Out of Darkness and into the LightBushra Wahabuddin was LRBT’s

18th millionth success story.

Bushra 20 is the youngest of her 8 siblings. Due to paucity ofresources she was forced to drop out of school when veryyoung.

After the death of her parents she began looking for a job as amaid as she did not have any formal education. She found a jobin a beauty parlour and soon she learned the trade. She thenopened up a small salon at her quarter, earning approximatelyRs. 4000 every month.

And then calamity struck: her eyesight began to deteriorate andit affected her job. She tried several local treatments but in vain.She asked her siblings for help but they could not afford to gether treated.

Bushra finally had to close down her parlour and there weredays when there was no food in the house.

A year later, hope flickered once again: A former colleague tookher to LRBT Free Eye Hospital in Korangi where she was diag-nosed with bilateral Cataract. Surgery was performed with verysuccessful results. Bushra has restarted her small salon to sup-port herself with a lot of pride.

Bushra says, “This is a miracle. My life has been transformedthanks to LRBT. I can now stand on my feet and look at thefuture with some hope”.

Accessibility ofGovernment Eye

Care FacilitiesManthar is a hari near BadinHe gets a share of the crops he tills in addition to off season jobs.He has to support a family of 9 on very meagre earnings. 5 of hischildren and his brother & sister are blind with congenitalcataract. There was no Government hospital nearby and Mantharcould not afford a private hospital.

It was Kismat (luck) he said. Allah’s will.

One day, a social worker wrote to daily DAWN relating Manthar’splight appealing for help. We sent a doctor to Manthar’s village toexamine the family. They were brought to LRBT’s Korangi Hospitalall expenses paid.

They were operated on but for 2 of the older children and hisbrother & sister it was too late they had lost God’s most preciousgift, the gift of sight. For the younger three the sun was shiningfor the first time in their lives. Hope was kindled.

The Miracle That Happened

Zubeida belongs to a poor family of 6 from avillage near Peshawar.

Her father is a rickshaw driver. He could barely feedhis family from his daily earnings. Zubeida lost hervision after a trauma which resulted in cornealopacity. She registered for a corneal transplant at aGovernment Hospital in Peshawar. But her turn nevercame. She lived in a state of despondency.

Sometime later her father moved to Karachi. Onhearing about LRBT, Zubeida’s father brought her toKorangi Hospital. A cornea was made available andgrafted. Her joy knew no bounds: She could onceagain see the Wondrous Colours of the World.

T A S KA H E A DDespite the great strides made against blindnessforbidding challenges lie ahead There are still

n 1.7 million blind causing an annual loss of Rs 65 billion($ 0.76 billion) in terms of lost productivity.

n 17 million Visually Impaired (10% of the population).

n 2.6 million children are either blind or suffer fromimpaired vision.

The 2/3rd of our people currently living aroundthe poverty line are unlikely to diminishappreciably.

Government spending on health is unlikely tosignificantly increase from the current 0.55% of the GDP.

LRBT which currently treats 36 % of all eyepatients visiting hospitals will have to play aneven bigger role. This will require support fromour loyal donors and supporters.

Chairman’s ReportI still remember like it was yesterday LRBT»s establishment 25 years ago.Our dream was a network of free eye hospitals located so strategically thatvirtually no Pakistani is more than a 3 hour bus ride away. It seemedimpossible and the realists amongst us scoffed at our ambition - a classiccase they said of overweening ambition. They are letting their imaginationrun wild and no good will come out of this. It is doomed to overstretch andwill collapse. We were driven by the belief that civil society must developtheir own solutions to pressing problems as Government due to othercompeting demands on resources would never be able to resolve them.They were driven by realism. It was a huge task I must admit but passion,commitment and perseverance helped us in surmounting the challenges.We started with a capital of Rs 1 million which had been donated byGraham Layton & Zaka Rahmatulla equally. Of this Rs 100,000 went towardsconstructing the mobile unit which was designed by Graham and builtunder his supervision by associates from his old company MacdonaldLayton. It was initially based in a village near Thatta in November 1985where the first surgery was performed and subsequently moved to TandoBago. The mobile unit was replaced by a purpose built hospital in 1989.Korangi was next: we started in tents in 1986 while we gathered funds forthe building and equipment. Lahore followed in 1987 starting in rentedpremises and then moving into a half finished hospital owned by the LahoreTownship Cooperative Society. From then on there was a steady stream ofnew hospitals with a new one being opened every 18 months. At the same time the influx of patients was forcing us to expandour existing hospitals and add new equipment. As I was looking afterFinance at the time I protested at this breakneck speed but Graham wasadamant. He had this unshakeable belief that God would provide. And so hedid in one guise or another.

