Dear Resident,

Everybody needs better information on the performance of local government:

city managers need to know where to improve services, and

residents need to know how well their taxes are spent.

As a result, my office annually collects information on how well our City departments are doing and prepares a report on the results. As a part of our study, we need to know what residents think about the quality of services. Your household has been randomly selected to be part of this study.

The person completing this survey should be at least 18 years of age. If there is more than one adult in your home, please use this simple method to choose who answers: the adult who has had the most recent birthday should fill out the survey. (Year of birth doesn’t matter.) This way of choosing will help us hear from all kinds of people. Please help us, so that the results will truly represent the thinking of our entire community.

Your answers will be completely anonymous. Remove this page (with your address on the back), refold the completed survey, and return it to us. The postage is paid.
