Neurapraxia
Classification of Nerve Injury Classification of Nerve Injury
Neurapraxia
temporary conduction block with demyelination at the
site of injury
electrodiagnostic study ; normal above and below the
level of injury, no denervation muscle changes
recovery may take up to 12 weeks.
![Page 5: 3. pns injury_._](https://reader035.vdocuments.net/reader035/viewer/2022081718/5566dc82d8b42a70198b46b1/html5/thumbnails/5.jpg)
Axonotmesis
Classification of Nerve Injury Classification of Nerve Injury
Axonotmesis results from a more severe trauma or compression
cause Wallerian degeneration distal to level of injury and
proximal axonal degeneration to at least the next node of Ranvior
electrodiagnostic - : denervation changes of muscles
axonal regeneration = 1 mm/d or 1 in/mo
complete recovery with axons reinnervating (2nd degree injury)
incomplete (3rd or 4th degree injury)
![Page 6: 3. pns injury_._](https://reader035.vdocuments.net/reader035/viewer/2022081718/5566dc82d8b42a70198b46b1/html5/thumbnails/6.jpg)
Neurotmesis
Classification of Nerve Injury Classification of Nerve Injury
Neurotmesis complete transection of nerve
need surgery to restore neural continuity
electrodiagnostic findings are the same as fourth-degree injury
Sixth – degree injury (Mackinnon) mixed nerve injury that combines the other degrees of injury commonly occurs when some fascicles of the nerve are working normally while other fascicles may be recovering
![Page 7: 3. pns injury_._](https://reader035.vdocuments.net/reader035/viewer/2022081718/5566dc82d8b42a70198b46b1/html5/thumbnails/7.jpg)

Brachial plexus injury
Brachial plexus lesions can be divided into two types:
Upper brachial plexus lesion - Erb's palsy.
Lower brachial plexus lesion - Klumpke's paralysis
![Page 11: 3. pns injury_._](https://reader035.vdocuments.net/reader035/viewer/2022081718/5566dc82d8b42a70198b46b1/html5/thumbnails/11.jpg)
Brachial plexus injury Upper brachial plexus lesion
› Occurs from excessive lateral neck flexion away from shoulder
› Forceps delivery or falling on neck at an angle causes upper plexus lesions leading to Erb's palsy.
› Characteristic sign called Waiter's tip deformity due to loss of lateral rotators of shoulder, arm flexors, and hand extensor muscles.
![Page 12: 3. pns injury_._](https://reader035.vdocuments.net/reader035/viewer/2022081718/5566dc82d8b42a70198b46b1/html5/thumbnails/12.jpg)
Brachial plexus injury Lower brachial plexus lesion
› Sudden upward pulling on an abducted arm (as when someone breaks a fall by grasping a tree branch)
› C8 and T1 nerves are injured "either before or after they have joined to form the lower trunk.
› Klumpke's paralysis
› The subsequent paralysis affects, principally
characteristic sign is clawed hand, due to loss of function of ulnar nerve
intrinsic muscles of the hand
flexors of the wrist and fingers
![Page 13: 3. pns injury_._](https://reader035.vdocuments.net/reader035/viewer/2022081718/5566dc82d8b42a70198b46b1/html5/thumbnails/13.jpg)
Pain, especially of the neck and shoulder
Paresthesias and dysesthesias
Weakness in the extremity
Diminished pulses, as vascular injury may accompany traction injury
![Page 14: 3. pns injury_._](https://reader035.vdocuments.net/reader035/viewer/2022081718/5566dc82d8b42a70198b46b1/html5/thumbnails/14.jpg)

from the posterior cord and nerve
segment (C5-T1) called “Great
Extensor Nerve”
Sensory innervations are on the
posterior forearm and dorsum of
hand
supplies: all the extensors,
supinator muscle, skin of posterior
arm and hand, lateral hand and
thumb
![Page 16: 3. pns injury_._](https://reader035.vdocuments.net/reader035/viewer/2022081718/5566dc82d8b42a70198b46b1/html5/thumbnails/16.jpg)
high radial nerve injury above the
elbow => cannot extend the wrist (wrist
drop), thumb, upper arm (triceps damage)
low radial nerve injury at the level of
the lower forearm => no loss of motor
function + only loss of sensory function

Motor damage : result in not being able to make OK sign with fingers and inability to oppose thumb and 5th digits
