W e are pleased to present your 2017 Open Enrollment information. As in past years, we encourage you to attend an open enrollment meeting that is convenient to your schedule and location. We will have Human

Resources staff, as well as vendor representatives available to answer your questions. These meetings are not only to review the benefit choices for 2017, but also to provide information on all programs offered.

This newsletter provides an overview of the benefit plans effective January 1, 2017. It also describes programs and resources available to you to help you be an informed healthcare consumer. Please read it thoroughly and be sure to take action by November 30th to select your benefits for 2017.

Each year we review the value of our health insurance plans and the impact that regulatory or legislative changes have on our policies. We continuously strive to provide you with a comprehensive and flexible benefits program to keep costs manageable for everyone. Both the County and our employees continue to share in the cost of most benefit programs, with the County continuing to absorb the majority of the costs.

Highlights of Your 2017 Benefit Plans:

The County of Montgomery is pleased to announce there will be no increase to the employee benefit contributions for 2017, even though the cost of our benefit plans have increased.

Basic/Supplemental Life and AD&D Insurance and Long-Term Disability: Beginning January 1, 2017 our Life and Disability insurance will be administered by The Hartford Group.

PA 529 College Savings Program: Sign up and take advantage of contributing through payroll deductions

Express Scripts Home Delivery Program: If you no longer want to participate in the Express Scripts Home Delivery program, you must opt-out during the Open Enrollment.

Flexible Spending Account(s): Your spending account elections do not roll over from year to year. If you would like to participate a Flexible Spending Account for the 2017 plan year, you must make your election during the Open Enrollment period.

Supplemental Life Insurance

During this enrollment period only: An employee may elect up to $500,000 in $10,000 increments, up to the Guarantee Issue Amount ($200,000 or 5

times annual earnings, whichever is less) without providing Evidence of Insurability.

An employee may elect coverage for their spouse up to the Guarantee Issue Amount ($50,000) without providing Evidence of Insurability.

An employee may elect coverage for their dependent child(ren) up to $7,500 for each eligible child from birth to 26 years old.

Please Note: There will be no changes to employees’ current supplemental amounts for their self or dependents going forward unless they elect an additional amount during the Open Enrollment period.

The County of Montgomery will continue to provide all regular full-time employees with Basic/Supplemental Life and Accidental Death and Dismemberment (AD&D) and Long Term Disability coverage for the 2017 plan year. Basic Life and AD&D as well as Long Term Disability will now be provided through The Harford Group and continue to be no cost to you.

Through Member Services, you can obtain account information, refill prescriptions using our automated system, set up payment options and more. There are also special support services for the blind and hearing-impaired. The ESI toll-free Member Services number is on your member ID card and your home delivery prescription bottle.

Express Scripts pharmacists are available anytime.

All ESI pharmacists are available anytime, day or night, to speak with you about your medication. To speak with a specialist pharmacist, call Member Services at 1-800-711-0917 and ask the representative to transfer you to a pharmacist who handles your specific condition.

Access details about your prescription benefit, manage your prescriptions and get health and wellness information at Express-Scripts.com. Plus, you can easily order refills of your home delivery prescriptions, check the status of your orders and review your prescription history. With the Express Scripts mobile app, you can also access these services on your iPhone®, BlackBerry® or Android™ smartphone. Go to your smartphone’s app store, search for “Express Scripts” and download the app for free today.

Prescription Drug Coverage

W hen you enroll in one of the IBC medical plans, you automatically receive prescription drug coverage through Express Scripts

Pharmacy Management Formulary Program. The program provides a defined list of FDA-approved medications chosen for their medical effectiveness and value. The formulary list includes both generic and brand-name drugs. Your share of the cost will always be less for drugs that are on the formulary list; however, coverage is available for many non-formulary drugs.

Keep in mind that generic drugs are as safe and effective as their brand name counterparts, and significantly less expensive. If you are taking several medications, the difference in cost for generics and brand name drugs can be significant. When you need a new prescription, ask your doctor whether a generic can be substituted for a brand name. Also check your local pharmacies and supermarkets for discounts on generic brands.

Generic Medications: Keep You & Your Wallet Healthy

If you do not want to participate in the Express Scripts Home Delivery program, you must opt-out during Open Enrollment. You may call Express Scripts at 1-800-711-0917 to do so. Members who do not make a

selection to opt in-or-out of Home Delivery will pay full cost at retail pharmacy for maintenance medications. A new Rx will be required.

Please note: When opting-out of the Home Delivery program, you must also un-enroll in the Worry Free Refill Program.

Medical Benefits

W e will continue to offer medical coverage through Independence Blue Cross (IBC) for the 2017 plan year.

