In and for consideration of my child’s par-ticipation in the ETBU Football 2010 Skills Camp, I hereby agree that I will not hold East Texas Baptist University, the staff, the Athletic Department, or it’s employees responsible for any loss, damages, or per-sonal injuries that he may receive as a result of par-ticipation. This waiver of liability expressly includes camp activities or while in, on or upon the premises whereby the activity is being conducted and trans-portation to and from, or in connection with said camp. I also agree to follow all instructions and procedures in order to maintain a maximum level of safety. I also understand that no medical insur-ance coverage will be carried by the ETBU Football 2010 Skills Camp. I also give my permission for any emergency medical care or treatment by a physician, surgeon, hospital, or medical care facility that may be required, and accept the responsibility for the cost.

ETBU Football 2010 Skills CampRegistration Information:

The ETBU Football 2010 Skills Camp is designed to take each partici-pant to the next level in their individu-al football development. Not only will students have the opportunity to work with our excellent Tiger coaching staff, members of the staff from Oklahoma State University will be on hand to pro-vide additional top-quality instruction.

Registration begins at 8:30 a.m. at the ETBU Football Field house. The camp will conclude at 11:30 a.m. The camp fee is due at the time of registra-tion.

Certified Athletic Trainers will be on duty to provide any necessary assis-tance.

Each student is encouraged to bring a copy of their insurance information in case of the need for treatment.

About ETBU Athletics:East Texas Baptist Uni-

versity offers student-ath-letes, students and sports fans a wide variety of in-tercollegiate athletics pro-grams from which to choose. The ETBU Athletic Depart-ment seeks to provide qual-ity Christian education and

competition for all of its student-athletes.

ETBU is a member of the National Collegiate Athletic Association (NCAA) and competes at the NCAA Division III level. Division III is non-scholarship, meaning ETBU does not offer athletic scholarships in any sports program. Student-athletes can receive financial aid through loans, grants or academic schol-arships, however. ETBU has been a member of Divi-sion III since 1998, joining the ranks from status as an NAIA and NCAA Division II institution.

ETBU joined the American Southwest Conference (ASC) in 1998. The ASC is divided into East and West Divisions, with a current total membership of 15 col-leges and universities.
