Personalrates apply to all customers purchasing for the use of one person (end-user name must be provided).

Definition of relevant Full Time Equivalents (FTEs):

Hospitals: Total number of clinicians, pharmacists and researchers (does notinclude nurses)

Corporate institutions: Total number of researchers and medical informationemployees

Government institutions, regulatory agencies, and all others: Total number ofprofessionally qualified staff

Academic Institutions: Total number of medical faculty (academic staff andstudents) and researchers

Important information

Address changes

All change of address requests must be submitted promptly in writing. Any claim

requests relating to a late address change notification will only be fulfilled in

exceptional circumstances.

Cancellations

Once an order has been placed it is not possible to cancel and receive a refund,

unless an exact duplicate subscription has been ordered in error. In this case we

will accept a cancellation of one of the orders. We cannot accept a cancellation toa subscription that involves the reduction in size of an institutional FTE tier or

publication, once an order has been processed.

Please note that all end user customers will be subject to our terms and

conditions and the relevant online licence, which other than as stated below must

be brought to their attention via the agent/bookseller before placing an order.

Consortia and Corporate Licences will be supplied once an order is placed which

requires the customers agreement.

Consolidation mailing address

We will accept the use of consolidation mailing addresses, but full end-user

address details must be provided. This is particularly important where there is an

online component to the subscription.

Customer email addresses

For all orders that include online access we require a customer email address.

This will enable us to inform the end-user of the activation process for their

subscription.

Delivery

All subscriptions are sent to European destinations by priority mailing services

and the rest of the world by standard mailing services. Priority mailing services to

most non-European countries are available, on payment of an additional fee.

These rates are available on request.

Cheques should be made payable to BMJ Publishing Group Ltd. Credit cards are

VAT

All UK subscription Agents must pay UK VAT on Online only and the online portion

(currently 59%) of all Print and Online subscriptions.
