Welcome to Open Enrollment!

Dear Colleague, It’s that time of year again! Similar to the excitement we feel at the start of a new academic year, our annual benefit enrollment period also creates a sense of anticipation for many of us. The benefit open enrollment period runs from September 25th – October 6, 2017. Similar to last year, this will be an “Active” enrollment meaning that all benefit eligible employees must actively make benefit elections online. This open enrollment period is your one-time annual opportunity to make changes to your benefits. The benefit choices you make now take effect November 1, 2017 and end October 31, 2018. You may only make changes after this period if you experience a “qualified life event” such as marriage, birth of a child or employment status changes. Many of you have been working really hard to maintain good health as proven by the lower claims experience with Independence Blue Cross. While there are no guarantees this positive trend will continue, we’re optimistic that with your help, it will. We encourage you to double your efforts to maintain an active and healthy lifestyle which translates into cost savings to you and the University. Additionally, we are glad to see by the numbers that folks continue to take advantage of the free preventive care services and various wellness programs offered by our carriers such as the (6) free dietician visits, weight loss challenges, gym membership discounts and more. Your HR team has also been working really hard over the summer! We take great pride in offering a competitive benefit package to attract and retain talented employees. We are pleased to share that our benefit enrollment site with Cigna has been enhanced to provide you with a better customer experience, more resources and easy access to information you need to make good benefit decisions. We have worked with our carriers to manage costs and limit changes for the upcoming plan year. Therefore, we are pleased to share that our current benefit plan carriers will remain the same and employee contribution cost for medical insurance is unchanged for the third consecutive year. The benefits available to you such as medical, prescription, vision, dental, life insurance, short term disability, long term disability and many intrinsic benefits are important parts of your total rewards at Arcadia University. More importantly, they provide access to quality medical care and peace of mind for ourselves and our loved ones. What’s New! Newly enhanced Cigna benefit enrollment site: http://www.cgsmarketplace.com

New Accident and Hospital Care Voluntary Plans: Through Cigna, these two plans will now be offered on a Voluntary basis. They provide cash payouts to anyone who experiences an injury, receives medical treatment, or has a hospital stay. Various level of payout is provided based on the type of situation. More information is provided later in this guide.

Dental PPO Benefit Enhancement: We heard you and made an enhancement to our Dental PPO plan! All major services will now be covered at 80%, up from 50% last year.

Pet Insurance: We will begin offering a health insurance plan for pets that covers their illness and various injuries. This will provide coverage at Veterinarians, ER's, and Specialists. Be on the lookout for a separate communication and link to this terrific new benefit!

The IRS has increased the annual medical spending account deferral to $2,600, up $50 from last year!

Some dental plan tiers will see a modest increase to employee monthly contribution amounts.

What’s Staying the Same:

Active Enrollment: Active enrollment means that all benefit eligible employee of Arcadia MUST actively make benefit elections for Medical, Dental and Flexible Spending Accounts or will default to “no coverage” for these benefits. You must complete enrollment no later than midnight Friday October 6th or you will not have coverage effective November 1, 2017. If you don’t enroll by the end of the open enrollment period, you will not be allowed to enroll in any benefit plans until next year’s annual enrollment period unless you experience a qualified life event during the year that allows you a new opportunity to enroll.

We will continue to offer Independence Blue Cross medical plans and Cigna Dental plans, Life Insurance, Short Term Disability and Long Term Disability.

The Employer Health Spending Account (HSA) contributions to Personal Choice High Deductible plan remain the same for 2017-18 plan year.

Any Cigna Voluntary Life Insurance coverage amounts already approved will remain the same unless you elect to increase or decrease the existing coverage amounts. Increasing life insurance amounts will require an evidence of insurability form to be completed before being approved. Please note that the voluntary life insurance premiums you pay are age based and therefore will increase as you enter into higher age brackets.

Benefit Eligibility

Benefit eligibility is defined as:

You are eligible for coverage if you are an active, regular full time employee and work at least 1,500 hours per academic year.

Eligible dependents include:

Your spouse.

Your married or unmarried children until the end of month of their 26th birthday. Such children include (a) a blood descendant of the first degree, (b) a legally adopted child (including a child living with the adopting parents during the period of probation), (c) a child for whom you have legal guardianship, (d) a stepchild or (e) a child who is the subject of a Court Order.

Your married or unmarried children covered prior to attainment of age 26 and who are incapable of employment by reason of mental retardation or physical handicap and who became incapable prior to age 26, Newborn children for thirty-one (31) days after birth, a Domestic Partner, Married or Unmarried Children of a Domestic Partner.

