What is Open Enrollment?Open Enrollment is your once-a-year opportunity to make benefit changes for you and/or your eligible family members. This is your chance to enroll in your 2019 benefits whether you are adding, removing, or changing plans. We encourage you to give careful thought to your needs prior to making any decisions.

The benefits you elect during Open Enrollment will be effective January 1, 2019, and will remain in effect through December 31, 2019, unless you experience a qualifying life event that would allow you to make a mid-year change.

If you do not enroll during this 2019 Open Enrollment period, your benefits will default to your 2018 benefits except your Flexible Spending Account (FSA). You must re-elect your FSA for 2019. If you currently refuse/decline the medical coverage and do not enroll, your election to refuse/decline will remain for 2019.

Benefits At-a-Glance2019 Open Enrollment Guide

MEDICAL PLANPremiums for full-time employees are paid by Collier County Public Schools. Premiums for your dependents are paid by the employee. Detailed benefits for the plan can be found in the Summary Plan Description (SPD) and Summary of Bene-fits and Coverage (SBC). These documents provide plan participants with information on how the medical plan operates, when an employee can begin to participate in the plan, how services and benefits are paid, covered benefits and exclusions, deductibles, co-pays and co-insurance. Both documents are located on the Benefits website at www.collierschools.com/Page/5318.

In-Network/Out-of-NetworkAn in-network provider is a physician or medical facility that is part of the group of contracted providers. Using an in-network provider offers cost-savings advantages because a covered person pays only a percentage of the scheduled fee for services provided. A covered person who goes to an out-of-network provider will pay more for the services rendered and his or her share of the cost may not apply to the out-of-pocket maximum.

Pathways to Enhanced Health Incentive ProgramPathways is a voluntary incentive-based wellness program which allows a covered employee to qualify for lower out-of-pocket costs (incentives) by participating in a variety of research-based wellness activities. These activities are designed to improve health awareness, support and maintain good health and address chronic disease or serious medical conditions. Covered employees who choose not to participate in wellness activities will remain in the Basic Pathway for the subsequent plan year. Covered dependents will be enrolled in the same Pathway as the employee. The three Pathways are:

• Basic Pathway - no incentives• Custom Pathway - partial incentives• Enhanced Pathway - full incentives

In 2018, two enhanced benefits were added to your medical Plan:

• Emotional Wellness Program (through Community Health Partners) Counseling services and support at NO cost to employees and their family members. Licensed therapist available to meet Monday through Friday at the Administration Center or CHP offices.

• MD LIVE Consult with a doctor for non-emergency conditions such as cold/flu, fever, nausea, rash, UTI and more without leaving home! Consultations by phone, secure video or MDLIVE app anytime, anywhere. Average wait time is 10 minutes to see a state-licensed, board-certified physician. Family members enrolled in the Plan are eligible and pediatricians are available 24/7.

If you and your family members are enrolled in the District’s medical Plan, you have access to both of these benefits.

FLEXIBLE SPENDING ACCOUNTS (FSA)

• Up to $2,650 ($132.50 per paycheck) can be deducted pre-tax to pay for eligible health care expenses.

• Up to $5,000 ($250 per paycheck) can be deducted pre-tax to pay for eligible child care expenses.

• Over-the-counter items will only be eligible if they are prescribed. As always, retain copies of receipts for all FSA expenses in case you are audited.

• Includes a debit card that can be used to provide immediate access to FSA funds (medical only) at the point of purchase.

• Allows up to $500 carryover of unused funds from current to subsequent year. (Medical only)

• Employee selects an in-network dentist. Family members are not required to choose the same dentist as the employee; however the dentist must be in-network. (Network may be limited to Florida.)

• Fixed schedule of co-payments.

• No pre-existing condition limitations.

BASIC LIFE/ACCIDENTAL DEATH & DISMEMBERMENT• The District provides basic life insurance in the amount of one times your salary or a minimum of $30,000

up to $100,000.

SUPPLEMENTAL EMPLOYEE LIFE• Employees have the option of purchasing $20,000 of supplemental life insurance.

VOLUNTARY GROUP TERM LIFE AND AD&D• Choose from $10,000 up to $500,000 for yourself and/or your spouse. (evidence of insurability may apply)• The benefit amount does not have to be the same and your spouse can be covered without the employee.• Employee and/or spouse must be covered in order to cover children. Employee and spouse cannot have dual

coverage on children• Spouse coverage cannot exceed employee coverage.

UNIVERSAL LIFE WITH LONG TERM CARE BENEFITS• Coverage is available to you, your spouse, your children, and your grandchildren.• Provides a death benefit to your beneficiaries and a living benefit to you through a long term care rider.• Modified Guaranteed Issue $21 per week or $200,000 (age 64 max) whichever is less; subject to underwriting

approval; Simplified Issue up to $300,000.

New policies are Simplified Issue only for employees (and their dependents) who previously applied for coverage which has been not taken, declined, cancelled, withdrawn, lapsed, or surrendered and those with current Trustmark coverage.

ACCIDENT• Policy pays cash benefits directly to you, over-and-above any other coverage.• Protects you 24 hours a day and provides benefits for injuries that occur either on or off-the-job.• Wellness benefit of $100 per year payable for doctor visits, chiropractor visits, physical therapy, ER,

hospital stays, fractures and dislocations, ruptured disk, burns, lacerations, slip and fall, etc.• Guaranteed Issue for active employees.• Helps pay for cost of accidental injury. If your child breaks a leg at soccer practice the sports package can increase

the total benefit payment by more than $900