Women’s Preventive Health Care

The health care reform law (Affordable Care Act) requires health plans to cover certain preventive care services for women without any cost-sharing, such as deductibles, copayments or coinsurance. The following items are included in this coverage:

Well-woman visits (annual preventive care visit in which adult women obtain recommended preventive services)

Gestational diabetes screening for women 24 to 28 weeks pregnant, and women at high risk

Human papillomavirus (HPV) testing for women 30 and older, once every three years

Annual counseling for HIV and sexually transmitted infections, plus annual HIV testing for all sexually active women

Contraceptives and contraceptive counseling

Breastfeeding support, supplies and counseling

Domestic violence screening and counseling

Though plans are required to provide these services free of charge, they do have the option of using cost-control measures, such as requiring you pay for a brand name drug if a comparable generic drug is available, or charging a copayment for preventive services received at out-of-network facilities.

It is important to keep up-to-date with your annual physical, which is covered at 100% in-network.

GOOD TO KNOW: ER versus Urgent Care

When to Use Urgent Care

Urgent care centers are usually more cost-effective than ERs. In addition, the waiting time in urgent care centers is often much shorter. Some examples of conditions that might be better handled at an urgent care center are:

Ear infections

Sprains

Urinary tract infections

High fever or vomiting

When to Use the ER

You should go directly to the nearest emergency room if you experience any of the following:

Chest pain

Shortness of breath

Severe abdominal pain following an injury

Uncontrollable bleeding

Confusion or loss of consciousness, especially after a head injury

Poisoning or suspected poisoning

Serious burns, cuts or infections

Inability to swallow

Seizures

Paralysis

Broken bones

More than 10 percent of all emergency room visits could have either been addressed in an urgent care facility or solved in a doctor’s office. But how can you determine which is more appropriate for your condition?

This digital diary is automatically updated each time your provider submits a claim for things like office visits, surgeries, tests, immunizations, and certain lab results — like those cholesterol levels you have a hard time remembering. You can update your Personal Health Record with additional information, such as over-the-counter medications you take, and set up reminders for annual doctor’s appointments and tests.

Dental Plan

D ental benefits help you maintain good oral health, which has been linked in studies to good overall health. The County of

Montgomery will continue to provide you with dental benefits for the 2017 plan year through Aetna.

Dental PPO Plan

The PPO plan allows you to access both in- and out-of-network providers. Remember, you will maximize your benefits and reduce your out-of-pocket costs by utilizing a participating dentist. If you decide to use a non-participating dentist, benefits will be paid using the Usual, Customary and Reasonable (UCR) dental allowance and you may be balance billed for amounts that exceed the UCR limit.

Flexible Spending Accounts (FSAs)

A Flexible Spending Account (FSA) is an easy and convenient way to get more out of your paycheck. It allows you to set aside a pre-determined amount of your pre-tax dollars to cover certain out-of-pocket expenses as they

occur throughout the plan year. Two types of accounts are available—Health Care Spending Account and Dependent Care Spending Account.

Health Care Spending Account

A Health Care FSA lets you pay for medical and dental care expenses not covered by your insurance plan with pre-tax dollars. For example, cash that you now spend on deductibles, copays, or other out-of-pocket medical expenses can instead be paid for with the Medical FSA with pre-taxed dollars. The annual maximum contribution to the Health Care FSA is $2,500.

Dependent Care Spending Account

The Dependent Care FSA lets you use pre-tax dollars toward qualified dependent care, such as child care and elder care expenses. The annual maximum amount you may contribute is $5,000 (or $2,500 if married and filing separately) per calendar year.

Use It or Lose It! As you think about your FSA for this plan year, be sure to

carefully estimate your expenses, as well as the

amount you want to contribute to your account. The IRS has a “use it or lose it” rule, which

means if you don’t spend everything in your FSA by the

end of the plan year, you’ll forfeit whatever funds remain.

Vision Plan

T he three Keystone HMO Plans automatically include vision benefits through Davis Vision. This benefit covers hardware up to $35 and an eye exam every 24 months. To find Davis Vision providers, you can visit this website

For those employee enrolled in the Personal Choice Plan they receive their vision benefits through EyeMed Vision Plan. Eye examinations are covered once every 12 months for children under 19 years of age and once every 24 months for adults 19 years of age or older. Qualified Life Events

T he choices you make during Open Enrollment will be in effect for the entire year. During the year, you may only make

changes if you experience a qualified status change, known as a “life event”. Some examples of life events are:

Birth or adoption of a child

Marriage

Divorce and/or legal separation

Death or loss of a dependent (including loss of dependent status)

Change in your spouse’s employment status causing loss or gain of benefits coverage

Change in your own employment status causing a loss/gain of benefits coverage

Eligibility for Medicare

You must notify the County of Montgomery’s Human Resources Department within 31 days if one of the events listed occurs, so that we can provide you and your family with the necessary paperwork to continue coverage.

Eligibility and Enrollment

Who Is Eligible to Enroll?

You are eligible to enroll in the employee benefits program if you are a regular, full-time employee. To learn more about benefits eligibility and to obtain enrollment forms, please contact the County of Montgomery’s Human Resources Department.

