In consideration of being allowed to participate in any way in the Ice-Plex hockey tournament program, its related events and activities, I the undersigned, acknowledge, appreciate and agree that: 1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, 2. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation; and, 3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the Ice-Plex immediately; and, 4. 2016 THANKSGIVING EXTRAVAGANZA SKILLS COMPETITION

Each team will need to submit four players for the three events

Submission Deadline: 10/31/2016 PRIZES WILL BE AWARDED!

Target Shooting: Targets will be attached to the net and each player will have 10 pucks to shoot as many as possible. The athletes will have a two-minute time limit to hit all targets. Speed: Each athlete will skate one lap around the ice as fast as they can. Holding their sticks is permitted. Break Away: Two players will participate, a goalie and a shooter. We will be keeping stats on goalie saves and how many goals are made by the shooter. The shooter will be given 5 pucks and timed in case of a tie. PRIZES!!: Awards will be given to the individual winner in each division for each event. Awards will also be given to the winning team in each division! 2016 THANKSGIVING EXTRAVAGANZA Skills Competition Rules

Target Shooting Rules: Each player will have 10 pucks. They will try to hit the targets on the net. They will receive 1 point for hitting the bottom targets and 2 points for hitting the top targets. If they hit the target in between the legs of the goalie they will receive 5 points. The players will be standing at the hash marks to shoot the pucks. Wait for the announcer to call on the skaters before sending them onto the ice. At the end of the competition, winners will be announced. Speed Skating Rules: Each player will be assigned to one side of the rink on the center red line. They are racing the clock and NOT each other. They must skate counter-clockwise and they may also use their sticks. When they hear the whistle blow, they must skate as fast as they can around the ice once. They will be timed, and 3 points will be awarded to the skater coming in first, 2 points to second, 1 point to third. Don’t send the skaters on the ice until the announcer calls for them. At the end of the competition, the winners will be announced. Break Away Rules: Each player will have the chance to shoot once on every goalie. Each player will shoot one puck and then skate back to the center ice where the second player will shoot their first puck. 1 point will be awarded to the player for each goal scored and 1 point will be awarded to the goalie for each save made. If a player misses the net completely, the goalie will be awarded the point. At the end of the competition, the winners will be announced.

17TH ANNUAL THANKSGIVING EXTRAVAGANZA ICE HOCKEY RULES

17TH Annual Thanksgiving is governed by rules of USA Hockey. In addition, the following rules apply:

1. All teams must show proof of USA Hockey team registration, team roster, and release forms for each player prior to their first game.

2. All teams must be available to start their games up to thirty (30) minutes prior to scheduled start time. 3. ALL TEAMS must provide a volunteer for the penalty box for every game. 4. All teams will play 15 minute stop time periods except Mites. All games are 75 minute curfew games

except Peewee AAA games, which will be 90 minute curfew. Running time will be implemented if the game is in danger of exceeding curfew and will be evaluated by the Tournament Director after the 3rd period. 5 minute warm up time. No overtime shall be permitted until championship games. See Mite ADM Cross-Ice Rules.

5. Each team is allowed one 30-second time out, except Mites. 6. If a five (5) goal differential exists at any time (i.e. 5-0; 6-1 etc.), running time will commence for the

duration of the game. Despite the occurrence of running time, penalties will remain the duration prescribed by USAH rules. If the goal differential comes within 2 goals (i.e. 5-3; 6-4 etc.), stop time will resume. EXCLUDES MITES.

7. For Preliminary play, points will be awarded in the following manner: 0 points for a loss, 1 point for a tie, and 2 points for a win.

8. The following tie-breakers formula will be used to determine the order of finish after preliminary play: a. Total Points b. Head to Head record between teams with equal points (not relevant in a 3-way tie) c. Most wins overall d. Fewest goals allowed overall e. Fewest penalty minutes overall f. Most goals scored, maximum of 8 per game. EXCLUDES MITES. g. Coin Toss

9. Forfeits: If a team forfeits one or more games in the tournament, all games it has played shall be recorded as 1-0 victories for the non-offending team(s).

10. All tournament “Championship” games that end regulation in a tie shall use the following process to determine a winner:

a. Five (5) Minute Run-Time “Sudden Death” tiebreaker-first to score wins b. If the tie still exists after the “Sudden Death” tiebreaker, a shootout to determine a winner will go

into effect. Shootouts will follow USA Hockey rules, beginning with a 3 person shootout. 11. Protests: No protests shall be made on the playing rules. Otherwise, protests must e registered on the

back of the signed score sheets and must contain all known facts. Protests not written on the back of the score sheet before final signing will not be considered. Tournament Director shall act expeditiously on all properly submitted protests. Tournament Director’s decision shall be final. A protest fee of $50 will be implemented.

12. All penalties and suspensions made by the referees/game officials are final. (updated 6/24/16)
