WelcomeThe MAPP Group, LLC knows how important it is to provide quality employee benefits to our employees and their dependents� We always strive to provide a total benefits package that meets your needs as well as the needs of the company�

Please take the time to read the following benefit summaries carefully� This information will help you in deciding the best benefit selections for you and your family�

At other times during the year, you may request changes ONLY when there is a Family Status Change, and the proposed change is consistent with the Family Status Change event� Family Status Changes include:

• Change in legal marital status (e�g�, marriage or divorce);

• Change in the number of dependents (e�g�, birth, adoption or placement for adoption, death);

• Change in employment status or residency of the employee, spouse or dependent that affects eligibility;

• Change in coverage under another employer’s plan�

Changes, additions or voluntary cancellations generally cannot be made during the plan year unless the employee experiences a Family Status Change� You have 30 days from a change in family status to make changes to your current coverage� Otherwise, the employee must wait until the annual enrollment period to change or cancel an election�

Any employee who has health, dental, or vision insurance through The MAPP Group, LLC who has a premium deducted from their pay, can have their insurance premiums paid through our Premium Conversion Plan� As allowed by the IRS Code- Section 125, this benefit allows you to pay your payroll deducted insurance premiums with “pre-tax” dollars� As a plan participant, the employee’s share of the health, dental, and vision premiums will be paid on a pre-tax basis through salary reduction� Under the Plan, once an election is made that election will continue unless a change in election is made due to a Family Status Change, annual enrollment, or termination of employment�

As a new employee, you have the opportunity to enroll in the Mapp Group, LLC employee benefits� The benefits you elect may be subject to carrier approval and will remain in effect until January 31, 2021�

Virtual VisitsWith UnitedHealthcare, you no longer have to spend time or money assessing, selecting, and implementing an external virtual visit provider� We seamlessly integrate virtual visits into your health plan� The virtual visit provider groups we contract with deliver care using live audio and video technology based on quality standards aligned with American Medical Association (AMA) and Federation of State Medical Boards (FSMB) guidelines�

When employees are sick they often miss work� With virtual visits, their covered family members can see and speak to a doctor 24 hours a day/7 days a week using a mobile device or computer, wherever they are� If needed, employees and prescription drugs can also be sent to a local pharmacy� This service is especially beneficial to those members living rural areas who may have difficulty accessing care� In most scenarios, scheduling a virtual visit will save time and money� Virtual visits are covered under member health plans administered by UnitedHealthcare with some member cost share�

Access Virtual VisitsLog in to myuhc�com and choose from provider sites where you can register for a virtual visit� After registering and requesting a visit you will pay your portion of the service costs according to your medical plan, and then you will enter a virtual waiting room� During your visit you will be able to talk to a doctor about your health concerns, symptoms and treatment options�

Use Virtual Visits When:• Your doctor is not available

• You become ill while traveling

• You are considering visiting a hospital emergency room for a non-emergency health condition

Not Good For:• Anything requiring an exam or test

• Complex or chronic conditions

Real Appeal is a revolutionary 52-week, fun and engaging, interactive, intensive lifestyle intervention program, centered on weight loss and keeping it off� It is delivered face-to-face, live over the internet, through live virtual coaching, and online group participation�

Members get everything they need for success, except an excuse! Best of all, it’s offered at NO COST to members!

Adult members with at risk BMI (over 30) or BMI between 25-29 with a comorbidity (for example: Diabetes, High Blood Pressure, Tobacco User) are eligible for this 52-week program with UHC covering the claims cost. As the member participates, claims are filed with UHC at no member cost share.

Health Savings AccountA Health Savings Account, or HSA, can be a valuable tool to help you save money for medical expenses for you and your family� An HSA is a tax-exempt account that can be used to pay or reimburse certain medical expenses you incur�

Employees who choose to participate in the HDHP (High Deductible Health Plan) may also open a Health Savings Account (HSA)� The MAPP Group, LLC will contribute to your HSA account to offset the high deductible plan� Please see MAPP contributions below� Health Savings Account Dollars will be accessible via an HSA debit card� The HSA debit card can be used for qualified medical, dental, and vision expenses as defined by the Internal Revenue Service Publication 502 (You may reference www�irs�gov for the complete list)� The HSA plan will be administered by Optum Bank�

HSA monies are exempt from federal income taxes and may be exempt from State income taxes as well� They belong to the employee and can be rolled over to subsequent years� HSA funds may also be invested in the same type of funds as IRA’s and 401(k)’s�

Important Notes:• If you or your spouse participate in a health FSA or an HRA that pays or reimburses qualified medical expenses

