Dear Beth David Bar/Bat Mitzvah Parents,

We are very excited to be guiding you on your journey toward celebrating your child’s becoming a

Bar or Bat Mitzvah. While this joyous milestone is only one of many you will have in your lives,

the overall all experience—from choosing the perfect date to watching your child be lifted in a

chair during the hora, will be an experience of a life time.

Through this journey, your child will benefit from structured study sessions and homework

assignments, practice in speech writing and presentation, opportunities for leadership roles,

involvement in social action projects, attending/learning/leading our prayer service, engaging with

other Jewish teens and learning how to support our Jewish community. This experience will help

you further connect to Judaism, to Beth David, and to each other, not only on the Bar/Bat Mitzvah

day, but hopefully through the rest of your lives.

The following pages are designed to help you navigate the details to becoming a Bar/Bat Mitzvah

at Beth David. Please note that there are some forms included that you need to return to the office

In order to reserve your requested date, you must be members in good standing of Beth David Congregation.

To update your membership status, please contact Paula Arias, at (305) 854-3911, Ext. 201.

The fees associated with Bar/Bat Mitzvah preparations total $1,800. This fee pays for your child’s religious school tuition,

Bar/Bat Mitzvah tutoring and materials, as well as some of the expenses associated with the costs of operating the

synagogue for the Bar/Bat Mitzvah services (See Religious School Form on pg # 22). In addition, this fee covers: Service program

design and printing (100 pcs), Bar/Bat Mitzvah certificate, and individually wrapped soft candy (approx. 200 pcs). Bar/

BatMitzvah fee may be paid in two $900 installments. We ask that the first payment be made when selecting the date for

your child’s Bar/Bat Mitzvah ceremony. The remaining balance, if any, must be paid four weeks prior to the Bar/Bat

Mitzvah celebration.

Bar/Bat Mitzvah

“Coming of Age”

On the occasion of a Bar or Bat Mitzvah (son or daughter of the commandment, respectively) Jewish

children mark their transition from youth to adulthood in Judaism.

The Bar Mitzvah ceremony originated in the 14th century, when boys were called to the Torah as they

passed their 13th birthday. Not until the middle of the 20th century did the ceremony include girls.

To become a B’nei Mitzvah (plural) Jewish children study in religious and Hebrew schools. They

study Jewish history, holidays, life cycle events and the Torah. They also study Hebrew words that

shape Jewish values.

During the worship service, the B’nei Mitzvah will lead the congregation in prayer, read from the

Torah and a section of the Prophets or Writings called Haftarah, and deliver a speech that conveys a

lesson from Torah for today.

The B’nei Mitzvah ceremony is important to the families and to the Jewish community. The passing

of the Torah from parents to children is a symbol of Jewish continuity, and its transmission from

generation to generation. In the Torah we are taught, “You shall teach them diligently to your

children.” It is a commandment which assures that our children and our children’s children will be

recipients and keepers of God’s covenant.

Welcome to our congregation and welcome to our Shabbat service. We extend a special welcome to

those who are here for the first time. To help you understand the significance of the ceremony, we

have prepared this guide. We hope it makes the service more meaningful and comfortable for you. If

you have any further questions, please do not hesitate to ask Rabbi Jacobs following the service.

The Significance of Becoming a Bar/Bat Mitzvah:

In Jewish tradition, a child reaches the age of maturity at the age of thirteen. At this time, the child

become a bar mitzvah (boy) or a bat mitzvah (girl) and is now obligated to fulfill the commandments

of the Torah. Children study for many years in religious school to prepare for this day. It is a cause for

celebration.

Passing the Torah:

The Torah is a sacred scroll that contains the first five books of the bible. It has been transmitted

though the generations for thousands of years. As the B’nei Mitzvah receive the Torah, they accept

the responsibility of carrying on this eternal covenant.

