How you save.The County’s wellness program is voluntary. However, if you enroll in the medical plan but choose not to participate, you will miss out on the chance to earn significant financial incentives. If your or your spouse’s medical effective date is May 2, 2019 or later, you are exempt from these activities and will automatically earn the appropriate incentive(s) the current and next year.
Wellness Incentive:To qualify for the wellness incentive(s), the employee and/or spouse must complete one of the two options listed below no later than September 3, 2019. Spouses can only participate if they are on the employees medical plan.
1. Attend an on-site health screening or
physician exam and submit a provider
screening form using the Bravo portal.
2. Complete the Bravo health risk assessment or Voya financial assessment. Spouses are exempt from this activity.
3. Achieve the desired range for at least four of five biometrics in the following categories:
Body Mass Index ≤ 27.5 Waist measurement automatically corrects elevated body mass index (BMI) due to lean muscle mass, even if you fail the BMI goal. (Female ≤ 35 inches, Male ≤ 40 inches)
Blood Pressure ≤ 130/85
HDL Cholesterol ≥ 50 Female; ≥ 40 Male

FAQ: Appeals
What if I don’t meet my program goal? Is there an alternative?
Since your program requires the achievement of goals, there may be alternative ways
to qualify. You will be provided the information you need to request an alternative
goal in your results letter. Alternatives are typically based on improvement from prior
results or on completion of a program that is related to health improvement within a
certain biometric. Contact Bravo and we will work with you (and, if you wish, with your
physician).
Where do I obtain an appeals form?
You can find and download an appeals form by visiting your participant portal at
www.bravowell.com/johnsoncountyks or by calling Bravo toll-free at 844.275.4731 to
receive it via email or mail.
How soon should I file an appeal?
You should begin working with your physician to complete the appeal form and provide
any additional documentation needed as soon as you know you did not meet the desired
range for 4 of 5 lab values. Medical appeals must be filed by September 30, 2019. Submit
your appeal and supporting documentation via fax, by mail or by email.
Who pays for the retesting when an appeal is submitted?
Any retesting will be at your expense.
What if my results are not consistent with recent tests?
In the event that screening results differ significantly from recent medical results, a Type 1
(dispute of accuracy) appeal may be submitted with supporting documentation before
the September 30, 2019 deadline.
What if I have a medical condition and I cannot meet the goal or the alternative
goal (if applicable)?
If the County’s goal(s) or reasonable alternative goal(s) are considered unreasonably
difficult due to a medical condition or medically inadvisable (Type 2 appeal), Bravo
manages appeals and coordinates personalized alternatives for these goals. Contact us
for more information.

1. Create a Bravo Account & Register for the Bravo Program
Visit the URL above and follow the step-by-step instructions to create an account,
or log in if you have accessed the site in the past. Complete the registration step
by providing Bravo with your updated information, ensuring your results are
delivered quickly and accurately. You will be able to view and track your incentive
status through this account.
2. Complete Your Screening One of Two Ways:
A. Schedule an On-Site Screening Appointment
Follow the step-by-step instructions to schedule your screening
appointment. The County is offering limited on-site screenings at
pre-determined locations county-wide.
B. Screen with Your Provider (preferred by the County)
You can also opt to download the provider screening form to visit your own
provider or an approved clinic.
3. Complete Bravo’s Health Risk Assessment or Voya’s Financial Assessment
(Employees Only • Spouses are exempt from this activity)
Bravo’s Health Risk Assessment (HRA) blends your responses to health-habit
questions with biometric data from your screening. (PLEASE NOTE: In order for
this step to be considered complete and to receive a comprehensive report,
full participation in the biometric screening is required. These results and your
questionnaire answers generate a personalized, private report that highlights
information regarding your current health status, areas of risk, and suggestions on
how to improve your overall health.
Voya’s Financial Assessment is an interactive educational experience to help
guide you to a healthier financial life. The Financial Wellness Experience offers
you a comprehensive understanding of where you are financially and what you
can do about it. The experience begins with an assessment of six financial wellness
pillars: 1) debt management, 2) emergency savings, 3) protection, 4) retirement, 5)
spending and 6) saving. Visit voyaretirementplans.com to access.
Once the assessment is completed, you can see a personalized dashboard with
a summary of areas for improvement and the best next steps to take meaningful
action for a stronger sense of control and well-being. This allows you to live for
today, while preparing financially for tomorrow.

