The “Invest In Your Health” wellness program is voluntary for all Independent Bank employees. However, if you choose not to participate, you will miss out on significant financial rewards. See program requirements and health metric details below!

1. Complete registration - OPEN NOW. Log in to your wellness portal at www.bravowell.com/independentbank to complete this step.

2. Complete the health risk assessment - OPEN NOW. Log in to your wellness portal at www.bravowell.com/independentbank to complete this step.

3. Complete two wellness activities - available on your wellness portal March 20, 2017. See page 3 for your wellness activity options.

4. Complete a 2017 biometric screening and meet or improve upon at least three of five biometric goals compared to your prior year results - available on your wellness portal March 20, 2017. Review the My Health section on the wellness portal for your specific goals. If this is your first year participating in the program, you are only required to submit results for the five biometric measures listed below. As these results are for informational purposes only, you are not required to meet specific goals to earn the incentive. Your medical premiums will be affected if these are not met.

• Body Mass Index < 25.0 or Waist < 40 inches for males/ < 35 inches for females OR improvement

• Blood Pressure < 130/85 OR improvement

• HDL ≥ 40 male / ≥ 50 female OR improvement

• Glucose < 100 OR improvement

• Triglycerides < 150 OR improvement

Medical plan participants = lowest 2018 healthcare premiumEmployees who meet all program requirements will qualify for the lowest 2018 healthcare premium.

Non-medical health plan participants = $150 incentiveEmployees who are not enrolled in the health plan will receive a $150 incentive if they meet the program requirements.

The wellness portal will show the status of your incentive and what is complete/incomplete. It will also track your progress towards your 2017 goals … be sure to log in to check progress!

ACTIVITY OPTIONS

Please complete two of the following wellness activities below to earn credit towards your 2018 wellness incentive. On March 20, this step will be available on your wellness portal (step 3).

Engagement Platform - 8 Week InvestmentLog in to the wellness portal at least three times a week for 60 days and use the resources below to earn credit for one required wellness activity.

Online Health University - 8 to 12 Week InvestmentYou will have access to six Online Health University courses that are self-led. These courses will be provided by a partner of Bravo, Self Help Works. The content is structured around Cognitive Behavioral Training, and takes 8 to 12 weeks to complete. Self Help Works will send reminder emails to help track course progress. Course must be completed to earn credit.

Group Challenges - 6 Week InvestmentCore 4+ Challenge: May 1 - June 9, 2017Encourages focus on one or more of the four areas of activity, nutrition, stress management and/or weight. With tracking and logging, you’ll work toward a healthy lifestyle!

Family Challenge: July 17 - August 25, 2017Families learn how to incorporate healthy activities in four areas: community, physical activity, nutrition and family togetherness.

Preventive Care - 1 Hour InvestmentMaximum: One Preventive Exam Eligible for Incentive RequirementIndependent Bank knows it is important to stay on top of your healthcare. If you see your provider to complete one of the following exams, you will receive the activity credit. Note: Your Preventive Care Provider Form will be available for download on the web to document completion of the exam(s).

If you completed a 2016 biometric screening, you will have access to a one-time consultation with a health coach to review your results, program resources, available incentives and health plan information.

Ongoing Telephonic Wellness Coaching - 12 Week InvestmentMust have 2016 biometric results and three or more biometric risk factors. Deadline is July 31, 2017. If you qualify for telephonic coaching, you will have access to four sessions of consultation with a health coach. Your record will be updated and you will be notified on the Independent Bank wellness portal if you qualify.

BIOMETRIC APPROVED GOALS

Bravo offers an appeal process if you believe your results are incorrect or think you may qualify for a medical exception. Contact Bravo at 877.662.7286.

What qualifies as improvement?For example: If your 2017 screening result for triglycerides is 188 and the program goal is less than 150, you would earn credit for triglycerides by hitting a result of 178, even though you did not hit the triglyceride goal. Bravo will review your year-to-year results (2016 screenings to 2017) and if 5 percent improvement is made, credit for that metric will be awarded. If you did not screen in 2016, the 2017 screening will be your baseline event.

Additionally, if you barely missed the goal in 2016, a full 5 percent improvement may not be needed. If your glucose was 102, you only need to reduce it to the goal level of 99 to earn credit for glucose.

Improvement levels are not intended to identify risk or medical appropriateness. Always consult with your medical doctor before starting any new exercise or nutrition program. If you participated in the 2016 screenings, Bravo will automatically review your results to see if improvement was made.

What if I do not have prior year results?If this is your first year participating in the program, you are only required to submit results for the five biometric measures listed under Biometric Goals (page 2). As these results are for informational purposes only, you are not required to meet specific goals to earn the incentive.

If you do not meet an individual biometric outcome goal, but improved by the amount shown below since your last screening, you will earn credit for that metric. You must meet or improve in three of five health metrics to earn your incentive.

WELLNESS PORTAL ACCESS DIRECTIONS

Create an AccountVisit www.bravowell.com/independentbank to begin the “Invest In Your Health” wellness program.

Step 1. Complete RegistrationBy providing your updated information, you are ensuring your results will be delivered quickly and accurately.

Step 2. Complete the Health Risk AssessmentThe health risk assessment (HRA) is an added feature to provide you with additional information about your current health status, areas of risk and suggestions on how to improve your health. This information is located in your results letter (on the wellness portal) attached to your personal record.

Step 3. Choose Your Wellness Activity RequirementsComplete two wellness activities to earn credit for the “Activities” portion of the wellness program. You can find these wellness activities at www.bravowell.com/independentbank.

Step 4. Select Your Biometric Screening OptionYour health is important! This is why your employer is giving you the opportunity to participate in a health screening. You can schedule your screening on the wellness portal or find resources to complete this screening at a time that is convenient for you.

Filing an Appeal

SCREENING RESULTS & SUPPORT

Why would I file an appeal?There are a two reasons for filing an appeal.

1. You are disputing the accuracy of your results.

2. Your healthcare provider feels it is unreasonably difficult due to a medical condition or they feel it is medically inadvisable for you to meet the employer’s goals or alternative.

How soon should I file an appeal?Appeals must be filed by the deadline date listed on your results letter posted to your wellness portal, which is typically 30 days. Submit your appeal and supporting documentation to Bravo via fax, mail or email.

What paperwork is needed to file an appeal?1. A completed appeal form with documentation and signature by an approved provider is required. Approved providers include: Doctor of Medicine (M.D.), Doctor of Osteopathy (D.O.), Physician Assistant (P.A.), or Nurse Practitioner (N.P.). If you are filing an appeal due to pregnancy, a midwife (CNM) is also considered an approved provider. 2. Copies of any additional testing results and supporting documentation may be required. The details for filing an appeal are outlined on your appeal form.

Where do I obtain an appeal form?An appeal form can be printed from your wellness portal at www.bravowell.com/independentbank or by calling Bravo toll-free at 877.662.7286.

Who pays for the retesting when an appeal is submitted?Any retesting will be at your expense.

When will I be notified of the decision of my appeal?A notification will be sent through the wellness portal within three to seven business days that your appeal has been received and is being processed. Appeals can be resolved typically within 10 to 15 business days unless additional documentation or results are required.

When will the financial adjustment be made?If the appeal is approved, it will be as if you passed the category originally.