The Prayer Service:

The prayers recited during the service are part of the traditional Jewish liturgy for a Shabbat

(Sabbath) service. The prayers affirm the unity and holiness of God, the sanctity of the Shabbat, and

the hope for a world of peace, justice and light.

Reading of the Torah:

The Torah is divided into 54 portions to be read throughout the year. Each week, the same portion is

read in synagogues throughout the world. The scroll is handwritten by a scribe using special ink on

animal-skin parchment. It is a great challenge to read from the Torah as the Hebrew writing contains

no vowels or punctuation.

The Haftarah:

The Shabbat service also features a weekly reading from the Prophets called the Haftarah portion.

D’var Torah:

The B’nei Mitzvah prepare a sermon based on the Torah portion. It is a personal reflection on the

meaning of the portion.

Personal Prayer:

The B’nei Mitzvah also recite a personal prayer to God that serves as an expression of their gratitude

and a recognition of the significance of this day.

Bar/Bat Mitzvah Party at Beth David

Once a Bar/Bat Mitzvah date has been confirmed, rooms may be reserved for a Kiddush Luncheon or an Evening Party.

A 50% non-refundable deposit will be required to reserve a date on the synagogue’s calendar. This deposit will be apply to

the cost of the event. The facility fee will be calculated based on the number of guests attending the event; refer to the

rental brochure on page 12 for information about room available, fee schedules and security policies. Sixty (60) days prior

to the event, the remaining 50% of the facility fee will be due. Final count and/or additional fees will be due to the

synagogue’s office ten working days prior to your event.

Caterer

Once you have selected your kosher caterer from the list provided on page 11, please have the caterer contact the

synagogue’s office to set up schedules and security deposit. There is a $1,000 security deposit which is refundable two

weeks after the event. Please refer to the Caterer Licensing Agreement. See page 13.

Kiddush Luncheon:

It is customary and part of the policy to invite the congregation for a Kiddush luncheon the same morning of the

Bar/Bat Mitzvah. We usually have about 40 regular attendees. No room fee is due to BDC as the congregation is

invited.

If you would like to have your evening party at BDC, as a member, you will receive 50% discount for the rent of

our facility. Please know that catering fees have no discounts.

Music

If you would like a musical accompaniment for your service, please speak with Rabbi Jacobs

DJ or Entertainment

Must provide his/her own sound equipment. To ensure that all accommodations can be made, the power requirements

must be communicated to the Synagogue one (1) month prior to the event.

In the event that the Synagogue’s sound system is required, our sound engineer must be hired separately. Please contact

the Synagogue’s office.

Flowers

If you are getting flowers for the weekend, please provide Paula Arias or Amy Anderson the company’s contact infor-

mation, so we can arrange drop off times.

Pictures/Rehearsal:

The family usually takes the pictures on the same day as the last rehearsal. Please coordinate with Rabbi Jacobs and

provide Paula Arias or Amy Anderson the company’s contact information, so we can arrange set up times. No photo-

graphs will be allowed during Shabbat and only still video will be allowed from the balcony of the bimah.

Baskets

Baskets for candy, program (if you decide to have one), and kippot should be brought into the synagogue’s office the

Wednesday prior to the Bar/bat Mitzvah ceremony.

Gift Shop

Candy (individually wrapped soft candy):

Congregation provide you with 200 pcs of candy as part of your purchase. If you require additional candy, you

may place an order at BDC Gift Shop. Please contact someone in the synagogue’s office to place your order.

Candy is sold per bag ($18) containing approximately 100 pcs.

Tallitot, Yadayim (Torah pointers), Kippot, and more:

May be purchase at BDC’s Gift Shop with a 10% discount. Gift Shop is open on Wednesdays from 11am until 3am.

Please call the synagogue’s office to arrange a different time if necessary.