FAQ: Program Overview
Does everyone have to participate?
No, participation in this program is voluntary. However, if you choose not to
participate and you’re enrolled in the health plan, you will miss out on the
chance for significant financial rewards, in the form of discounts to your
insurance premium. Employees under age 18 are not eligible to participate.
How will I receive my results?
Visit www.bravowell.com/johnsoncountyks to view your results on your
secure account. You will receive an email notification to alert you when they
are available. Bravo will also mail a results letter to your home, unless you
choose to receive an electronic copy only when you register. If you have not
received your results letter within three weeks of completing your screening,
contact Bravo immediately.
Will my employer see my results?
No, Bravo will not share your individual results with the County. They will be
given a summary of the number of wellness dollars each employee received
as a result of the health screening, but will not know specifics regarding
categories or laboratory results. The County may request that your results be
sent to an approved third party for coaching or to be included in your health
risk assessment.
Do spouses need to be screened?
Since studies show that engaging spouses can help improve the health of
the employee, the County is encouraging spouses to be screened and is
rewarding them for meeting goals.
Is this program legal? Can my employer really base my premium on my
health results?
Bravo administers programs in compliance with the Affordable Care Act’s
(ACA) Incentives for Non-Discriminatory Wellness Programs in Group
Health Plans. This allows employers to adjust healthcare cost for those on
the health plan who meet certain health goals. At Bravo, we also adhere to
the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and
Protected Health Information (PHI) protocols. This ensures that your personal
information, including your results, will not be shared with any unauthorized
parties, including the County.

I recently had a preventive health screening. Can I submit those results?
Bravo will accept results from health screenings that include all required biometrics
and took place on or after January 1, 2019 and before September 3, 2019. To submit
recent screening results, your physician needs to complete a provider form and you
need to submit it to Bravo.
What type of appointment am I scheduling?
When calling your healthcare provider’s office to schedule your appointment,
tell them you would like to schedule a “preventive screening visit” or an “annual
wellness visit.” These are the frequently used industry terms and your provider’s
office should be able to easily accommodate your request. The County’s medical
plan covers one annual exam/ visit per calendar year.
If a blood test is required, what type should I request?
Your doctor’s office should perform a standard lipid panel blood test for you. For
the most accurate results, fasting 9 to 12 hours prior to your health screening
is recommended.
What should I bring with me to the appointment?
Do not forget to bring your provider screening form. Visit www.bravowell.com/
johnsoncountyks to download and print it. Your healthcare provider is required to
fill it out and sign it. The County’ Benefits Department cannot download this form.
Who is permitted to sign the form or complete the screening?
Your provider must be an M.D., N.P., P.A. or D.O. to sign the form. Please remember
that a participant signature is also required to process the form. If you are filing an
appeal due to pregnancy, a certified nurse midwife (CNM) is also considered an
approved provider.
Once completed and signed, can my healthcare provider submit my form?
Your healthcare provider can fax the form and lab work results to Bravo on your
behalf. However, as the participant, it is ultimately your responsibility to ensure the
completed form is sent to Bravo on or before September 3, 2019. Bravo recommends
that you submit your results personally and retain a record of your submission.

How do I submit my provider form to Bravo?
1. Scan your completed provider form onto your computer.
2. Log on to your participant portal.
3. Click on the “Upload Document” button.
4. Upload documents and click “Submit.”
You also have the option to fax your completed form to Bravo. The fax number is listed on your provider form.

Have a smooth screening. Follow these steps to ensure a successful screening experience:
Arrive 5 minutes early with your ID. This is required to properly verify your identity.
Fast for 9-12 hours prior to your screening for accurate cholesterol and glucose results. You may have water and black coffee/tea. Take all medications as prescribed.
For an accurate blood pressure result, do not consume tobacco or caffeine 30-60 minutes prior to screening.
For an accurate blood pressure result, no strenuous activity 30 minutes prior to screening and try to be as relaxed as possible at least 5 minutes before.
Drinking 6-8 oz. of water before your screening and making sure you aren’t cold helps make your blood draw easier.

Carefully read and sign your screening form.
Additional screening information.Remove your shoes and empty your pockets. The
health professional will want to make sure your
height and weight are accurately captured. Height
will be recorded to the nearest 1/4 inch. Weight will
not be rounded.
The waist measurement will be taken at the navel
using a 360 degree spin method, rounding to the
nearest 1/4 inch.
Your pulse will be recorded for 60 seconds.
The blood draw will take place after your blood
pressure reading has been recorded. No more than
three attempts at the blood draw should be made.
Both the second and third attempts require your
approval.
If you can, wear a shirt that allows you to have the blood pressure cuff directly on your skin.
Dress Appropriately
A second blood pressure reading will be taken only if
your first reading is greater than or equal to 120/80.

