Instructions

Looking at the picture on the first page of this document, your team has been designated indesigning a Job Hazard Analysis (JHA) Assessment Table for the task being carried out by the worker highlighted in the red box. (The worker is cutting open the 45 gallon drum marked “flammable” with a portable rotary cutter)

[Total = 100 marks]

In your discussion, you would need to include the following points from observation of the whole scenario.

1. What are the hazard/hazardous situation you have identified? (10 marks)2. Where is the hazard located? (10 marks)3. What could the injury/illness/disease outcomes be if an accident occurred? (10 marks)4. What is the likelihood of this accident occurring? (10 marks)5. In your opinion/your team’s opinion, are the existing hazard control measures adequate for the risk? (10 marks)6. Reason(s) (10 marks)7. What control measures/improvements can you suggest to remove or reduce the risks of this hazard? (10 marks)

TABULATE the response to the above for easy of cross referencing.

It is necessary to include all of the above points when designing your control measures, however you may not be limited to the above and other points can be suggested and marks will be awarded accordingly.