25 years on our dream has been fulfilled. We have grown from a humblemobile clinic to 16 purpose built hospitals and 42 Community Eye HealthCentres (CEHC). In the first year 1984/5 we treated a mere 11,699 patients.In 2009/10 this had grown to a massive 1,967,993. Surgeries increasedfrom 546 in 1984/5 to 209,742 in 2009/10. From an isignificant proportionwhen we began, we now treat 36 % of all eye patients visiting a hospital andperform 27 % of eye surgeries. Our contribution to Paediatric eye surgeriesat 32 % is even more pronounced. There are very few NGO's in the worldwhich have our scale and which make such a significant impact in theirchosen field of endeavor.

LRBT is not all about volume. Our hospitals are staffed by dedicated, skilledand highly experienced doctors and paramedics who use the latestequipment and procedures to deliver high quality treatment whichconsistently surpasses the standards set by the World Health Organisationand the International Centre for Eye Health

Another unique feature about LRBT is its institutionalization. We haveinstituted an effective system of governance and put in place procedures andcontrols to ensure transparency. As a consequence the progress of LRBT wasunaffected by the deaths of its founders: Zaka Rahmatulla in 1989 andGraham Layton in 1999. The number of hospitals has doubled from 8 to 16and the number of patients treated by us has tripled in the 11 years sinceGraham Layton's death. I am confident that the organization in the next 25years will make an even more significant a contribution to the prevention ofcurable blindness as it has done in the past quarter of a century. LRBT's success has been made possible by the generosity of our donorsboth large and small most of whom have been supporting us for a very longtime, our supporters who have given us their very valuable time, the mediawho have generously given us free airtime/space, the dedication andcommitment of our staff who work long hours for little material reward.May God bless them, their families and their enterprises.

OOPPDD VVIISSIITTSS // SSUURRGGEERRIIEESSDuring the year ending June 2010, 1.96 million patients visited the Out PatientDepartments (OPD) of our 16 hospitals and 42 Community Eye Health centers, a growth of12% over the previous year.

During the same period 209,742 major & minor surgeries were performed, an increase of13%. Cataract which is the leading cause of curable blindness constituted 64 % of totalsurgeries. Other surgeries included Vitreo Retina, Glaucoma, DCR, Squint etc. We are thebiggest provider of Vitreo-retinal surgery for the poor people in the country.

1596 patients were treated with Avastin for Age Related Macular Degeneration.177 corneas were transplanted at Lahore & Korangi: 136 in Lahore with corneas beingdonated by Begum Zahra Manzoor Illahi, President Binai Eye Bank Pakistan and 41 inKorangi. Syed Muslim Hasany of Toronto Eye Bank, Canada donated 13 whilst PakistanEye Bank Society donated 6 corneas.

EEXXPPEENNSSEESSOur operating expenses during the year were Rs.411 million being 21% above last yeardue to a growth of 12% in patient visits to OPD and 13% in major surgeries. CapitalExpenditure including Work in Progress at Rs.124 million was 8% lower than last yearmainly due to completion of major expansion projects last year. Our overhead expenseswere 5.1% of operating expenses and are among the lowest in the world.

EEXXPPAANNSSIIOONN OOFF CCAAPPAACCIITTYYMANDRA (RAWALPINDI)To reduce the backlog of surgeries the operation theatre was doubled in size. This projectwas completed in December 09 and the cost of Rs. 1.30 Million was funded equally by thePunjab Welfare Trust for Disabled (PWTD) and LRBT.

GAMBAT (KHAIRPUR)To reduce the surgical backlog Gambat's operation theatre was expanded by 50 % . Theproject was fully funded by a donation arranged by Mr. Justice (R) Shamim AhmedSarwana who also arranged money for a generator to ensure the uninterrupted running ofthe hospital during load shedding.

NEW OPD COMPLEX KORANGI (KARACHI)To provide better facilities for our patients and to allow for future growth a new OPD wing(covered area 14,000 sq.ft.) consisting of patients' waiting area, lecture hall, dispensary,

screening, refraction and consultants' rooms was completed. This project of Rs 18 millionwas funded by the Bhaimia family who had also financed the construction of the 2nd floorof the old building which houses the operation theatre complex, the paediatricsubspeciality area, the low vision clinic, the library & the conference hall. Mr. Zain K.Anjarwala oversaw the project.

Our Korangi hospital is a Tertiary care facility and a postgraduate training centre andserves the population of Karachi, the Indus delta, the coastal areas of Baluchistan & thosepatients from Sindh & Baluchistan who require tertiary level care.

EXPANSION OF KHANEWAL HOSPITAL:New male and female wards each consisting of 20 beds were constructed. This will enableincreased surgery thereby reducing the backlog. This project was funded by Mr. NasirAhmed Mehmood from Dubai.