BMI: your body’s check engine light
Your weight to height distribution, or
BMI, gives you a glimpse of your overall
health. It’s not a perfect measure, but if
your BMI is out of the healthy range and
you’re not a bodybuilder, your weight
may be putting you at risk for a number
of health problems.
Blood pressure: the traffic
in your arteries
Like on a highway, too much traffic in
your arteries causes accidents. When
your blood pressure is high, the tubes
that transport your blood get blocked or
break easily from the force of the blood
pushing against them.
Cholesterol: the trash truck
Processed foods are often full of bad
cholesterol that can clog your arteries.
But good cholesterol in beans, grains
and fruits scoops up the bad cholesterol
and dumps it into your liver so it can be
removed from your body.
Glucose: brain food
When you eat more carbs than your
body needs, the leftovers get turned
into glucose. Your brain and many
other cells need glucose for energy,
but too much can lead to diabetes and
other serious diseases.
Triglycerides: battery packs
Our bodies are built to store up
energy for later in fatty blobs called
triglycerides. Most of the time, we
don’t need the extra energy, so extra
triglycerides, stored in fat cells, get in
the way by blocking our arteries if we
don’t burn them through exercise.

EEOC Privacy Notice
Federal law requires employers that offer wellness programs that collect employee health information to provide a notice to employees
informing them what information will be collected, how it will be used, who will receive it, and what will be done to keep it confidential. The
notice below fulfills these requirements.
Notice Regarding Wellness Program
Johnson County has contracted with Bravo Wellness, LLC to administer all or part of its voluntary employee wellness program. The
program is administered according to federal rules permitting employer-sponsored wellness programs that seek to improve employee
health or prevent disease, including the Americans with Disabilities Act (ADA) of 1990, the Genetic Information Nondiscrimination Act
(GINA) of 2008, the Affordable Care Act (ACA) and the Health Insurance Portability and Accountability Act (HIPAA), as applicable, among
others. If you choose to participate in the wellness program you may be asked to complete a voluntary health risk assessment or “HRA”
that asks a series of questions about your health-related activities and behaviors and whether you have or had certain medical conditions
(e.g., cancer, diabetes, or heart disease). You may also be asked to complete a biometric screening or other examinations, which may
include a blood test for cholesterol levels (Total, HDL, LDL), triglycerides, serum cotinine (nicotine) and glucose as well as a blood pressure
reading(s), height, weight, waist measurements and your pulse. When possible, your blood specimen will be confidentially processed by a
laboratory that provides a panel of common preventive wellness measures provided solely for your information.
The information from your HRA and the results from your biometric screening will be used to provide you with information to help you
understand your current health and potential risks, and may also be used to offer you services through the wellness program, such
as targeted health education, online and telephonic health coaching and health challenges. You also are encouraged to share your
results or concerns with your own provider. You are not required to complete the HRA or to participate in the screening or other medical
examinations.
However, if you choose to participate in the wellness program you may receive an incentive for participating. More specific details
regarding the wellness program, including how incentives are earned can be found in the Program Guide.
As noted in the Program Guide, a portion of the incentives available may be linked to certain health-related activities or to the
achievement of certain health outcomes. If you are unable to participate in any of the health-related activities or achieve any of the
health outcomes required to earn an incentive, you may be entitled to a reasonable accommodation. You may request a reasonable
accommodation or an alternative standard by contacting Bravo Wellness at 844-275-4731. See the Program Guide for more details
concerning reasonable alternatives. Additional information will be provided to you in your results summary as well.
Protections from Disclosure of Medical Information
We are required by law to maintain the privacy and security of your protected health information (PHI). Although the wellness program
and the County may use aggregate information collected to design a program based on identified health risks in the workplace, Bravo
Wellness and its contracted partners will never disclose any of your personal medical information either publicly or to the employer,
except as necessary to respond to a request from you for a reasonable accommodation needed to participate in the wellness program,
as necessary to support health plan or wellness program administration or as permitted by law. In no event will medical information that
personally identifies you that is provided in connection with the wellness program be used to make decisions regarding your employment.
Your health information will not be sold, exchanged, transferred, or otherwise disclosed except to the extent permitted by law to carry
out specific activities related to the wellness program, and you will not be asked or required to waive the confidentiality of your health
information as a condition of participating in the wellness program or receiving an incentive. Anyone who receives your information for
purposes of providing you services as part of the wellness program will abide by the same confidentiality requirements.
In addition, all medical information obtained through the wellness program is required to be maintained separate from your personnel
records, information stored electronically will be encrypted, and no information you provide as part of the wellness program may be used
in making any employment decision. Appropriate precautions will be taken to avoid any data breach, and in the event a data breach
occurs involving information you provide in connection with the wellness program, we will notify you immediately. You can ask to see or
get a copy of the health information we have about you. We may charge a reasonable cost-based fee.
You may not be discriminated against in employment because of the medical information you provide as part of participating in the
wellness program, nor may you be subjected to retaliation if you choose not to participate.
If you are a member of the County-sponsored health plan, the provisions of the health plan privacy notice may also apply. Please contact
your health plan administrator for a copy of the notice. If you have questions or concerns regarding this notice, or about protections
against discrimination and retaliation, or if you would like a paper copy of this notice or a copy of Bravo’s Privacy Statement mailed to
