MY IBD Portal aims to engage people with IBD and provide greater opportunities for people wanting to take a more active role in their disease management.

The system should greatly improve communication between GPs and hospitals, but even more importantly (pause) it will provide a direct link with the person with IBD.

The project lends on the foundations of managing chronic conditions through Improving self care, patient empowerment and engaging patients in care planning are widely acknowledged as being the key to improving health outcomes and the patient experience for people with long-term conditions.MY IBD Portal will be an innovative internet-based tool for IBD Patients providing users with up-to date information about their condition. This system will allow IBD Patients to view their IBD health record securely online and obtain useful information linked to their condition. It will be secure, easy to use and will provide advice, helping patients manage their IBD.

Fortunately there is now quite broad consensus on what a better future should be. This Forward View sets out a clear direction for the NHS showing why change is needed and what it will look like.
