Dear Parents/Guardians/Cheerleader: Welcome to the West Seven Rams Youth Club

We are excited to have your child as a participant in our program, and we’re looking forward to having a

successful season. During this season, we would like to establish and maintain a positive relationship between

our parents and our staff that will be beneficial to your children.

We need each parent or guardian to complete a registration form and keep us updated when there are changes to

your contact information. Provided at the bottom of this form are the names and numbers that are available to

help you with any situation regarding your child and the West Seven Rams Youth Club.

Registration Requirements

Refund Policy & Additional Information Parents/Guardian of a player/cheerleader dismissed or injured (upon doctor’s request) can receive a refund after (5) business days from the dismissal or injury date. Refund fees will be returned within five (5) business days after receiving an eligible claim. Parents not claiming a registration fee refund within five business days will forfeit refund. The schedule below indicates the formula for receiving eligible refunds: ZERO ROSTER GAMES: FOOTBALL REGISTRATION FEE MINUS $50.00 CHEERTEAM REGISTRATION FEE MINUS $40.00 If a child is terminated from the program for any of the following reasons, NO FEES will be refunded; failure to provide necessary documents, failure to turn in fundraising money; if child is disciplinary problem or voluntarily terminates his/her participation

NO REFUNDS GIVEN AFTER THE BENEFIT GAME.

No child shall receive a uniform until the entire registration fee is paid in full no exceptions. No child shall participate in any game or practice until their physical, birth certificate, and registration forms are on file with the organization.

Cheerleading Registration Fee Summary We would like to take this opportunity to explain to you what is expected of your child. As a Cheerleader or Pep Squad member, your child will participate in eight games providing that he or she meets all of the requirements for each game.

Registration Fee - Due 6 /1/ 2010: $100.00

Fee Includes: Game Uniform Rental, Annual Awards Banquet Ticket, Trophy, Replacement and Cleaning of Game Uniform, Liability Insurance, Administrative Costs and Other Miscellaneous Items. Payment Options: Option 1: Paying the Registration Fee of one hundred dollars ($100.00) in full. Option 2: Pizza Kits Fundraiser Option 3: Credit Card, Debit Card or Bank Account Option 4: Fundraising Scratch Card -*By request only ***See the registration desk or website for details on all of the above Registration fee will be collected in the form of money order, certified checks and on-line payments only. (NO CASH OR PERSONAL CHECKS ACCEPTED). All registrations fees are due on June 1, 2010 or upon acceptance to the cheer team.

Cheer Package - Due 7/16/2010: $120.00

Fee Includes: Gym Shoes, Custom Cheer Socks, Benefit Game Uniform, *Bodysuit, *Briefs and Cheer Backpack. (Optional items may include: Larger Cheer Bag, Headbands, Winter Jacket, Shorts, and Annual Competition Sleepover) *These items are required, but it is not mandatory that you purchase them through organization.

Warm-up Package - Due 8/13/2010: $ 80.00

Fee Includes: Team Custom Warm-up Jacket & Pant *Please note that warm-up is a custom item. Due to delivery time (6 – 8 weeks) NO late orders will be accepted.

I understand that heckling is unacceptable and may result in my ejection from the game.

I understand that I cannot coach from the sidelines and must allow the coaching staff to instruct the players.

I agree to inform the staff of any medical condition, illness or injury my child may have.

I agree to help my son/daughter accept defeat with honor, as well as be a gracious winner.

I agree that any problems that may occur can be addressed with the Coach at the appropriate time and in a respectful manner.

I agree that there will be no negative statements made in the presence of our youth towards the officials, other team, staff, etc.

I understand that the atmosphere, conversation, and conduct on the field (practice and games) will be that of encouragement and sportsmanship.

I understand that my child needs to attend every practice and game, as each child is a vital part of the TEAM.

I agree to make a commitment to volunteer a minimum of four (4) hours, one (1) concession stand shift and volunteer at one (1) major event during this season assisting the organization. I understand failure to do so will result in my child not being able to participate.

Team member/s that is absent two (2) or more days in a week without valid notice/reason will not be allowed to participate in the game. Multiple incidents may result in an automatic drop from the team.

Team members must be picked up ON TIME after practice. Failure to do so will result in dismissal from the program.