Health Savings Account cont.How to Open your HSA Account:It only takes minutes to enroll in a health savings account (HSA) from Optum Bank, Member FDIC�

1� Go to www�optumbank�com and select “Open an HSA”�

2� Read through the page to confirm your eligibility and select “Continue”�

3� Complete your online enrollment�

When enrolling, you can choose to receive your welcome kit electronically or by mail� You will also receive a health savings account Debit Master Card by mail within ten days in an unmarked envelope�

If you have family coverage, you can order extra debit cards for your spouse and dependents on your plan when enrolling in your account� You’ll need each cardholder’s name, Social Security number and date of birth�

Flexible Spending AccountThe MAPP Group, LLC offers you the opportunity to pay for out-of-pocket medical, dental, and dependent care expenses with pre-tax dollars through the Flexible Spending Account� The plan is administered by UnitedHealthcare.

B� Dependent Care Expense: The Internal Revenue Service allows you to set aside money BEFORE taxes to reimburse yourself for Dependent Care Expenses each plan year up to a maximum of $5,000, if your tax filing status is Single or Married Filing Jointly� You may elect up to $2,500 if Tax Filing Status is Married Filing Separate. Again, this results in a TAX SAVINGS to you!!!

Important to Remember• Any monies placed in the Medical Reimbursement Account or Dependent Care Account not spent within the plan year

are forfeited – “Use it or Lose it”�

• Should your employment terminate during the year, you have 30 days from your term date to file claims for eligible expenses received prior to your term date�

• Due to the Patient Protection and Affordable Care Act, employee will NOT be able to receive pre-tax reimbursements from their FSA for non-prescribed over-the-counter medications� OTC drugs, medicine and biologicals will be eligible for reimbursement only if the request is accompanied by a doctor’s prescription� This means items such as cough medicine, pain relievers, acid controllers, to name a few, will no longer be reimbursed unless there is a doctor’s prescription submitted along with the reimbursement request� Please contact your FSA administrator for more details�

This benefit is regulated by the Internal Revenue Service and consists of three parts to help you pay your premiums and out-of-pocket health and dependent care expenses with pre-tax dollars, which lowers your tax liability. The Cafeteria Plan

Enrollment� Employees are unable to enroll / change at any other time during the year unless a qualifying event occurs�

A� Medical Reimbursement: The Internal Revenue Service will allow you to “set aside” money from your paycheck BEFORE taxes to “reimburse yourself” for certain items not typically covered under your health, dental or vision insurance� This results in a TAX SAVINGS to you! Examples of things that you can claim for reimbursement include: Medical and drug deductibles and coinsurance, contacts or eyeglasses, hearing aids, dental expenses (whether you have dental insurance or not), etc� The maximum allowed for 2020 is $2,750.

Basic Life & AD&D PlanThe MAPP Group, LLC provides full-time eligible employees with $10,000 Life and Accidental Death & Dismemberment Insurance� This policy is offered through UNUM and is 100% employer-paid� You will want to keep your Beneficiary information current at all times�

Benefits reduce 65% of original amount at age 70 and 50% of original amount at age 75�

Voluntary Term Life PlanEmployee: All full-time employees can purchase Voluntary Life and Accidental Death & Dismemberment (AD&D) coverage in increments of $10,000 to a maximum of 5 times salary not to exceed $500,000� This benefit is provided through UNUM and is 100% Employee paid� Elections in excess of the $110,000 guarantee issue amount will be subject to medical evidence of insurability and will be pended until approved by UNUM�

In order to purchase Life coverage for your spouse and/or child, you must purchase Life coverage for yourself�

Short-Term Disability The MAPP Group, LLC provides full-time employees an opportunity to purchase Short-Term Disability coverage through UNUM� In the event you become disabled as a result of a non-work related accident or illness, tax-free Disability Income Insurance benefits are provided as a source of income� You are not eligible to receive Short-Term Disability benefits if you are eligible to receive workers’ compensation benefits�

If you are enrolling in the STD coverage for the first time and you are not a new hire, you will be subject to evidence of insurability and must complete additional forms�

Important Legal Notices Affecting Your Health Plan Coverage THE WOMEN’S HEALTH CANCER RIGHTS ACT OF 1998 (WHCRA)

If you have had or are going to have a mastectomy, you may be entitled to certain benefits under the Women’s Health and Cancer Rights Act of 1998 (WHCRA). For individuals receiving mastectomy-related benefits, coverage will be provided in a manner determined in consultation with the attending physician and the patient, for:

