Several parking areas are controlled by “Park and Pay Stations,” which are electronic vending machines located on the main floor of the parking garage. Because this course is meant to educate physicians with what is currentlyin use and what may be available in the future, there maybe “off-label” usediscussed in the presentations. Speakers have been requested to inform the audience when off-label use is being discussed. The registrationfees below entitles participants admission to the conference, all course mate-rials, continental breakfasts, refreshment breaks and lunches.

We are unable to process any registration without payment. On-site registrationwill be available; however, we cannot guarantee course materials and meals/re-freshments on site to anyone registering after April 26, 2013.

The course is designed for practicing otolaryngologists, residents and fellows
in training, and other health care providers interested in a comprehensive
update on advances in medical and surgical care of rhinologic disease.
Describe current treatment strategies for sinonasal diseases with an
emphasis upon comprehensive evaluation and medical treatment as well
as minimally invasive endoscopic sinus surgical techniques.
The University of Texas Southwestern Medical Center is accredited by the
Accreditation Council for Continuing Medical Education to provide continuing
medical education for physicians.
The University of Texas Southwestern Medical Center designates this live
activity for a maximum of 20.5 AMA PRA Category 1 Credits™. Physicians
should only claim credit commensurate with the extent of their participation
in the activity.
Upon completion of this activity, participants should be able to link the

n Review the endoscopic and radiographic anatomy of the paranasal sinuses

in training, and other health care providers interested in a comprehensive
update on advances in medical and surgical care of rhinologic disease.
Describe current treatment strategies for sinonasal diseases with an
emphasis upon comprehensive evaluation and medical treatment as well
as minimally invasive endoscopic sinus surgical techniques.

The registrationfees below entitles participants admission to the conference, all course mate-rials, continental breakfasts, refreshment breaks and lunches.

Valet parking is available for a nominal fee at the main entrances of the buildings listed below. Hours and rates are posted at the valet stations in front of each building. Validation is required to receive the patient rate.

NEW! PARK AND PAY STATIONS
Several parking areas are controlled by “Park and Pay Stations,” which are electronic vending machines located on the main floor of the parking garage. The parking ticket is fed into the machine, the rate is displayed, and payment is made by voucher, cash, or credit card. The parking ticket is returned, and then is inserted at the exit gate.

It is UT Southwestern’s policy that participants in CME activities should bemade aware of any affiliation or financial interest that may affect thespeaker’s presentation(s). Each speaker has completed and signed a con-flict of interest statement. The faculty members’ relationships will be dis-closed in the course syllabus.
Because this course is meant to educate physicians with what is currentlyin use and what may be available in the future, there maybe “off-label” usediscussed in the presentations. Speakers have been requested to inform the audience when off-label use is being discussed. The registrationfees below entitles participants admission to the conference, all course mate-rials, continental breakfasts, refreshment breaks and lunches.
Registration is confirmed upon receipt of registration fee. See registrationform for schedule of fees.
We are unable to process any registration without payment. On-site registrationwill be available; however, we cannot guarantee course materials and meals/re-freshments on site to anyone registering after April 26, 2013.