I authorize WSYC to use, reuse, publish, republish and copyright any photography necessary, for promotional purposes as they relate to this organization only, with or without the use of the above mentioned player/cheerleader’s name.

I agree to observe practice/tryouts from the designated area set forth by the WSYC.

Success is not only in winning the games; success is also measured in the manner in which the game is played and the conduct displayed from the players, staff, and parents/guardians. It is expected that all participants and supporters will adhere to the following conduct policies. West Seven Youth Club and Think Detroit PAL have a zero tolerance policy in place for ANYONE unable to conduct themselves in a sportsmanlike manner.

Parent Volunteer Policy and Contract

The West Seven Rams Football and Cheer Program is a volunteer-based organization and depend greatly on thecommitment and sacrifice of many volunteers. We do not compensate any of our volunteers, including coaches, boardmembers and team moms, yet they and many others devote a substantial amount of their time and energy to support ouryouth sports program. Parental involvement is mandatory and critical to our success. In addition to supporting and participating in our fundraising activities, we require all parents/guardians volunteer at least four (4) hours of volunteer time, one (1) shift of concession stand duties and one (1) event (Ex: Benefit Game, Homecoming Game, Homecoming Dance, Cheer Competition, Banquet, etc.) during the season or contribute $50.00 to WSRYC, to support the program. Although the monetary donation is greatly appreciated, your volunteer time is much more valuable. There are many opportunities and ways to contribute four hours of volunteer time and we have found that most families generously contribute more than the required four hours. Our 2010 policy for the concession stand and game day volunteer time will be to schedule you to work a game other than your child’s (either prior to or after) so you can fully enjoy your child’s game. Please be advised that failure to adhere to the terms of this contract will result in your child not being able to participate in the West Seven Rams Cheerleading and Football program. To make it convenient for you and to not leave it up to chance, volunteer when you know that you will be available. If you are unable to fulfill your obligation as scheduled, you must find an alternate who is at least 18 years of age. Failure to replace yourself will prevent your child from playing in the following weekend game. It is necessary to enforce this ruling to remain fair to all volunteers. All of our lives are busy, by doing your fair share, no one person will be overly burdened

Fund Raiser Information The West Seven Rams are sponsoring a fundraiser. We are asking that all players and cheerleaders participate in this function. Each player or cheerleader will receive 40 tickets at the cost of $2.00 per ticket full amount of $80.00 due by Friday, August 13, 2010. All proceeds raised will be allocated toward the following: Team Warm-Up Suit Each participant must submit the full amount due in order to be eligible to receive team warm-up suit. Funds submitted for less than the full amount will be forfeited to the Homecoming fund. All unused tickets MUST be returned back to registration table.

Our fundraisers are designed to provide assistance with most of the expense for needed cheerleading and football items. Most of us are parents too, we understand that the essentials in life are priority and everything else is secondary. That being said; we have planned fundraisers to assist in decreasing parents/guardians out-of-pocket expenses. Fundraisers are a necessity, however participation is optional. Please take time to look over your child’s raffle tickets and reach out to your family and friends and tell them.

Waiver of Responsibility West Seven Rams Youth Club

I, the parent/guardian of , agree to save harmless the West Seven Youth Club and the Think Detroit Police Athletic League against and from any and all liabilities, obligations, damages, penalties, claims, costs, charges, losses and expenses (including without limitation, fees and expenses of attorneys) which may be imposed upon, incurred by or asserted against the West Seven Youth Club or the Think Detroit Police Athletic League. I will assume all risks and responsibility for any injury occurring during his/her participation or transportation involving the West Seven Youth Club.

UNIFORM REPLACEMENT WAIVER

I understand that if my child destroys, or does not turn in his/her game uniform, helmet, shoulders pads, pompoms, or practice jersey at the end of the season, that I am financially responsible for the replacement cost of the following:

Football $200.00 Cheerleader $150.00

Failure to comply with this waiver will result in my child not being able to participate with the football/cheerleading program next season. I will assume all responsibility for penalties, claims, and court cost, including expenses of attorneys retained to sue for damages of unrecoverable equipment or uniforms.

This obligation shall terminate at the conclusion of the 2010 West Seven Youth Club football season.


Registration fee is $125.00 for players & $100 for cheerleaders. No refund will be issued to any player or cheerleader who quit voluntarily.
