Purpose of PHR

• The goal of the PHR is to improve the health of IHS patients through:– improved patient/provider collaboration– patient self-management– increased access to health information

• Research shows that when consumers actively participate in their own care, their outcomes are better. – Patients twice as likely to receive suggested

routing screening– Improves health communication– Patients perception of quality increases– Decreased no show rates– Time and cost saving for providers

Unlink PHR from Medical Record

• Triggers– PHR Account linked to wrong medical record– Patient no longer wants access– Other?

PHR Account Linked To Wrong Record

• The PHR Account Holder is viewing somebody else’s medical information

• Is a breach of privacy and security– With 30 minutes of discovery must file an

incident report– Implement breach notification procedures– Investigation of this incident is required



Recover Credentials for Patient

• Verify Patient’s Identity

• Find Patient’s PHR Account

• Re-set Patient’s Password

• Inform Patient of New Password

• Instruct Patient to Change Password– Patient Log-In– Go to Profile Tab
