramme is handed over to thePatient on discharge and will befollowed daily with the Terapisttrained at the Centro Giusti andwith the support of all the necessary

    R.I.C. material);- Outpatient R.I.C. therapy atthe Centro Giusti for 3/5 weeks

    (to check on Patient and R.I.C.therapy and to advance the R.I.C.rehabilitation programme thehome Terapist is required to attendduring the last week);

    - Home R.I.C. therapy for about3/5 months (the rehabilitationprogramme is handed over to thePatient on discharge and will befollowed daily with the Terapisttrained at the Centro Giusti andwith the support of all the necessaryR.I.C. material);- Outpatient R.I.C. therapy atthe Centro Giusti for 3/5 weeks (to check on Patient and R.I.C.therapy and to advance the R.I.C.rehabilitation programme the

    The Fisiotek 2000 equipment line is operated by a direct-current motor controlledby a micro-chip card. Functions programmable via a fixed keyboard. A movable keyboard allows patients to start and stop movementby pressing the relevant keys.

    Te board can be attached tohinges, half-spheres or sphericalttings through a series of holesand magnetized pins, which makesit possible to use it for a variety ofexercises.

    (See picture above) Tis is aneffective recognition exercise,requiring the user to transfer bodyweight and to recognize the differentpositions of the hinge pivoting onone pin connected to the board. Byincreasing the accuracy of the inputs,it is possible to obtain more andmore accurate results.

    Kinesthetic recognition tasks canalso be assigned making use ofa series of numbered cylindersin different heights. Te level ofdiffi culty of the exercise can beincreased by positioning springsunder the board. In this way, thesubject can be engaged in balancingtasks and in the recognition ofresistive forces, where he / she isasked to process and become awareof sensory afferent impulses.

    Laser offers a visual feedback. Tisresult is enhanced by the circlingof the board connected to the half-sphere.

    (See picture above) Tis exercisehelps teach how to assume anupright position when seated. Tepatient is asked to reach differentpositions on the work surface,keeping the laser pointed at theidentied target, while performingbimanual tasks involving trunkstretching movements.

    ANTENNAS LASER POINTERTe antenna and laser point allowviewing pelvis and trunk behaviouron the different planes andstructuring positional recognitionexercises within tasks requiringweight transference.

    INCLINOMETERTe inclinometer can be positionedon all body areas and providesaccurate information in case therange xed on the frontal andsagittal planes is exceeded. It ispossible to set the desired degrees of

    hemipelvis or left rendelenburg.In this case, the subject is asked notto increase his / her dorsal kyphosis.Exceeding the xed range activatesthe acoustic feedback (FB).

    Useful for controlling anycompensations or failedstabilizations of the trunk in musclestrengthening exercises, makingthese more accurate and thereforemore effective.

    GONIOMETERTis instrument allows controllingthe relative movement betweentwo segments. In this case also, it isvery simple to set the articular levelbeyond which an acoustic signal isactivated to indicate either an error(negative FB) or that the task hasbeen successfully completed (positiveFB). In the exercise suggested, theinstrument warns of an undesiredhyperextension.

    Te peculiarity of this instrumentlies in the fact that we can interveneon body areas that are usuallyuncontrollable, giving importantinformation about the ways ofperforming the gesture. In thiscase (see picture) the attention isfocused on the failed xation of thescapulothoracic joint in upper limbabduction tasks. An alternative to the luminous acoustic feedback is an instrumentthat generates a cutaneous, vibratorysignal.Te device can be applied whenusing either the goniometer or theinclinometer. Tese allow activatingthe vibratory feedback directly onthe muscle to be recruited, uponreaching the desired range ofmovement.Te goniometer can be attached

    to the ankles by means of specialsupports and be used to intervene onthe length and symmetry of steps.

    A goniometric biofeedback device is a tool which canhave several interesting applicationsin the rehabilitation field. Thoughit does not replace any technique,it can be a complement to therehabilitators work, facilitate it and be useful for thepatient in some situations and in given clinical stages.

    06856GONIOMETRIC BIOFEEDBACK A goniometricbiofeedback deviceisa signal transducer for joint anglesduring voluntary movement andrange of motion adjustment. It can replacetheelectromyographic biofeedback ,when it is impossible or hard tomonitor or isolate a muscle.Te international literature has shownstudies on goniometricbiofeedbackeffectiveness in controlling kneehyperextension of hemiplegic patients,

    in postural head control training and joint recovery of elbow (g. 1), wrist(g. 2), foot (g. 3) and knee (g. 4).Te device is lightweight (500 kg),easy-to-use, no potentiometer andelectrical box needed.It comprises two plastic (Plexiglas ),mutually constrained, revolving arms.One arm has a magnet, the other hasa handle with a magnetic detector thatcan trigger an audio alarm. Te angleposition to be detected by an acousticsignal is determined by the handle

    magnetic detector. Tis is always ononce it overlaps the magnet on theother arm.Te goniometric devices arms aremounted on the body segments withVelcro bands and must be placedsidewise to the mechanical axes of thesame segments. Te device rotationcentre matches the joint rotationcentre.

    When the patient is

    familiar with the tooland the required task,the same tool can beused at home without theoperators supervision.
