DETAILS OFTHE CONTRACTORS SITE MANAGEMENT PLAN INCLUDING OCCUPATIONALHEALTH AND SAFETY PROCEDURES MUST BE LODGED WITH THE MACEDONRANGES SHIRE COUNCIL PRIOR TO COMMENCEMENT OF WORKS.

5. ON COMMENCEMENT OF CONSTRUCTION WORKS ON SITE, THE CONTRACTORMUST COMPLY WITH THE RECOMMENDATIONS OF THE ENVIRONMENTPROTECTION AUTHORITY PUBLICATION “CONSTRUCTION TECHNIQUES FORSEDIMENT POLLUTION CONTROL” (PUBLICATION NO 275).

6. APPROPRIATE SILTATION CONTROL IS TO BE CARRIED OUT DURING THECONSTRUCTION AND MAINTENANCE PERIOD.

7. NO EXCAVATION SHALL BE CARRIED OUT WITHIN THE CANOPY DRIP LINE OR5.0m, WHICHEVER IS GREATER OF ANY EXISTING TREE UNTIL APPROVAL HASBEEN GIVEN BY COUNCIL'S SUPERVISING OFFICER OR HIS/HER APPOINTEDREPRESENTATIVE.

8. A TEMPORARY 1.5m HIGH POST & WIRE FENCE SHALL BE ERECTED ANDMAINTAINED AROUND EXISTING TREES, NOMINATED TO BE RETAINED AS PER ATREE PROTECTION ZONE (TPZ) CONDITION, AT ALL TIMES DURINGCONSTRUCTION. UNDER NO CIRCUMSTANCES SHALL SITE SHEDS, VEHICLES,MACHINERY AND EQUIPMENT BE STORED OR PLACED WITHIN FENCED ZONE OFTHESE TREES.

9. THE CONTRACTOR IS TO OBTAIN THE NECESSARY ASSET PROTECTION PERMITPRIOR TO UNDERTAKING ANY WORKS WITHIN A PREVIOUSLY CONSTRUCTEDROADWAY.

10. THE CONTRACTOR SHALL TO THE SATISFACTION OF THE CONSULTINGENGINEER AND COUNCIL'S REPRESENTATIVE PROVIDE AND MAINTAIN ALLNECESSARY WARNING SIGNAGE, LIGHTING AND BARRICADING TO COMPLYWITH THE REQUIREMENTS OF THE ROAD MANAGEMENT ACT.

11. THE LOCATION OF EXISTING SERVICES SHOULD BE DETERMINED BY THECONTRACTOR PRIOR TO COMMENCING ANY EXCAVATION BY CARRYING OUT ADBYD AND CONTACTING ALL RELEVANT SERVICE AUTHORITIES AND/OR THEIRAGENTS. ANY EXISTING SERVICES SHOWN ON THE DRAWINGS ARE OFFEREDAS A GUIDE ONLY AND ARE NOT GUARANTEED AS CORRECT.

12. SERVICE TRENCHES WITHIN THE ROAD RESERVE MUST BE BACKFILLED ASSPECIFIED BELOW, TO A DRY DENSITY NOT LESS THAN 97% OF THE MAXIMUMFOUND IN THE STANDARD COMPACTION TEST FOR THE FOLLOWING:

i. SERVICE TRENCHES BENEATH THE ROAD PAVEMENT, FOOTPATH OR DRIVEWAYCROSSOVER, TO THE UNDERSIDE OF THE PAVEMENT OR CROSSOVER BEDDINGWITH 20mm CLASS 3 FCR WITH 3% CEMENT ADDITIVE.

ii. DRAINAGE PIPES ADJACENT TO KERBING OR CONCRETE STRUCTURES WITH20mm CLASS 4 FCR TO A LEVEL THAT IS NOT AFFECTED BY A 45 DEGREE ANGLE

OF REPOSE FROM THE NEAR LOWER EDGE OF SUCH STRUCTURE, OTHERWISETO FULL DEPTH.

13. ALL EXCAVATIONS OF A DEPTH 1.5m OR GREATER MUST COMPLY WITHWORKCOVER VICTORIA. 17. 100mm DIA. AGRICULTURAL PIPE DRAINS TO BE PLACED BEHIND ALL KERB ANDCHANNEL ON 20mm AGGREGATE BEDDING, 200mm BELOW SUBGRADE, ATGRADE AND WHERE DIRECTED BY THE CONSULTING ENGINEER.

18. MINIMUM COVER TO OBVERT OF DRAINAGE PIPES SHALL BE 300mm INEASEMENTS AND 500mm IN ROAD RESERVES, EXCEPT WHERE PIPE CLASS '4'WILL BE INSTALLED.