Dependent Eligibility

You may enroll your eligible dependents when you enroll yourself. Dependents who are eligible for benefit coverage include:

Your legally married spouse

Your dependent children

Your domestic partner

Included in the definition of dependent child(ren) are:

Your naturally born child(ren), legally adopted child(ren), step-child(ren) who reside in the same household as the employee or court-ordered dependent child(ren) for whom you are the court-appointed legal guardian

Your dependent child(ren) up to age 26, whether they are a full time student or not, for all plans. Coverage ends at the end of the month following the date they turn 26

Your continuously disabled dependent child(ren) [if disabled prior to age 26] who are incapable of self-sustaining employment and dependent upon you for support, regardless of age

How to Enroll

Complete your enrollment forms and make sure they are received by Human Resources within the Open Enrollment Period of November 1st—November 30th.

What if I Don’t enroll?

Your benefit elections from 2016 will rollover automatically, but will reflect the 2017 contribution amounts. Please Note: You must re-enroll each year for your Health Care and Dependent Care Spending Accounts.

Group Off-the-Job Short-Term Disability Insurance Is your existing coverage enough?

Additional Short Term Disability can provide additional protection from financial hardship if you become disabled.

Features include: Up to 60% of your weekly pre-disability earnings (up to 40% in states with state disability) up to a maximum benefit of $6,000 per week.

Benefits are payable after the elimination period and continue for up to 3 months. You can select a 0/7 (accident/sickness) or 14/14 (accident/sickness) elimination period. For example, if you select a 0/7 elimination period this means benefits are payable immediately for a covered accident and after 7 days for a covered sickness.

Group Accident Insurance Because Accidents are Unpredictable!

Accident Insurance provides benefits for out-of-pocket expenses associated with a covered accident such as emergency room treatment, broken bones, physical therapy, doctor visits, and more. Benefits are paid directly to you, unless otherwise assigned in addition to any other benefits received. A high and low plan are available.

Group Critical Illness Insurance Are you protected?

Critical Illness Insurance helps protect you and your family members against the financial strains/expenses of a Critical Illness.

Features include: Lump sum benefit paid upon initial diagnosis of each covered illness and a $50 Annual Health Screening Benefit.

Benefits are paid directly to you, unless otherwise assigned in addition to any other benefits received and may be used for any purpose.

Help Protect your savings – if diagnosed with a Critical Illness, focus on recovery, not finances!

Group Supplemental Hospital Indemnity Insurance Does your major medical insurance cover all of your bills?

Supplemental Hospital Indemnity Insurance can help provide financial assistance to enhance your current coverage.

Features include: 2 available plans with various benefit options:

Plan 1 provides benefits for hospital confinement, hospital admission and intensive care.

Plan 2 provides benefits for hospital confinement, hospital admission, and intensive care as well as ER/physician visit, out-of-hospital prescriptions and well baby care.

V oluntary benefits give you an opportunity to enhance your other coverage by providing important benefits that help protect you, your family, and your assets in the event of the unexpected. These benefits, which you pay for

through an a group premium, supplement your other health insurance when you need it most—in the case of a cov-ered disability, critical illness, hospitalization and/or accident. These plans pay cash directly to you, unless otherwise assigned and you can use the cash wherever it’s needed, for example, helping with out-of-pocket medical costs, home healthcare, travel to and from treatment facilities, childcare, and more.

2017 Health Care Reform Update

T he Affordable Care Act (or ACA) continues to affect health insurance plans for employers, like County of Montgomery, and County of Montgomery employees. For the company, it means we continue to:

Comply with all applicable health plan coverage, administration, and tax reporting requirements of the ACA.

Pay all applicable taxes and fees as required by the ACA.

For individual employees, the law requires most individuals to have health insurance or pay a tax penalty. If you enroll in an County of Montgomery medical plan, you will meet the ACA’s requirement for health coverage. County of Montgomery pays the majority of the cost for this coverage.

If you do not enroll in a County of Montgomery medical plan, you have other options, as shown below. We encourage you to evaluate all your options and compare their costs to make the best choice for you and your family.

Elect coverage through your spouse’s employer.

Participate in a federal or state program such as Medicare or Medicaid (if eligible).

Elect coverage through a plan you purchase through the Health Insurance Marketplace (www.healthcare.gov).

It’s important to note that because you are eligible for coverage through County of Montgomery, you may not qualify for any subsidies if you purchase a plan through the Marketplace—that means you would pay the full cost of that coverage.

If you do not obtain coverage through County of Montgomery or another source, you may be subject to a penalty on your taxes. For 2016 the annualized penalty is the greater of:

A) 2.5% of your yearly household income above the tax filing threshold (up to the national average premium for a bronze plan in the individual insurance marketplace ($2,676 for an individual and $13,380 for a family of 5)), OR

B) $695 per adult and $347.50 per child under age 18 (up to $2,085 per family).

For 2017, the fixed dollar amounts in a) above will be based on average 2017 marketplace premiums and the fixed dollar amounts in b) above will be adjusted for inflation.

You can find the Summary of Benefits and Coverage (SBC)—the format required by the Affordable Care Act—for the Independence Blue Cross medical and Express Scripts Rx plans on the County of Montgomery intranet. These summaries may be helpful to provide more information about County of Montgomery’s medical and Rx plans, or to compare our plans to others, such as plans available to you through your spouse’s employer. You may also request a paper copy by contacting Human Resources.