Choose your benefit plans and coverage levels carefully. The IRS only allows changes to your benefit elections for certain qualifying status changes:

If you experience a qualifying status change and want to make a change to your benefits, please contact Human Resources within 30 days of the event. Documentation will be required to substantiate the Qualifying Life Event and the transaction should be done using the online benefit enrollment site.

Summary of Benefit Plans

Arcadia University provides colleagues with a choice of competitive medical benefit options to meet an array of individual and family needs for health promotion, preventive care and when necessary, major medical expenses. In 2017-18 Arcadia will offer three Independence Blue Cross options; Personal Choice High Deductible Health Plan (HDHP), Keystone Flex and Keystone 515. All three plans include a formulary prescription drug plan and vision coverage. Note: The Keystone HMO plans are available only to employees who live in the five-county area (Philadelphia, Montgomery, Bucks, Berks, Delaware, and a small portion of Southern New Jersey).

“Personal Choice HDHP” - a high deductible PPO plan. After the deductible is met, there is 100% coverage on all in-network medical expenses, with the exception of prescription coverage, which carries a co-payment of $5/$20/$45. Out-of-network coverage is subject to a $5,000/$10,000 deductible and 50% coinsurance. When enrolled in a High Deductible PPO plan, employees can open a Health Savings Account referred to as HSA. An HSA account permits employees to set aside pre-tax dollars to pay for medical expenses until the deductible is met. Employees enrolled in Medicare or have other health insurance are not permitted to contribute to an HSA. An HSA is a tax-advantaged health care account that you own. You contribute to it with tax-free or tax-deductible funds. You can use those funds to pay for eligible health care expenses now and in the future. This includes expenses for you, your spouse and your tax dependents. This is true even if your spouse and dependents are not on your health plan. To contribute to an HSA you must be enrolled in the High Deductible Health Plan (HDHP). The IRS maximum limit on contributions for this coming plan year are $3,450 self only and $6,900 Family. Additionally, Arcadia contributes annual amounts of $750 towards Single coverage and $1,500 towards family coverage to help you meet the required deductible amounts. The funds that you contribute but don’t use will roll over year to year. In addition, an HSA is portable which means that if you change employers or leave the work force, the HSA stays with you. "Keystone Flex” – an HMO plan with a $20 or $40 co-pay for office visits and a $15/$35/$50 co-pay for prescription drugs. In exchange for a lower monthly premium, this plan has co-pays for some services, such as physical therapy and diagnostic tests. The co-pay for hospitalization is $150/day (capped at $750). There is also a co-payment of $75 for out-patient surgery. See additional covered services and details on the following pages. "Keystone 515” – an HMO plan with a $15 or $25 co-pay for office visits and a $15/$35/$50 co-pay for prescription drugs. There is also a co-payment of $500 for a hospital admission and a co-payment of $250 for out-patient surgery. See additional covered services and details on the following pages.

Cigna DHMO or Cigna PPO Dental insurance is currently provided by Cigna. Note: The Cigna DHMO plan is available only to employees who live in the five-county area (Philadelphia, Montgomery, Bucks, Berks, Delaware, and a small portion of Southern New Jersey). Once you enroll in Cigna DHMO, register at myCigna.com. You can search for a dentist using your home zip code. You can search for a network dentist by name, specialty or location. Just follow these easy steps: › Log in to myCigna.com. Click on “Find a Doctor, Dentist or Facility” at the top. Under “Find a…” click “Dentist.” This information is also available on the myCigna mobile app. Call Cigna at 800.Cigna24 where you can speak directly to a customer service representative. Flexible Spending Accounts enable employees to set aside pre-tax income to pay for eligible child/dependent care costs and/or out-of-pocket medical costs. This is accomplished through pay reductions, which lower your federal taxable income. Money set aside in the FSA is not subject to federal income taxes or FICA taxes, and is used to reimburse the employee for actual dependent and health care costs paid. In addition, funds that are set aside for Health Care FSAs are also exempt from PA state taxes. There are two types of flexible spending accounts: Dependent Care Spending Accounts and Health Care (including dental and vision) Spending Accounts. Employees may set up one or both accounts. Establishing a FSA is generally limited to the beginning of employment and the open enrollment period. Debit cards are available for both the Health Care and Dependent Care FSA. Use of the debit cards enables direct payment for eligible out-of -pocket medical expenses or costs at participating dependent care facilities and eliminates the need to submit claim forms and wait for reimbursement. Dependent care expenses are limited to the current account balance each month.