SHAHPUR HOSPITAL (DOCTORS ACCOMODATION):With increased patient inflow we have had to increase our complement of doctors. Toattract doctors to serve in rural areas we provide free furnished accommodation. Wetherefore have had to build an additional family accommodation and a bedroom for abachelor. This was funded by Mr. Usman Paracha.

NNEEWW HHOOSSPPIITTAALL:: SSWWAATTl With Kalakalay hospital damaged during the troubles LRBT had opened a temporary

facility in Tehsil Headquarters hospital at Barikot. On the conclusion of the militaryoperation we reopened this temporary facility on 3rd August 2009.

l Kalakalay is not ideally situated and it was therefore decided to convert it into aCommunity Eye Health Centre. It was also decided to replace it with a new hospital atOdigram near Mingora which will serve all of Malakand and Swat provided we canobtain grants to cover the construction and road building costs.

l We have been successful in obtaining a donation of 20 Kanals (12,000 sq yards) ofland at Odigram about 8 km short of Mingora from the local community. The land isabout 1.5 Kms. away from the main road and there is no approach road. General OfficerCommanding (GOC) - Swat has made a commitment to construct an approach road.

l LRBT applied for a grant of Rs.59.6 million to construct the hospital from the PakistanItalian Debt Swap Agreement (PIDSA) funds given to Provincial Reconstruction andRehabilitation Settlement Authority (PaRRSA) for Swat. A PC1 was sent to PaRRSA.The Army also submitted a PC1 for the approach road to our hospital. We understandthat sum of Rs 50 million has been earmarked by PIDSA/PaRRSA for the hospital.Another 3 million has been earmarked for the approach road.

Training Courses6 CEHC workers & 7 OT Technicians were trained in the month of August 2009 & January2010 respectively for one month making a total of 57 paramedics of different trades whichhave been given refresher training till now at Korangi hospital in seven batches.

Organisational Developmentl Policy manual for Hospitals and CEHC's has been compiled, incorporating all relevant

rules/regulations in force.

l Community Services including Community Eye Health Centers, School Screening, EyeScreening Camps have been reorganized. Standards for each segment has beenformulated and put in place.

l Induction criteria for paramedics / support staff and Interview Assessment Form havebeen designed and implemented.

l Assessment Form for Confirmation has been improved.

l Procurement, Repair & Maintenance department has been separated from theAdministration for better control.

l Dr. Zahid Kamal (FCPS, FRCS) Assistant Professor in Post Graduate MedicalInstitute & General Hospital Lahore along with Mr. Azhaid Hafiz Consultantoculist visited LRBT Korangi hospital on Saturday 4th July 2009, Dr. Kamalassisted by Dr. Hari Krishin of Korangi hospital gave a live demonstration onEnucleation. He also readout his research papers on Dry Eye Syndrome,Enucleation implant, PESS and DCR with LJ Tube on Enucleation + SS implant.The live surgery was attended by surgeons of LRBT as well as Professor JavedHassan Niazi (Head of eye department of JPMC) & Commodore Waseem ofPakistan Navy Hospital Shifa. The session was conducted by Dr. Munira Shakir.

l The research work on "Comparison of Visual Outcome between Superior ScleralTunnel and Temporal Scleral Tunnel Approaches in Manual Small Incision CataractSurgery" written by our doctors from Lar hospital was published in Al-Shifajournal of Ophthalmology.

l Two papers, one on "Role of PFCL in retinal detachment surgery" and other on"Vitreo on a Perfluorocarbon Liquid as Vitreous substitute in RetinalDetachment surgery" were published in Annual Journal of Jinnah PostGraduate College (JPMC) Vol. I, 2 & 3 and Pakistan Journal of Ophthalmology2009, Vol. 25 No. 2 respectively. l Dr. Zaheeruddin Aqil Qazi, (Chief Consultant Ophthalmologist) Lahore was invitedas guest speaker at the Annual Ophthalmic Conference held in Hyderabad from01st to 09th November 2009.

l Ophthalmological Society of Pakistan (Sahiwal Branch) organized "Live Surgery" atSahiwal on 11th December 2009. Dr. Zaheeruddin Aqil Qazi (Chief ConsultantOphthalmologist) was invited to perform live surgery in order to share and upgradethe knowledge of Ophthalmology amongst the Ophthalmologists of Sahiwal andsurrounding areas.

l Dr. Zaheeruddin Aqil Qazi (Chief Consultant Ophthalmologist) and Dr. KhurramAzam Mirza (Consultant Ophthalmologist) both from Lahore attended KarpothConference on 13th February 2010 in Karachi.