All stages of reconstruction of the breast on which the mastectomy was performed; Surgery and reconstruction of the other breast to produce a symmetrical appearance; Prostheses; and

NEWBORNS ACT DISCLOSURE - FEDERAL Group health plans and health insurance issuers generally may not, under Federal law, restrict benefits for any hospital length of stay in connection with childbirth for the mother or newborn child to less than 48 hours following a vaginal delivery, or less than 96 hours following a cesarean section. However, Federal law generally does not prohibit the mother’s or newborn’s attending provider, after consulting with the mother, from discharging the mother or her newborn earlier than 48 hours (or 96 hours as applicable). In any case, plans and issuers may not, under Federal law, require that a provider obtain authorization from the plan or the insurance issuer for prescribing a length of stay not in excess of 48 hours (or 96 hours).

NOTICE OF SPECIAL ENROLLMENT RIGHTS If you are declining enrollment for yourself or your dependents (including your spouse) because of other health insurance or group health plan coverage, you may be able to enroll yourself and your dependents in this plan if you or your dependents lose eligibility for that other coverage (or if the employer stops contributing toward your or your dependents’ other coverage). However, you must request enrollment within 30 days after your or your dependents’ other coverage ends (or after the employer stops contributing toward the other coverage). In addition, if you have a new dependent as a result of marriage, birth, adoption, or placement for adoption, you may be able to enroll yourself and your dependents. However, you must request enrollment within 30 days after the marriage, birth, adoption, or placement for adoption. MICHELLE’S LAW DISCLOSURE MICHELLE’S LAW DISCLOSURE Under the ACA, dependent children are covered by the group health plan until age 26. If your child has extended coverage as a student but loses their student status because they take a medically necessary leave of absence from school your child may continue to be covered under the plan for up to one year from the beginning of the leave of absence. This is available if, immediately before the first day of the leave of absence, your child was (1) covered under the plan and (2) enrolled as a student at a post-secondary educational institution (includes colleges and universities). STATEMENT OF ERISA RIGHTS As a participant in the Plan you are entitled to certain rights and protections under the Employee Retirement Income Security Act of 1974 (“ERISA”). ERISA provides that all participants shall be entitled to: Receive Information about Your Plan and Benefits

Examine, without charge, at the Plan Administrator’s office and at other specified locations, the Plan and Plan documents, including the insurance contract and copies of all documents filed by the Plan with the U.S. Department of Labor, if any, such as annual reports and Plan descriptions.

Obtain copies of the Plan documents and other Plan information upon written request to the Plan Administrator. The Plan Administrator may make a reasonable charge for the copies.

Receive a summary of the Plan’s annual financial report, if required to be furnished under ERISA. The Plan Administrator is required by law to furnish each participant with a copy of this summary annual report, if any.

Continue Group Health Plan Coverage If applicable, you may continue health care coverage for yourself, spouse or dependents if there is a loss of coverage under the plan as a result of a qualifying event. You and your dependents may have to pay for such coverage. Review the summary plan description and the documents governing the Plan for the rules on COBRA continuation of coverage rights. Prudent Actions by Plan Fiduciaries In addition to creating rights for participants, ERISA imposes duties upon the people who are responsible for operation of the Plan. These people, called “fiduciaries” of the Plan, have a duty to operate the Plan prudently and in the interest of you and other Plan participants. No one, including the Company or any other person, may fire you or discriminate against you in any way to prevent you from obtaining welfare benefits or exercising your rights under ERISA. Enforce your Rights If your claim for a welfare benefit is denied in whole or in part, you must receive a written explanation of the reason for the denial. You have a right to have the Plan review and reconsider your claim. Under ERISA, there are steps you can take to enforce these rights. For instance, if you request materials from the Plan Administrator and do not receive them within 30 days, you may file suit in federal court. In such a case, the court may require the Plan Administrator to provide the materials and pay you up to $152 per day (up to a $1,527 cap per request), until you receive the materials, unless the materials were not sent due to reasons beyond the control of the Plan Administrator. If you have a claim for benefits which is denied or ignored, in whole or in part, and you have exhausted the available claims procedures under the Plan, you may file suit in a state or federal court. If it should happen that Plan fiduciaries misuse the Plan’s money, or if you are discriminated against for asserting your rights, you may seek assistance from the U.S. Department of Labor, or you may file suit in a federal court. Important Notice from The MAPP Group About Your Prescription Drug Coverage and Medicare Please read this notice carefully and keep it where you can find it. This notice has information about your current prescription drug coverage with The MAPP Group and about your options under Medicare’s prescription drug coverage. This information can help you decide whether or not you want to join a Medicare drug plan. If you are considering joining, you should compare your current coverage, including which drugs are covered at what cost, with the coverage and costs of the plans offering Medicare prescription drug coverage in your area. Information about where you can get help to make decisions about your prescription drug coverage is at the end of this notice. There are two important things you need to know about your current coverage and Medicare’s prescription drug coverage:

