SPEAKER PROFILEProf Syngcuk Kim is the Louis I. Grossman Professor and Chairman of the Department of Endodontics at the School of Dental Medicine, University of Pennsylvania. He earned his undergraduate and his dental degrees from Columbia University in New York. Prof Kim also earned his Ph.D. in circulatory physiology at the Medical School of Columbia University, Department of Physiology.

Prof Kim is well known and respected for his research in dental pulp physiology and microcirculation and the AAE has awarded him the Louis I. Grossman Award for outstanding research.

Upon joining the Dental School at the University of Pennsylvania in 1992 as Chairman of the Department of Endodontics, Prof Kim not only brought the department into the “New Age” of the Microscope, but also established the Microscope Training Center at the school. This Center is the first university-based microscope training program in the world that teaches microendodontics and microsurgery. Since its inception,

leaders in endodontic education and clinical practitioners from the USA, South America, Europe, Asia and Australia have been trained at the Center.

In 2010 Prof Kim was appointed by the Dean to the role of Associate Dean of Global Affairs at the School of Dental Medicine. Serving in this capacity, Prof Kim travels extensively creating relationships with Dental Schools throughout Europe and Asia, South America, etc. with the goal of promoting academic and scholarly exchange between the teaching institutions.

Extensive speaking engagements take Prof Kim to universities, conferences, study clubs and professional associations in the U.S. and abroad. In addition to over a hundred articles in peer reviewed journals, Prof Kim is the primary author of the book titled “Color Atlas of Microsurgery in Endodontics”.

SYNOPSISPenn Endo Philosophy

Basic science: Pulpal inflammation, which leads to necrosis of the pulp, is the main topic of this portion of the lecture. The dental pulp consists of numerous connective tissues, but the main components are sensory fibers and microvasculatures. Interaction of these two elements in health and disease is the basis of pulpal inflammation. Due to the very close interaction of these two elements, via many neuropeptides as the mediators such as Substance P (SP) and CGRP, pulpal inflammation is now called neurogenic inflammation. In this process the inflammation begins with excitation of sensory “C” fibers which release CGRP and SP then bind with microvascular receptors and in turn cause vasodilation and edema formation. This intricate relationship and inflammatory step will be discussed with clinical implication in kind.

Clinical Science: Each phase of the endodontic procedure has been changed in the last decade, taking advantage of new technologies and materials. These include the use of the microscope, ultrasonic instruments, rotary NiTi files, electronic apex locator, digital radiograph, resin bonded root canal filling material, resilon, cordless obturation system and MTA (mineral trioxide aggregates). These changes allow dentists to perform easier and more precise root canal therapy, thus providing higher success rates. In apical surgery the changes are even more fundamentally drastic. This procedure is now called Endodontic Microsurgery in which no more amalgam and straight hand-pieces with burs are used. In place of the old tools and materials we now use specially designed ultrasonic instruments and MTA under the microscope. Now microsurgery provides over 90% success rate over a long time of failed root canal therapy. The above changes are the crux of modern endodontics and provide high success rates, allowing patients to save their teeth. In the second portion, the new controversy regarding Implants and/or Endodontics will be discussed. Implants are the most important advancement in clinical dentistry in recent years. However, its use has to be judicious and implemented when indicated. In this presentation the success rate of implants will be critically examined using literature and an objective comparison with endodontic treatment will be made. Endodontic treatment is here to save the tooth and implant treatment is here to replace it when truly indicated.
