    Every $1 donated is matched by in grant funding. $6 Less than of our annual

    How Funding is Designated

    Our Mission is to assess, strengthen, and promote the health of Buff alo County.

    Our Vision is that everyone from all corners of Buff alo County work together to improve the quality of life of those who live in and work in this community.

    Bu alo County Community Partners

    People create the story in our community. We choose to live in strong and vibrant communities and push to create a better future for our families. Buff alo County Community Partners provides networking for 2,500 community volunteers and150 businesses engaged in Building a Healthy Community.

    The past three years serving on the Community Partners board has shown me that this is an organization that gets measurable results in their eff orts to make Buff alo County a healthier place to live, work and play.

    Our Safe Routes to School program promotes walking and biking to school and teaches students how to be safe when they do.

    Each $10 investment generates a ve-year net return of $50.60

    I joined Community Partners because I saw an organization that can improve the health of many in our community by working together collectively to nd the answers and develop and deliver programs.

    Buff alo County Community Partners is a non-pro t organization that aims to improve the health status of Buff alo County residents by:

  • What We Do

    Access to Care1. Built a national community health worker

    model using pathways to engage the community in coordinated care.

    2. In three years, the model generated $1.14 million in cost savings to CHI Health Good Samaritans ER.

    3. Expanding the model by developing a referral network of diabetes support programs.

    4. Expanding the model by developing a behavioral health integration concept.

    5. Evaluating success of programs to increase access to care, health outcomes and cost savings.

    Building Social/Emotional Skills1. Working with child care centers and

    schools to implement the Second Step social-emotional learning curriculum in Buff alo County.

    2. Supporting and working to expand the Friends peer mentoring program in schools throughout the county.

    3. Encouraging trauma-informed workplaces and organizations in order to build a community culture of mutual support.

    4. Promoting creative and constructive youth empowerment through the PhotoVoice program.

    5. Evaluating program success with the goal of reducing youth violence.

    Building Social/Emotional Skills

    schools to implement the Second Step

    Safe Routes to School1. Working

    with schools in Ravenna, Pleasanton and Kearney to research safe routes to schools.

    2. Identifying barriers and solutions to safe routes by engaging schools, parents and youth in program.

    3. Hosting Bike Rodeos, Walk/Bike to School, Bike Safety and more in area communities.

    4. Developing safe routes to school curriculum to sustain program.

    5. Evaluating success of programs to increase the number of youth who walk and bike to school.

    Alzheimers Disease and Dementia1. Developed four community

    sessions for families and their loved ones with Alzheimers and other dementia.

    2. Families received more information on estate planning, nutrition and patient care during holiday season.

    3. Working alongside physicians, law enforcement and other supportive services to create coordinated care.

    4. Provide a network for families and care givers to create a learning environment.

    5. Evaluating success of programs to meet needs of Alzheimers patients and their families.

    Alzheimers Disease and Dementia

    Take a Stand1. Implemented a year-round blog to assist

    parents in starting conversations with teens about important issues.

    2. Promoting cyber bully prevention, substance abuse prevention, mental health promotion, and discussing emerging youth drug trends.

    3. Developing Webinar with Dr. Atri to present information on signs of substance use, mental illness, or other behavioral trouble in teens.

    4. Partnering with Youth Advisory Board to develop individualized prevention plans for schools and communities.

    5. Evaluating success of media campaign to increase parent, school and community involvement.

    You may have noticed these signs on Cruise Nite. Theyre part of our Be Safe, Be Smart task forces work to curtail after-Cruise Nite partying.

    Be Safe, Be Smart1. Developed tips for parents, retailers, youth, and property owners

    to Be Safe. Be Smart during after-Cruise Nite activities.

    2. Created yard signs and lea ets to hand out along 23rd - 26th Street properties.

    3. Coordinating eff orts between law enforcement, county attorney o ce, property managers, retailers, UNK and others.

    4. Empowering youth to develop ideas for alternate activities.

    5. Evaluating success of media campaign in the reduction of youth and adult substance abuse.

    Employee Wellness1. Adopted Wellness

    Works program to incorporate into Activate Buff alo County coalition.

    2. Hosted Walk Out On Your Job.

    3. Launched new employee wellness membership program.

    4. Launched new Be Well task force to develop strategies for community well living.

    5. Developing evaluation plan to measure success of increasing well living.

    Americas Best Communities1. Organized community leaders to develop a vision for Kearney to compete in Americas Best Community Award program.

    2. Named one of Americas Best Communities 50 quarter nalists communities.

    3. Developing a community revitalization plan to address workforce development, housing, transportation.

    4. The team goal is to develop a career development center to meet the needs of Kearney area workforce.

    5. Evaluate the success of a career center in meeting the needs of Buff alo County residents.

    Activate Bu alo County hosted the 25th annual Walk Out On Your Job as part of its new employee wellness initiative. Sixty businesses participated. Is your workplace activated?

    Community Partners is instrumental in promoting healthy families and providing children with a community in which they can thrive. As a family, we are honored to be a small part of this great organization.

    - Bryan, Candy, Brady, and Skylar Kuntz

    Bryan is the VP of Operations for Intellicom. He has previously been active in our community as a volunteer with the United Way and the TeamMates mentoring program.

    Candy is a stay-at-home mom, part time grant writer, and independent ambassador with Plexus Worldwide. As a grant writer, Candy was involved with Safe Routes to School and the Buff alo County Housing Coalition.

    Skylar and Brady enjoy joining their parents for walks, reading, and exploring Buff alo County.
