    INCIDENT REPORT AT SITE ON 28/04/2013

    On 28/04/2013 at around 4:00 pm there was a tornado followed by heavy rain & hail storm. As

    a result the following incidents have taken place at the CPP project site.

    1.) Toppling of 75 TON TATA-955 mobile crane belonging to Hazemag /Giriconstruction.

    The mobile crane had just been shifted from TG Building area to CHP area &was kept

    near TP-1,

    With the complete booms up to 150 feet fitted in position.

    With the storm blowing from north to south, the crane has toppled from the

    front to the rear position with the boom bent from the base, and toppled to the rear, as

    shown in the attached photograph.

    No other damage has taken place with this incident.

    OBSERVATION:-

    Normally when the mobile crane in idle, the boom of 150 feet should be lowered fully to

    avoid the risk of overturning. However in this case the boom was in full mast position ,

    while the incident took place

    2.)Columns of transfer tower-2 of CHP have got twisted and bent.As shown in the photographs, all the columns of TT-2 have got twisted & bent, with the

    severity more in the staircase position. The columns of the main structure are bent from

    the middle up to the top.

    Details of damages

    1.) All the columns at the northern side are bent from middle position up to top2.) The columns (4nos) of the staircase have got twisted at the bottom3.) Foundation of A1 column three foundation bolts sheared and the civil foundation

    damaged with cracks.

    4.) Three to four nos bracings have fallen with the bolts sheared ,and in one case thebracing itself was sheared .this is in row B ,between column B2&B3

    OBSERVATIONS:-

    1.) The damage has taken place due to the a faulty erection work in addition toinadequate bolting &welding which therefore could not take the wind pressure.this has swayed the structure &twisted the base out of four nos of guy ropes

    provided, two nos were found broken during the storm

    2.) THE TIE BEAMS& vertical bracings were erected only up to a level of 36.7 M. THECOLUMNS WERE erected up to full level of 46.127M &no tie beams or bracings were

    provided from 36.7M UP TO 46.127M &therefore this position of the columns were loose

    without tie beams ,bracings, or top girder. The contractor have posto the position of the

    work due to inadequate manpower & crane, though he was repeatedly instructed to

    complete this wok by lifting members manually with rope &pulley

    3.) also ,in all the tie beams, & bracings the members were only bolted & there was nowelding. The contractor in total was having only three welders to complete the enormous

    welding work of CHP AND TG building .despite of repeatedly informing Hari machines

    ,Hazemag & the contractor till date has not mobilized a single extra welder since the last

    3 weeks.

    4.) As the structure is to the highest elevation at site (46.12M) after chimney,&the job beingincomplete ,without bracing &TIE beams at the topmost critical positions,&the bracings

    also only being bolted without full welding ,all these together have resulted to the inability

    of the structure to counter the wind pressure.

    There was no other damage or accident linked with this incident .

    SUGGESTIONS:-

    1.) As of now the structure is in a tilted position & with a potential danger offurther tilting which could cause damage to boiler or TG Building . hence the

    structure is to be dismantled on an emergency basis by hiring additional crane.

    2.) Hazemags team should visit site immediately & execute the dismantling workon a war tooting basis to tide over the situation .

    3.) HaZEMAG TO recheck the design by an expert to ensure that the structure areadequately strengthened.

    4.) Hazemag to check the quality of steel used for fabricating the structures