Using the mail order program for your maintenance medications will save you money. You will receive up to a 90-day (3-month) supply for two retail copays. Hospital Care Insurance Cigna

Arcadia University offers you a choice of two Cigna dental plans: Dental HMO and Dental PPO. The Dental HMO (DHMO) plan helps to streamline your dental care and makes most preventive and diagnostic services available at a reasonable cost or no additional cost to you, including yearly fluoride treatments for covered children. There is a copay for covered services, but no deductibles or annual dollar maximums, no coverage waiting periods and no claim forms to complete. A Flexible Spending Account (FSA) allows you to set aside money for eligible expenses on a pre-tax basis. See the example below how this account can save you cash! You can carry over $500 from your medical FSA into the following year. Examples of eligible FSA expenses include doctor office visit copays, prescription drug copays or vision care, For a complete list, visit www.irs.gov.

Health Care Flexible Spending Account (FSA) The minimum annual election is $130 and the maximum election is $2,600. PayFlex will issue a debit card with your elected amount to use on eligible expenses during the year. Up to $500 of unused FSA dollars can be carried over into the next plan year. You will not receive a new debit card until you reach the expiration date on your current card.

Dependent Care Flexible Spending Account The minimum annual election is $130 and the maximum election is $5,000. You can reimburse yourself on daycare expenses paid during the year by submitting a claim form or using your debit card. You can receive up to the amount contributed into the dependent care FSA at anytime during the year. Unused dependent care FSA dollars are forfeited and cannot be rolled over into the next plan year.

Example of how FSA Contributions can increase your spendable income! Bob and Jane’s combined gross income is $30,000. They have two children and file their income taxes jointly. Life, AD&D and Disability Insurance Cigna

Basic Life and AD&D Insurance All active, full-time employees regularly working at least 35 hours each week are automatically enrolled in the Basic Life and Accidental Death and Dismemberment (AD&D) plan. This coverage is available to Arcadia employees at no cost - the univeristy pays 100% of the Basic Life and AD&D premium. The Basic Life and AD&D benefit is 1.5 times annual earnings to a maximum amount of $500,000.

Supplemental Voluntary Life and AD&D All full-time employees who work a minimum of 1,500 hours per academic year may purchase supplemental life insurance for themselves in an amount of up to 5 times base salary in increments of $10,000 (not to exceed $500,000). Coverage may also be purchased for spouses or qualified domestic partners in increments of $5,000 up to an amount of 100% of the employee’s supplemental insurance. Dependent children (up to age 26) may be covered in increments of $2,000 up to a maximum of $10,000. The maximum benefit for a child between the ages of live birth and 6 months is $1,000. Coverage for all or part of your supplemental insurance may be subject to evidence of insurability. Any increase amount during open enrollment will require an EOI. Coverage will reduce to 65% of the original amount at age 70, and to 50% of the original amount at age 75. Other limitations and exclusions may apply. Short-Term Disability Short-term disability support shall be available to full time employees of Arcadia University after one year of continuous full time employment. Short-term disability benefits may continue for up to six months with proper medical documentation.

Long-Term Disability After one year of continuous full-time employment, the University pays the premium for a disability group insurance plan. Under this disability program, after six months of total disability, eligible employees may apply for long-term disability benefits. If approved by the insurance company, employees will receive 60% of their base salary up to the plan maximum. Waiting period for enrollment may be waived if you can provide proof that you were covered under a similar group employer plan within the last 90 days.

Retirement Take advantage of the Arcadia University 403(b) matching plan and build your retirement. When you set aside up to 6% of your annual salary to the plan on a tax deferred basis, Arcadia University will provide a matching contribution up to 6% - a total contribution rate of 12%. And, if you set aside 6.5% or more of your annual salary to the plan on a tax deferred basis, Arcadia will provide an 8% match, bringing your contribution total up to 14.5%. Tuition Remission All full time employees are eligible for tuition remission at Arcadia for undergraduate, graduate and doctoral study. Tuition remission covers 100% tuition but does not include any fees, housing and other expenses. All state and federal grants and scholarships, including Arcadia scholarships, will be deducted from the tuition charges before tuition remission benefits are applied. All full time employees are also eligible to participate in the CIC Tuition Exchange program. Life Assistance Program Achieving a health work life balance can sometimes be challenging. If you would like help handling life’s demands, Call Cigna’s Life Assistance Program anytime 24/7 at (800)-538-3543. This Program provides guidance on topics such as legal consultation, parenting, senior care, child care, pet care, financial services and referrals. This program offers three face-to-face sessions with a behavior counselor available to you and your household members. You also get access to discounts on health and wellness products and services. Paid Time Off (PTO) Arcadia University offers a generous PTO allotment to all regular full-time 12 month employees. Part time staff members who are expected to work at least 1000 hours per year will earn prorated vacation. See PTO policy for further details. Vacation Vacation pay is earned according to classification and length of employment. Vacation will be accrued each payroll cycle at the appropriate rate, and balances will be reflected in total hours. Personal All regular, full-time staff members are given two personal each year. Personal time for those who have been employed by the University for less than one complete fiscal year is pro-rated. Sick All regular, full-time, non-faculty employees receive sick time. Employees will receive their full allotment, which is the equivalent of the yearly cap, of sick time on June 1st of each fiscal year. Sick time is not carried over into the new fiscal year. 9, 10 and 11 month employees receive a prorated amount. Employees are eligible to use sick time for personal illness or illness in the immediate family (spouse, children, and parent).