LICENSING AGREEMENT

FOR A FUNCTION AT BETH DAVID CONGREGATION

This is a license agreement dated the ______ day of ________________between Beth David Congregation, a Florida Not

For Profit Corporation (“Beth David”) and ____________________________ (“Licensee”) for the use of certain of the

premises at Beth David located at 2625 SW Third Avenue, Miami, FL 33129 to hold a function (the “Function”) on

_____________________________(“Function Date”) between the hours of _________________________________

The premises to be used are:

1. Use of Approved Caterers The Licensee understands that it must use only a caterer approved by Beth David.

Beth David has provided Licensee with a list of approved caterers. Even though Beth David has approved those

caterers, Licensee agrees and understands that Beth David makes no assurances with respect to any such caterer,

and Beth David shall not act as an intermediary between Licensee and the caterer, and should any dispute arise

between the Licensee and the caterer, it is to be resolved between them, and Beth David is not to be involved.

2. Security Security is required by Beth David for any Function, the Licensee will pay for all security selected

by Beth David, and will cooperate with Beth David in this regard. Beth David will obtain the security as agent for

Licensee.

3. Valet Service If any valet parking is required for the Function, Beth David will provide a list of approved

companies. The Licensee will arrange this with its caterer. None of the cost of valet parking shall be borne by Beth

David. The valet parking firm must provide insurance naming Beth David as a co-insured and a loss payee as its

interests may appear in an amount of $100,000 property damage and $1,000,000/$2,000,000 general liability

insurance. An appropriate endorsement must be provided no later than five days prior to the Function. The carrier

must be licensed to do business in Florida and must have an AM Best rating of A or better.

4. Observance of Kashruth Licensee agrees that it will observe Beth David’s Kashruth policies for the

Function. If Beth David determines that a Machgiach is required, Licensee shall be responsible for the expense of

the Machgiach.

5. Congregational Kiddush There may be circumstances where an event will occur immediately after the

conclusion of a Beth David service such as, but not limited to the Shabbat morning service. While Beth David does

not require Licensee to invite all those attending the service to Licensee’s Function, Licensee understands that

certain arrangements must be made for those congregants not invited to the Function. The minimum requirements

have been provided to Licensee. Licensee agrees to provide at its sole cost, a Kiddush which satisfies those

minimum requirements. Licensee shall have the option of selecting either the Beth David Provider or a caterer

approved by Beth David to satisfy this obligation.

6. Approval of Entertainment and other Service Providers All photographers, party planners, florists,

entertainment, musicians, etc. shall be subject to the approval of Beth David Congregation. The Licensee shall

provide such information regarding these providers to Beth David no later than one month prior to the Function.

Licensee or its agent designated to Beth David shall be responsible for ensuring that all of the providers are in

compliance with all of Beth David’s rules and policies, including, but not limited to set-up and delivery, and

appropriate activities in the Sanctuary and the Chapel. Insurance required as stated in paragraph 8.

7. License Fee The license fee shall be the sum of dollars, payable as follows: a. _____________ dollars upon the

execution of this agreement which shall be a nonrefundable reservation deposit (the “Reservation Deposit”).

b. _____________ dollars four weeks before the Function (the “Final Payment”).

TIME IS OF THE ESSENCE IN CONNECTION WITH LICENSEE’S OBLIGATION TO MAKE THE

FOREGOING PAYMENTS.

Licensee understands that this fee is subject to adjustment in order to ensure that Beth David is paid for all guests

in accordance with its fee schedule, a copy of which is attached hereto. Within seven (7) business days after the

Function, Beth David and Licensee shall meet to ensure that all of the guests have been paid for in accordance with

Beth David’s fee schedule. If, as a result of the post-function financial reconciliation, there are additional guests

which cause the attendance to fall into a higher bracket, Licensee will pay to Beth David the additional amount due

within five (5) business days after the reconciliation.

