13) Announcements: The safety committee met on Wednesday January 11, 2017. The 2017 Safety Incentive Program information was signed by those in attendance, other departments being obtained. The new Qual Lynx Report Format was distributed with only one incident listed. Through the end of 2016, one GL claim involving an incident at the Library, allegedly a window fell on the arm of Olga Matos Fracturing her arm on 12/2/2016. Many training issues were discussed ranging from MSI CYBER Training, an Emergency Action Plan, MSI Training events being scheduled and attempts to have AC Electric re schedule the Live Wire Training. Two Safety Directors Bulletins were reviewed, Snow Removal Best Practices and Conducting Annual MVR Checks. There was spirited discussion as to the recent snowstorm event that took place Saturday 1/7. Several plowing and salting issues were discussed. The safety committee met on Wednesday February, 8, 2017. In addition to all departments representatives, we were joined by Commissioner Tim Kriebel and Rob Garisch of J A Montgomery. We reviewed 1 accident that involved one vehicle on 1/19/17 where Insured city vehicle collided with another vehicle with very minor damage. City Hall emergency action plan is in final stages with among other items such as fire extinguisher and AED locations, and meeting location in the event of building evacuation.With regards to Training initiatives, Ernie Gratz reviewed future g training sessions. In that department, Rob Garisch stated that Ventnor has developed one of the best training programs, Ernie received a round of applause. Police reported that they are in final stages of re accreditation under the new guidelines. They also are moving with Nar Can usage for Opioid Over Dose Cases on the rise. They are looking into the training and certification for this program believed to be through the State of NJ. Wellness; In addition to distribution of the monthly newsletter and Move It Monday, quarterly programs are being targeted with AtlantiCare or other topics health related. Look for further information. Lastly there was much discussion concerning events being planned. With events mentioned, it was agreed that a subcommittee involving Police, Fire, Public Works, and other related parties review these events for validity. Specific information is being sent concerning Use of facility application and agreement, Special event safety bulletin and generic check list, and information regarding skate board parks for coverage through the ACMJIF/MEL Program. 17) Commissioners Report/Comments

The safety committee met on Monday, March 6, 2017. There were NO accidents/incidents for the month of February to review. A reminder and head count for the JIF Safety Breakfast on April 6 was included in the agenda. The J A Montgomery written report from February was extremely complimentary. Safe Lifting for First responders of patients bulletin and instructions for training was included. Chief Hazlett


had obtained a quote for a special lift chair to be placed on the second floor in the event of evacuation and the elevator being shut down. The cost of $751.20 was discussed for re-imbursement from the 2016 Safety Incentive Program funds. The Emergency Action Plan for City Hall is in final stages. Safety Training continues, Blood Born Pathogen training has been completed, AED/CPR, Fire Extinguisher training is on tap and the Active Shooter in the Workplace is scheduled for 3/30. Wellness materials for the month and “Move it Monday” were distributed as well. Baseball season will be starting up shortly, so increased activity will be taking place at and around Titus Field. Police will be sending a representative for certification as a ATV trainer. This will have an “in house” Trainer for use of the ATV’s primarily on the Beach for PD,FIRE/EMS/ and Beach Patrol. The cost of the training will be submitted for reimbursement from the 2016 Safety Incentive Program funds. 22) Commissioners Report/Comments

24) Approve Bills & Payrolls 25) Announcements: The safety committee met on Wednesday April 12, 2017. There were two auto incidents reviewed. Participation at the Active Shooter Training held 3/30/2017 was excellent. The recent JIF Safety Breakfast held 4/6/2017 was well represented by Ventnor. In addition, Ernest Gratz received recognition for his continuing aggressive training programs. Ventnor also received an award for Safety Excellence. On that topic, safe lifting practices DVD was given for Police and Fire/EMS training, Playground Safety Inspections, with National Playground Safety Week the last week of April, and Distracted Driving Training available on the MEL Web site. CPR Training was ongoing in the library, and the Live Wire Training scheduled for May 4 & 5. Wellness was recognized for the AtlantiCare events with future event notices to all personnel that might not have direct e mails at work. Police Department advised that the final Re-accreditation inspection is scheduled for 4/23. A reminder that any signs missing or having blocked views, damaged curbs, sidewalk areas, etc. should be reported immediately due to the ongoing construction. 26) Commissioners Report/Comments

Safety on the Agenda

City of Ventnor, NJ

May 2017 report

The safety committee met on Monday, May 8, 2017. There were NO incidents to review and the open claims report was not available at the time of this meeting. Discussion focused on training issues. The Police Department has a certified trainer for use of the ATV Quad’s on the beach. Specific training should be scheduled for anyone in any department that will be authorized to operate these pieces of equipment. Ventnor is hosting several MSI (MEL Safety Institute) training classes scheduled for May, June, and July for various Safety topics. Playground inspection form, along with the National Manual on Playground Safety was provided. Wellness Director for the ACMJIF, Valerie Smith reviewed the monthly newsletter focused on Stretching and Posture. She also suggested scheduling now for ACME to provide FLU shots at no cost to employees at your location. The next meeting is scheduled for June 14 at 9:00am for anyone to attend. 23) Commissioners Report/Comments

10) ANNOUNCEMENTS: JIF SAFETY COMMITTEE REPORT FOR JUNE 2017: The safety committee met on Wednesday, June 14,2017.