During these twelve months, Trustees have considered how best to meet the challengeof the ever-increasing commitment to LRBT to fund, on a best endeavours basis, therequirement for intra-ocular lenses (IOL) supplied to LRBT hospitals.

The Trust donated a total of £258,224 to LRBT including £57,267 from an anonymouscorporate donor for a specific programme at Mandra Hospital. This pleasing outcomewas the result of a wide range of generous donations and a number of fundraisingevents, notably TV commercials and the first live appeal on television. The success ofthese fundraising activities is due entirely to the unstinting effort and work of theTrustees, staff and the volunteers who give their time so generously and withoutquestion. Generous supporters have provided continued support from the UK, Europe,Saudi Arabia and Bahrain. GLT and its beneficiaries are deeply grateful to them all.

GLT was able to meet its commitment of funding the supply of intra-ocularlenses to LRBT's hospitals in 2009/10. Because GLT was only required to payfor three orders of IOLs during the twelve month period instead of theanticipated four, the £198,257 cost of this commitment was lower thanexpectations. It is likely that financial constraints caused by the internationalfinancial crisis will continue in 2010/11, during which the year-on-year demandby LRBT for IOL's has been forecast to increase to approximately $413,000.Raising this sum will be a significant challenge for GLT.

Finally, the Trustees would like to record their thanks to all the many supportersand volunteers who have, according to their means and time available,contributed so generously to GLT and to its performance in this period. TheTrustees wish also to recognise the substantial contribution made by Mrs.Neelum Amin during the time that she was a Trustee, particularly in respect ofthe organisation of the 2007 & 2008 GLT Winter Balls. Mrs Amin was Co-Chairman of the Organising Committee for both these Balls, and was tireless inher fund raising efforts on behalf of GLT/LRBT.

LRBT America has had another year of continued success in its core mission of fighting blindness through grants to theLayton Rahmatulla Benevolent Trust's hospitals and operations. This year's Annual Gala event was held at the EspaceBallroom on Manhattan's West Side, and attracted an audience of over 250 attendees. The evening featured a highlysuccessful live auction and musical accompaniment by the acclaimed DJ Rekha. As in previous years, the Annual Gala wasLRBT America's primary fund-raising event of the year. Two additional fund-raising functions were also held during the periodunder review. These were a Ramadan Iftari event graciously hosted by Ambassador Hussain Haroon at his residence, and ayoung friends of LRBT Club night. The total funds donated to LRBT in Pakistan for the period under review was $ 130,000.

Pakistan was then devastated by the floods of 2010. This occurred immediately after the review period and before thepublication of this report. LRBT-America had in its five years life 2005-2010 in addition to sending generous annualdonations to Pakistan, had also built up an incremental $600,000 of reserves for the proverbial "rainy day". With thefloods it was decided to immediately release these funds to Pakistan.

As a result of the acutely increased needs on the ground in Pakistan in the aftermath of this summer's floods, LRBTAmerica has now made significantly increased grants to LRBT's operating needs this year. In addition to fully supportingthe operating costs of the Arifwala Hospital, an additional special grant dedicated to flood relief activities has also beenmade. Furthermore, LRBT America has donated and established initial funding for the long term, future operations of theArifwala Hospital. This year's grants total $600,000 and are the largest by far in LRBT-America's history. This reflectsLRBT-America's highest level of commitment to fighting blindness in Pakistan through the support of LRBT's top-notchlarge-scale high quality operations. This extraordinary one time release in the wake of the devastating flood crisis inPakistan, has however also completely depleted our accumulated reserve funds. The entire savings which we hadaccrued, during the 2005 to 2010 lifetime of LRBT-America, have now been expended. Going forward, we have onceagain been a pioneer amongst our peers, in offering our donors the opportunity to make recommendations as to whetherthey prefer to have their funds allocated to specific current operating needs, or towards the future funding of LRBT, whilecontinuing to also provide for current needs. It is an important priority for us to rebuild the funds which were drawndown this year. In this we most earnestly seek our donors» vigorous support on an enhanced basis.

Finally we would like to thank our many donors, supporters and volunteers who contribute so generously of their timeand money and make it possible to continue our mission of fighting blindness. Our New York committee has beenoutstanding: dedicating their time and energy to countless hours of fundraising activities and being ambassadors ofLRBT America. LRB Foundation CanadaIn the third year of our establishment we continued our partnership with IDRF (International Development and ReliefFund) and together raised $ 24,533 which enabled the funding of 1189 Cataract Surgeries inclusive of 149 children.

Plans for 2010/11 are more ambitious and we are hoping to double our fundraising efforts.

We have decided not to pursue our tax exempt application with the Revenue authorities as the process is very tediousand expensive and our arrangement with IDRF is working smoothly.