8. Insurance Licensee understands that any caterer, entertainment company or any service provider selected by

Licensee will be required to provide Beth David with a certificate of insurance naming Beth David as a co-insured

and loss payee containing limits of liability acceptable to Beth David. It is anticipated that those limits will be a

minimum of $100,000 in property damage and $1,000,000/$2,000,000 in general liability insurance. In addition, the

certificate of insurance itself must be examined by Beth David’s insurance agent to ensure compliance in other areas

as well. While Beth David is not imposing an obligation on Licensee to carry such insurance, Licensee agrees and

understands that should the caterer fail to comply with Beth David’s insurance requirements, service provider will

not be permitted to service the Function on Beth David’s premises.

9. Obligations of Caterer Licensee understands that its caterer has certain obligations to Beth David in

connection with the Function, including, but not limited to the payment of fees, providing insurance, etc. and that if

Caterer fails to satisfy those obligations, Beth David may prevent the function from taking place even if Licensee is

completely free of fault or blame. Beth David encourages Licensee to communicate with the caterer regarding those

obligations. If requested by Licensee, Beth David shall update Licensee on the status of those obligations. Licensee

agrees to provide to Beth David a copy of its contract with the caterer within three days after it is signed by

Licensee and the Caterer, and to provide Beth David with copies of all amendments to the agreement if, as and

when signed.

10. Calculation of Caterer's Fee Licensee understands that Beth David's fee from the caterer is based upon the

number of guests attending the Function. Licensee agrees to provide Beth David with such information as Beth

David shall require in order to verify the number of guests attending the event as and when Beth David requests that

information.

11. Limitations of Responsibilities of Beth David Licensee understands that Beth David’s sole obligation hereunder

is to provide a facility for the Function, and the opportunity for Licensee to use the tables and chairs which Beth

David has on site (although Licensee may bring in its own tables and chairs), and a kitchen to heat and/or prepare

food for the Function. Beth David shall have no responsibility for providing set up or tear down services or for

cleaning the premises after the Function. Beth David shall not be responsible for providing pots, pans, dishes,

glassware, serving plates, hollowware, linens or eating or cooking utensils, etc. for the Function. Licensee shall look

solely to its caterer for those items.

12. Licensee’s Cancellation of Function Licensee understands that the reservation deposit is non-refundable under

any and all circumstances. Licensee may cancel the function for any reason no later than four weeks before the

Function Date at no cost or expense other than loss of the Reservation Deposit. If Licensee cancels the function

fewer than four weeks prior to the event for reasons other than as set forth in Paragraph 15, an amount equal to 25%

of the total fee shall be refunded to licensee so long as all prior payments have been made and other obligations of

Licensee have been satisfied. Subject to Paragraph 16, no refund shall be made if the cancellation occurs within one

week prior to the function.

13. Beth David’s Cancellation of Function Beth David may cancel the Function on seven days notice to Licensee at

anytime after:

a. Beth David has notified Licensee that Licensee has failed to satisfy any of its obligations hereunder

and Licensee has failed to cure within seven days after such notice. There shall be no grace period for

curing a failure to pay in that Licensee is well aware of its obligations at the time this agreement is

executed.

b. Beth David has notified Licensee that the caterer has failed to satisfy any of its obligations under the

Catering Agreement and Licensee has failed to cure within seven days after such notice.

Upon such Cancellation, Beth David shall be entitled to retain all amounts paid hereunder.

14. Extension of Function Date If, at any time prior to the Function Date, the premises are so damaged as a result

of fire, flood, windstorm or other catastrophe or act of G-d such that it is impossible to hold the Function on the

Function Date, Beth David shall have the option of either extending the Function Date for thirty days, or returning

to Licensee all payments which Licensee has made other than the Reservation Deposit. If the damage or catastrophe

is such that the Function cannot take place on the premises by the end of the thirty day period, the Function shall be

canceled and Beth David shall return to the Licensee all payments made by Licensee hereunder other than the

Reservation Deposit.

