CONFERENCE AGENDATHURSDAY, FEBRUARY 278:30 – 10:30 A.M. Hospice vs. Palliative Care and How They Can Reduce Hospital Admissions2 patient care hours
Speaker: Heather Bax, RN, BSN, MBA, Executive Director, Compassus Jefferson City, Missouri
By helping a patient and caregiver understand a disease process, symptom management and having nurses available on-call we can decrease the change of patients going back to the hospital. As we all care for patients we need to understand the patient’s goals of care. To begin, the patient needs to have a good understanding of their current health status and what the progression of their current disease processes look like. With identifying the patients goal of care we can assure the patient is receiving the appropriate level of care. Also with providing proper education the patient and caregivers can know what to do when symptoms arise. With all of these together provided by hospice or palliative care we can smooth transitions between levels of care, reduce readmissions and therefore improve utilization of Medicare funding.

Difficult behaviors in long term care whether merely disruptive or dangerous are among the greatest challenges to providing safe, effective residential care. Causes are varied, such as interpersonal conflict, confusion, or pain to name a few. This presentation will provide practical tips for managing difficult behaviors as well as tips for providing support to care staff that can lead to less conflict and a safer environment.

This session will explore recent marijuana guidelines and how it will impact your facility. Additionally, we will focus on recent and upcoming regulatory changes that have an impact on long-term care communities. Changes in both state and federal arenas will be discussed.

This presentation will review the interview/hiring process in an attempt to limit facility risk of hiring the wrong candidate and learn ways to identify candidate knowledge gaps and training needs to competently care for our residents.

This presentation will focus on two programs that the VA uses to address dementia-related behaviors; STAR-VA, an evidence-based protocol for behavior management in Community Living Centers, and Behavioral Recovery Outreach (BRO), a pilot program for addressing dementia- related behaviors of veterans receiving care in community settings such as VA-contract nursing homes, State Veterans’ homes, and at home with their families. There is no single way that people with Alzheimer’s disease or other forms of dementia think about or react to their symptoms; however, most are far more aware than we think. Many, perhaps most, experiencing cognitive decline become frightened. Without a trusted confidant, they may feel extraordinarily vulnerable. Having the conversation about care, especially care at the end of life, isn’t an intrusion or unkind. On the contrary, it may be the most intimate way to promise your loved one that you will be there for them. By listening carefully and respectfully, you share their worries and promise that even when they lose the capacity to make decisions, you will be their voice. You will respect them and their wishes. They will not disappear. Join us and learn conversation tactics to promote effective advance care planning communication with a person with dementia and use of the Where I Stand scales.
