Who are we?MARCH OF DIMES TODAY

March of Dimes helps moms have full-term pregnancies and healthy babies. And if something goes wrong, we offer in formation and comfort to families. We research the problems that threaten our babies and work on preventing them.

The March of Dimes is the leading nonprofit health organization that champions the needs of moms and babies in your community and across the nation. We funded the research that lead to the prevention of polio. And we haven’t stopped defending the health of babies since. From pioneering genetic research to promoting folic acid to fighting for lifesaving newborn screening, the March of Dimes continues to do all we can to make sure every baby is born healthy.

Signature Chefs Auction• The Signature Chefs Auction is a fine dining fundraiser in a

relaxed atmosphere with many networking opportunities. • Several local top chefs prepare their best hors d’oeuvres and

signature dishes for people to try.• The food is prepared by chefs from some of the most

respected, locally owned restaurants in the community. • There are hundreds of guests who enjoy fine dining in

attendance, including many community leaders. • Participate in Raffles, Auctions and the Super Silent Auction.• Enjoy music and dancing by a very special Central Wisconsin

band.

How to AttendThere are several ways to attend this event.1. Become an event sponsor: there are several sponsor

levels that include admission2. Join Stork Stories as a Story Sponsor or Captain3. Become an event volunteer4. Participate as a Chef 5. Be an event participant by buying tickets to attend

Participating ChefsThe restaurant business is one of the most competitive businesses there is

today. Our Signature Chefs event is a great way for restaurants to promote their business in the community and introduce their cuisine to new customers. The March of Dimes has a proven track record of bringing new business to participating restaurants.

Participating restaurants will receive the following:• Restaurant name and logo place cards on tables.• Restaurant name and logo on event invitations and posters.• One full-page ad in event program book and full page biography of

Executive Chef.• Placement of signage in registration area.• Acknowledgement in all media efforts and in advertising for the event.• Link to the restaurant/company website from the March of Dimes website.• Chefs invited to give a 5-minute presentation to promote their restaurant

and describe what they are serving at the event.• All chefs are entered in the Top Chef Competition. The Top Chef will be

picked by the event guests, who fill out comment cards. March of Dimes Volunteers will tally the comment cards and the winner will be announced that evening and given a prize, to be determined.

• Prepare and serve one or two sample-size specialty dishes that will serve 200 guests.

• Bring all equipment needed for serving. In order to control excess heat from chafing dishes and other flame type heating, please use CHOICE HEAT, a liquid heat with a wick.

• Arrive at least one hour before event for set-up (5:30 p.m.) and be present for the event.

• Allow March of Dimes permission to use chef’s name and restaurant in all print materials used in conjunction with the event.

• Participate in activities promoting the event including TV and radio interviews.

• Submit to March of Dimes a 200-word biography and photo of the chef/establishment for program book by 9/15/09.

• Provide logo and website link to March of Dimes to for Signature Chef’s Auction website.

• Participate in top Raffle Prize. Be willing to team up with 2 other participating chefs to prepare a 3 course meal for 8 at the winner’s house. One course prepared by each chef.

Give a gift to the March of Dimes and remember the babies who have fought, or are fighting the battle against prematurity, birth defects and infant mortality. Sign up to be a Stork Story Captain. Sign up to sell 8 Stork Story

seats at your table for $380.00 and you will receive 2 free seats for the event, 10 raffle tickets and recognition at the event for being a Stork Story Captain.

Pre-event: purchase your Stork Story sheet before the walk and e-mail your story and picture to March of Dimes. They will format it and post it at the event for you.

Beverage Donor OpportunitiesOur Signature Chefs Auction event is a great way for beverage donor

businesses to promote their products to restaurants, chefs, and businesses in the community. You can use the event to introduce new products to customers. The March of Dimes has a proven track record of bringing new business to participating companies.

Participating companies will receive the following:• Company name and logo place cards on tables.• Company name and logo on event invitations and posters.• One full-page ad in event program book and a full-page biography

of your representative or description of the company.• Placement of signage in registration area.• Acknowledgement in all media efforts and in advertising for the

event.• Link to company website from the March of Dimes website.• You are invited to give a presentation to those in attendance to

promote your company and describe what they are serving at the event.

• All beverage donor businesses are entered in the Top Beverage Competition. The Top Beverage will be picked by the event guests who fill out comment cards. March of Dimes Volunteers will tally the comment cards and the winner will be announced that evening and given a prize, to be determined.

consideration of the receipt of this document, the recipient agrees not to reproduce or make this information available in any manner to persons outside those directly responsible for the evaluation of its contents.PROPOSAL AGREEMENT

This proposal has been prepared to demonstrate the opportunities and benefits available as a Beverage Sponsor of the 2009 Signature Chefs Auction.

As a participating Beverage Sponsor, I will prepare and serve beverages in sample-sizes that will serve 200 guests with my own equipment needed for serving (glasses will be provided). I will arrive and be set-up by 6:00 p.m., then be present for the event until 10:00 p.m. Also, March of Dimes is given the permission by us to use company name and in all print materials used in conjunction with the event.

We agree to participate in activities promoting the event including TV and radio interviews. Also, we will Submit a 200-word biography and photo of the establishment for program book by 09/15/09 to March of Dimes and provide logo and website link for Signature Chef’s Event website.

Purchase a Stork Story for the 2009 Signature Chefs Auction for $125.00 and personalize it with the story and photo of a baby you wish to honor. These Stork Stories will be placed in a special area at the event to remind us all

about why we are here. You will also receive 2 seats for the event.

2 ways to get your Stork Stories:1. Pre-event: purchase your Stork Story sheet before the walk and e-mail your story and picture to March of Dimes. They will format it and post it at the event for you. 2. Event Day Purchase: buy your Stork Story sheet at the March for Babies registration area and fill in your baby’s story and add a photo at the event. It will be posted immediately.

Stork Story: Jessica VollJessica was born on her due date and everything went smoothly throughout the pregnancy until her delivery. Being our first child, we weren’t certain what to expect in the delivery room and when they whisked her away to ‘pink her up,’ we didn’t think anything of it. We didn’t even get to hold her before they took her away, but we figured it was what happened to all babies! It wasn’t until later we found out she needed oxygen right away. We realize that we have been very blessed that she and her sister have both been healthy babies. Here’s to Jessica!

This proposal has been prepared to demonstrate the responsibilities of a Stork Story Captain. As a Stork Story Captain, I have agreed to sell the remaining 8 seats at my table for a total of $380.00 or more. In exchange I will receive 2 free seats at the table, 10 raffle tickets and recognition at the event as a Stork Story Captain. I understand that I am responsible for selling all the seats. If all the seats are not sold I will make up the difference.