15. Cancellation of Event If, prior to the Function Date, any of the participants in the Function dies or becomes

seriously ill such that the participant cannot attend the Function, Beth David shall, in the event of the death of a

participant, return to Licensee all payments which Licensee has made other than the Reservation Deposit. In the

event of the serious illness of a participant, Beth David have the option of either extending the Function Date for

thirty days, or returning to Licensee all payments which Licensee has made other than the Reservation Deposit. If

the participant’s illness is so serious that the Function cannot take place on the premises by the end of the thirty day

period, the Function shall be canceled and Beth David shall return to the Licensee all payments made by Licensee

hereunder other than the Reservation Deposit. The term “Participant” shall be defined as the bride, groom, Bar or

Bat Mitzvah, honoree (in the case of a testimonial), other, and no other.

16. Miscellaneous

a. Any forbearance, failure or delay by any party in exercising any right, power or remedy hereunder shall not

preclude the further exercise thereof.

b. This Agreement may be amended, modified, superseded or canceled, and any of the terms, covenants,

representations, warranties or conditions hereof may be waived, only by a written instrument executed by

the parties hereto or, in the case of a waiver, by the party to be charged with such waiver. In the event of

such waiver, the same shall be effective only for the period and on the conditions and for the specific

instances and purposes specified in such writing.

c. This Agreement supersedes and revokes any and all prior Agreements, written or oral, relating to the

subject matter hereof other than the documents required to be executed hereunder or referred to herein and

the terms and conditions hereof and thereof shall be solely determinative of the subject matter hereof.

d. If any covenant or other provision of this Agreement is invalid, illegal or incapable of being enforced by

reason of any rule, law or public policy, all other conditions and provisions of this Agreement shall,

nevertheless, remain in full force and effect and no covenant or provision shall be deemed dependent on any

other covenant or provision unless so expressed herein.

e. Any notice, request, demand, instruction, or other communication to be given to any party to this

Agreement shall be in writing (including telegraphic communications) and shall be sent by registered or

certified mail, hand delivery by Federal Express or other reputable courier service, but not the US Postal

Service Express Mail, and shall be deemed delivered upon receipt of said notice or upon receipt of the

actual knowledge of the information contained in said notice by any means. The addresses for the purposes

of this Section may be changed by giving notice hereunder. Unless and until notice of a change of address is

given and received hereunder, the last addresses as provided herein shall be deemed to continue in effect for

all purposes hereunder. Notice may be sent by email, facsimile transmission provided the original notice is

h. Captions and headings to the various paragraphs of this Agreement are for convenience only, and carry no

legal effect and shall in no way effect the legal interpretation or construction of the paragraphs of this

Agreement.

i. LICENSEE UNDERSTANDS THAT THE PREMISES IS A NON SMOKING FACILITY AND

LICENSEE SHALL ADVISE ALL OF ITS GUESTS, PHOTOGRAPHERS, PARTY PLANNERS,

MUSICIANS, PHOTOGRAPHERS, ETC. THAT SMOKING INDOORS OR NEAR THE EXITS OF

ANY PORTION OF THE PROPERTY IS STRICTLY PROHIBITED.

CATERING AGREEMENT

This is a Catering Agreement between Beth David Congregation, a Florida non for profit corporation (“Beth David”)

and ____________________________ (“Caterer”)The Caterer has entered into an agreement with

(“Customer”) to cater a function _______________________ (the “Function”) on the premises of Beth David Congregation

located at 2625 S.W. 3rd Avenue, Miami, Florida 33129 on _________________________________( the “Function Date”)

between the hours __________________.

1. Coordination with Executive Director Caterer will coordinate with and obtain the prior approval of Beth

David for all of its activities hereunder, including, but not limited to delivery of food and equipment, set up of

tables and chairs, access to the premises before the event, etc.

2. Payment of Fees Caterer will pay to Beth David a usage fee (the “Usage Fee”) of $______________ dollars

due and payable upon the execution of this agreement.