1. Medicare prescription drug coverage became available in 2006 to everyone with Medicare. You can get this coverage if you join a Medicare Prescription Drug Plan or join a Medicare Advantage Plan (like an HMO or PPO) that offers prescription drug coverage. All Medicare drug plans provide at least a standard level of coverage set by Medicare. Some plans may also offer more coverage for a higher monthly premium.

2. The MAPP Group has determined that the prescription drug coverage offered by the The

MAPP Group is, on average for all plan participants, expected to pay out as much as standard Medicare prescription drug coverage pays and is therefore considered Creditable Coverage. Because your existing coverage is Creditable Coverage, you can keep this coverage and not pay a higher premium (a penalty) if you later decide to join a Medicare drug plan.

When Can You Join A Medicare Drug Plan? You can join a Medicare drug plan when you first become eligible for Medicare and each year from October 15th to December 7th. However, if you lose your current creditable prescription drug coverage, through no fault of your own, you will also be eligible for a two (2) month Special Enrollment Period (SEP) to join a Medicare drug plan. What Happens To Your Current Coverage If You Decide to Join A Medicare Drug Plan? If you decide to join a Medicare drug plan, your current The MAPP Group coverage will be affected. If you joined a Medicare drug plan after a COBRA qualified event, your COBRA coverage may end. If you do decide to join a Medicare drug plan and drop your current The MAPP Group coverage, be aware that you and your dependents will not be able to get this coverage back.

When Will You Pay A Higher Premium (Penalty) To Join A Medicare Drug Plan? You should also know that if you drop or lose your current coverage with The MAPP Group and don’t join a Medicare drug plan within 63 continuous days after your current coverage ends, you may pay a higher premium (a penalty) to join a Medicare drug plan later. If you go 63 continuous days or longer without creditable prescription drug coverage, your monthly premium may go up by at least 1% of the Medicare base beneficiary premium per month for every month that you did not have that coverage. For example, if you go nineteen months without creditable coverage, your premium may consistently be at least 19% higher than the Medicare base beneficiary premium. You may have to pay this higher premium (a penalty) as long as you have Medicare prescription drug coverage. In addition, you may have to wait until the following October to join. New Health Insurance Marketplace Coverage Options and Your Health Coverage

PART A: General Information When key parts of the health care law take effect in 2014, there will be a new way to buy health insurance: the Health

Insurance Marketplace. To assist you as you evaluate options for you and your family, this notice provides some basic

information about the new Marketplace and employment based health coverage offered by your employer.

What is the Health Insurance Marketplace? The Marketplace is designed to help you find health insurance that meets your needs and fits your budget. The

Marketplace offers "one-stop shopping" to find and compare private health insurance options. You may also be eligible

for a new kind of tax credit that lowers your monthly premium right away. Open enrollment for health insurance

coverage through the Marketplace begins in October 2013 for coverage starting as early as January 1, 2014.

Can I Save Money on my Health Insurance Premiums in the Marketplace? You may qualify to save money and lower your monthly premium, but only if your employer does not offer coverage, or

offers coverage that doesn't meet certain standards. The savings on your premium that you're eligible for depends on

your household income.

Does Employer Health Coverage Affect Eligibility for Premium Savings through the Marketplace? Yes. If you have an offer of health coverage from your employer that meets certain standards, you will not be eligible

for a tax credit through the Marketplace and may wish to enroll in your employer's health plan. However, you may be

eligible for a tax credit that lowers your monthly premium, or a reduction in certain cost-sharing if your employer does

not offer coverage to you at all or does not offer coverage that meets certain standards. If the cost of a plan from your

employer that would cover you (and not any other members of your family) is more than 9.5% of your household

income for the year, or if the coverage your employer provides does not meet the "minimum value" standard set by the

Affordable Care Act, you may be eligible for a tax credit.1

Note: If you purchase a health plan through the Marketplace instead of accepting health coverage offered by your

employer, then you may lose the employer contribution (if any) to the employer-offered coverage. Also, this employer

contribution -as well as your employee contribution to employer-offered coverage- is often excluded from income for

Federal and State income tax purposes. Your payments for coverage through the Marketplace are made on an after-

tax basis.