Holidays The following holidays will be observed: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving, Christmas Eve Day and Christmas Day. Most university offices are closed during the week between Christmas Day and New Year's Day. Some offices cannot close completely because of the nature of their operation. The supervisor can explain which employees would be required/asked to work during that time. Bereavement Leave In the event of a death in the employee’s immediate family, the University may grant leave with pay not to exceed three days. Immediate family is defined as spouse, domestic partner, brother, sister, child, daughter-in-law, son-in-law, stepchild, grandchild, parent, mother-in-law, father-in-law, and grandparent. One day off with pay may be granted by the University in the event of a death of an aunt, uncle, brother-in-law, sister-in-law, nephew or niece.

a. If you have been a Cigna customer within the last two years at your current employer, and have created a myCigna.com user ID and password, continue with your myCigna.com user ID and password to log in.

Notes:

Most coverages allow a two-year run-out period after the coverage cancellation date. If you have been a Cigna customer within the last two years at your current employer, and had created a myCigna.com user ID and password but have forgotten your user ID or password, click Forgot User ID or Forgot Password respectively to have either one reset.

Any myCigna.com user ID and password that you created while covered under a different employer will not work here. Your myCigna.com user ID and password must be associated with your current employer.

Do not use your login credentials from any previous Cigna online benefits enrollment tool because those credentials are no longer valid.

For security purposes, the system does not highlight specific errors. You must verify each key field carefully, and pay particular attention to your date of birth and zip code, because these are easy to mistype.

iii. Create your user ID and password, enter your email address, select your security questions and answers, and then click Next.

Note: The system uses your personal details and your security questions to validate your identity if you forget either your user ID or password. The system also uses the personal information that your employer sends to Cigna that allows you to sign up for Customer Portal access. For more information, see Troubleshooting.

Important! The user ID and password that you create for access into the CGS Customer Portal becomes your user ID and password for access into your myCigna.com account (with your current employer) in the following cases:

You do not have an existing myCigna.com user ID and password.

Your existing myCigna.com user ID and password is with a previous employer or under another existing coverage (as a dependent, for example).

If either case applies to you, remember that your CGS Customer Portal user ID and password will become your myCigna.com user ID and password.

Troubleshooting

Review the following troubleshooting information if you are having issues registering for the CGS Customer Portal.

Confirm Your Identity Tab

The following information describes issues that you may encounter in the Confirm Your Identity tab.

If you receive an error after you enter your Social Security number, it is because the system does not recognize your personal details, which can affect your ability to register.

You can receive an error because of any of the following reasons:

Recent name change

Hyphenated names – try entering your name using a space instead of a hyphen

Using a different name than what is on your paycheck – try using the name that is on your paycheck

Recent address change – if you moved recently, make sure that your employer has your new address. Otherwise, try entering your previous address.

Date of birth is incorrect with your employer

Social Security number is incorrect with your employer

When you receive an error, you can do either of the following:

Click Previous to go back to the Enter Personal Details tab to verify that you entered your personal details correctly.

Click Cancel to cancel the registration process.

The Cancel Account Registration dialog opens, prompting you to confirm whether you want to cancel your registration.

Click Yes.

You return to the Login to Cigna Guided Solutions page.

Note: You can only attempt to register your account a maximum of three times before you receive an error and the system prevents you from registering for one hour.

Keep the following in mind when you create your user ID and password:

Make sure that you use the correct format when you create your user ID and password, and that your intended user ID is available for you to use.

Do not use your Cigna ID or Social Security number as your user ID.

You can click the Check Availability link to make sure that your user ID is unused.

Make sure that your password is between 6-12 characters with one number, one letter, and no spaces. The only special characters allowed are _ ! . & @

In general, make sure that you are using the information that your employer has on file for you. In addition to validating the personal information that you enter, the system also uses the personal information that your employer sent to Cigna. Make sure that the information that you are using to sign up matches the information that your employer has on file for you.