CATERER UNDERSTANDS THAT TIME IS OF THE ESSENCE IN CONNECTION WITH ITS

OBLIGATIONS TO PAY THE FOREGOING AMOUNT.

3. Security Deposit In addition to the usage fee, Caterer agrees to pay to Beth David three days before the event

a clean-up, security and maintenance deposit of dollars. The deposit is to ensure

that upon the conclusion of the function, the kitchen and other premises used by the Caterer and the Customer will

be cleaned and left in good order, and that all other obligations of Caterer hereunder are satisfied. Caterer agrees and

understands that its damages and financial obligations are not limited to the amount of the security deposit.

Caterer’s clean up responsibilities are as follows:

a. All tables, chairs and other furniture must be broken down and stored in its place, as designated by the

Executive Director.

b. All trash must be placed into plastic bags and must be removed from the premises and must be put into the

dumpster immediately following the function.

c. All equipment belonging to the Caterer must be removed from the premises immediately following the

function.

d. All Beth David equipment inadvertently taken by the Caterer must be returned to Beth David.

e. All Beth David equipment used by the Caterer including but not limited to stoves, ovens, sinks, refrigerators,

dishwashers, etc. must be cleaned by the Caterer upon conclusion of the function.

f. All countertops and other food preparation areas must be washed.

g. The kitchen floor must be swept and mopped so that is free of all grease and debris.

h. So much of the premises as is used for the event, including but not limited to Falk Sanctuary, Spector Hall,

Traurig Hall, and the Museum, must be swept and vacuumed.

Beth David will inspect the kitchen and remainder of the premises to determine if the Caterer has satisfied its cleanup

and all other obligations hereunder. If this is the case, the security deposit will be returned to the Caterer. If the

Caterer has failed to satisfy any of its clean-up or other obligations hereunder, Beth David will do what Caterer has failed to

do, retain the deposit by the appropriate amount, and return any balance to the Caterer.

4. Maintenance Personnel. Caterer understands that Beth David maintenance personnel must be on the property

at all times when the facility is occupied. The fee includes hours. If the maintenance personnel are required

to stay beyond that period of time for any reason, Caterer agrees that $50.00 per hour per person shall be deducted

from the Security Deposit provided in Paragraph 3, above.

5. Satisfactory Condition of Equipment Caterer acknowledges that it is fully familiar with Beth David’s

food preparation and cooking facilities, including, but not limited to stoves, ovens, refrigeration equipment, and

water, disposal, gas and electrical supplies, and is fully satisfied that those facilities will enable Caterer to provide

the contracted services for the function.

6. Insurance Two weeks prior to the Function, Caterer shall provide proof of insurance and indemnification to

Beth David for any loss, damage or liability caused directly or indirectly by the Caterer, its agents, employees and

others in any way involved with providing services for the function, as well as by third party vendors. Beth David

shall be named as a co-insured and loss payee on the insurance certificate. Additionally, Caterer shall provide Beth

David with a copy of its insurance policy for examination and review by Beth David’s carrier for compliance.

Caterer shall provide a Certificate of Insurance naming Beth David as co-insured and loss payee. The limits of

liability shall be at least $100,000 in property damage and $1,000,000/$2,000,000 general liability coverage, with

no exclusions for food related liability and affirmative coverage for providing and serving alcoholic beverages, and

such other coverage as Beth David shall determine. The carrier must be licensed to do business in Florida and must

have an AM Best rating of A or better. In the event that Caterer uses any third party vendors, such as, but not

limited to valet parking and security services, such third party vendors must provide the same insurance coverage in

favor of Beth David, and must provide to Beth David the same certificate of insurance as the Caterer must provide.

7. Nature of Relationship Caterer acknowledges and agrees that this agreement is not intended to create and

does not,in fact, create any partnership, join venture, profit sharing arrangement, or other relationship other than a

licensor-licensee arrangement whereby Caterer is paying to Beth David a fee for Caterer’s use of the premises.

8. Beth David Not Intermediary Caterer acknowledges and agrees that should any dispute arise between Caterer

and Customer, that dispute shall be resolved between them, and Beth David shall not be a party to any such dispute.

9. Compliance with Kashruth Caterer agrees that it will, in all respects, and at its sole cost and expense, comply

with all of Beth David’s policies regarding Kashruth and will comply with all requests of Beth David’s Mashgiach.

Caterer agrees to provide the Mashgiach with access to all off site premises where any food or beverages are

prepared for service at the Function. A copy of Beth David’s Kashruth policy is attached.

10. Compliance with Laws Caterer will not perform or allow to be performed any services by persons or

entities not otherwise entitled, as a matter of law, to be present in the United States and perform services in the

United States. Caterer agrees to follow and cause to be followed all State, Federal, and local rules, regulations and

policies regarding:

a. The preparation and service of food, and the service of alcoholic beverages;

b. All other health, fire, building and other laws related to the performance of Caterer’s services.

11. Indemnity Caterer covenants and agrees to indemnify, keep indemnified, save, hold harmless and defend Beth

David from and against any cost, loss, damage, liability or expense arising from or attributable to Caterer or any of

its officers, directors, agents, employees, contractors and others which occurs in whole or in part as a result of

Caterer or its officers, directors, agents, employees, contractors or others being on the premises or providing food,

services or alcoholic beverages on the premises.

12. Cancellation Due to Impossibility If, at any time prior to the Function Date, the premises are so damaged as a

result of fire, flood, windstorm or other catastrophe or act of G-d such that it is impossible to hold the Function on

the Function Date, and as a result, Beth David has exercised its rights under Paragraph 15 of its agreement with the

Customer, Caterer shall likewise be bound by and governed by those provisions. Those provisions are as follows:

“If, at any time prior to the Function Date, the premises are so damaged as a result of fire, flood,

windstorm or other catastrophe or act of G-d such that it is impossible to hold the Function on the

Function Date, Beth David shall have the option of either extending the Function Date for thirty

days, or returning to Licensee all payments which Licensee has made other than the Reservation

Deposit. If the damage or catastrophe is such that the Function cannot take place on the premises

by the end of the thirty day period, the Function shall be canceled and Beth David shall return

to the Licenseeall payments made by Licensee hereunder other than the Reservation Deposit.”

13. Cancellation for Cause Beth David shall be entitled to terminate Caterer’s right to cater the Function on the

premises and retain all amounts paid by Caterer hereunder if Caterer fails to satisfy all of its obligations hereunder

in a full, faithful and timely manner and fails to cure such defect within seven days after notice from Beth David of

such failure, except that there shall be no notice or opportunity to cure in the event that Caterer fails to make timely

any of the payments required to be made hereunder.

14. Miscellaneous

BETH DAVID GORDON RELIGIOUS SCHOOL TRAVEL PERMISSION

The undersigned, the parent/guardian of____________________ (the “Child) hereby grants permission for

my child to participate in any and all field trips arranged or conducted by Beth David Congregation, a not-for-

profit Florida Corporation (“Beth David Congregation”) and/or arranged or conducted by any of the groups

associated with or affiliated with Beth David Congregation.

I further authorize Beth David Congregation to act as my agent to, and I hereby grant my permission for

Beth David Congregation to obtain emergency treatment for my child in any event where it may not be

possible for Beth David Congregation to contact me in sufficient time to receive permission for such emergency

treatment. All costs, which may be incurred by reason of such emergency treatment performed for my child,

shall be my responsibility.

I further agree to release Beth David Congregation (and all groups and entities associated or affiliated

with Beth David Congregation) from any and all liabilities, actions, damages, causes of action, fees, costs or

expenses which may be incurred or suffered by my child during any field trip activities arranged or sponsored

by Beth David Congregation (and/or by any groups and/or entities affiliated with Beth David Congregation).