There were two miner auto fender bender type items reviewed. Most of the discussion occurred during the round table session. Ernie Gratz is hosting several MSI training courses at the Library beginning with Haz Mat Training. He has also secured “live wire“ training through Atlantic Electric. Discussion regarding water treatment issues and chlorine options were discussed. Police Chief Biagi stated that Nar Can has been mandated by the County Prosecutors’ office to be carried by all officers. Additional training will take place to combat the opioid overdose issues that continue to plague society. It was also confirmed that Ventnor achieved re-accreditation, final certificate and approval letter to be forthcoming. Sari Appelbaum reported that there will be another wellness Fair event the last Wednesday of the month. Jerry Thomas stated that summer camp will be starting shortly and the Ventnor mile is scheduled for June 30th. There was a great deal of discussion on the April 27th Boardwalk Inspection performed by J A Montgomery, the ACMJIF/MEL Safety Director’s Office. There were many suggestions for improvement, mostly related to railing maintenance issues. The consensus was that most of these issues should be addressed between Labor Day and the end of April. These items are actively being worked on and progress anticipated. Next meeting is scheduled for July 12.

13) ANNOUNCEMENTS: JIF SAFETY COMMITTEE REPORT FOR JULY 2017: The safety committee met on Wednesday, July 12, 2017. After sign in and review of June information and approvals, 3 auto incidents were reviewed. A driver of a non-owned vehicle hit a City owned bench. The second item was a City owned vehicle rear ended another vehicle. At a stop signal. Damage was miner, however there were two occupants in the other vehicle. There is no predicting what their future actions could be. Lastly a large delivery box type truck struck a traffic light pole and left the scene. There were many witnesses, driver was found and summons issued. The Old Business section focused on the April 27th Boardwalk Inspection. Although, not directly under his department, Ernie Gratz stated it was his understanding that most of the improvements have been met. Donna Peterson will forward the appropriate form for response to J H Montgomery concerning these items. A fire drill at City Hall is in its final planning stages. Kathy Robbins stated that she was stuck in the elevator before she went on surgical leave and there was no apparent response to the “help” button. Discussion on this was that it should be connected to dispatch since there is always someone there. Training is going well per Ernie Gratz, he will be hosting several MSI (Mel Safety Institute) courses after reviewing those appropriate and needed. July 19 Ernie has scheduled Chlorine Gas training for water department, Police, OEM, and Fire/EMS. Deputy Chief Bloom stated that 70% of the Police has been trained and is currently carrying Nar Can and has already had 2 situations requiring use. Several Training Bulletins were included. Cyber Phishing posters were distributed by Jim Goss, look for these on bulletin Boards. Valerie Smith, Wellness Director reviewed the July Wellness Bulletin dealing with Summer Food Safety Issues. 12) ANNOUNCEMENTS: JIF SAFETY COMMITTEE REPORT FOR AUGUST 2017: City of Ventnor, NJ August 2017 report The safety committee met on Wednesday, August 9,2017. There were several auto related incidents in the agenda, one of which involved a rear end incident where Insured Vehicle rear ended other vehicle. This is the second time in two months that this has occurred, and a summons and complaint was already received on the first incident. From the open claims report, there were 5 WC incidents, 4 of which were medical only or report only. The 5th was an Indemnity, lost time incident to a fire fighter, the description of which was abbreviated and injury not defined. There was one General Liability claim reported of a slip and fall (trip) over loose boards on Boardwalk Ramp. We are still awaiting the completion of the 4/27/2017 boardwalk inspection items, hopefully this was not one of the items, as the loose boards were confirmed at inspection by PD. Other old business was the anticipated Fire Drill at City Hall, in its final stages awaiting alarm testing for functionality. Ernie Gratz confirmed that he will be holding a fire drill in the fall as required for the water utility, annually. Ventnor will host two Defensive Driving classes beginning 8/10 and one the following week. Wellness advised that Ventnor will hold a final event the last Wednesday of September, keep an eye out for this event. Next meeting is scheduled for 9/13/2017 at 9:00am.

City of Ventnor, NJ September 2017 report

The safety committee met on Wednesday, September 13, 2017. On the Old Business section, the April 27th, boardwalk inspection and SFI (suggestions for improvement) was updated and the report form completed and sent to J.A. Montgomery. There are a few remaining items that are still being worked on. On the Safety Training topic, continued activity included Jetter Vac training for those involved in the sewer division using this equipment. There was an inquiry as to Military Equipment for emergency use during events. The two vehicles currently stored at PW are on loan by the County and can be re-directed at will. With the President making Military Equipment available to Police Departments, it will be looked into further. The parking and maintenance issues were discussed as this evidently has been an issue. Hepatitis vaccinations were discussed for Public Works employees dealing with contaminated water, Fire stated that it was mandatory for EMS operations. Several outlets from the City Physician, County, and local Pharmacies would be explored for the best source. Basic Safety training was requested for all employees, this could be accommodated by in house training personnel in Fire and or Police departments, a good idea for. 14) ANNOUNCEMENTS: JIF SAFETY COMMITTEE REPORT FOR OCTOBER 2017:

Safety on the Agenda The safety committee met on Wednesday October 11. We reviewed 2 accidents a trip and fall at a large pot hole, and a slip and fall in City Hall on wet floor. Although there were signs and a fan present, there have 3 known incidents caused by this action by the cleaning contractor. There were two City Employees who fell, and the individual visiting City Hall. The discussion of these events suggested that the cleaning of the floor areas be done after hours to minimize traffic of both employees and visitors that are exposed to this exposure created by the cleaning service company. Furthermore, evidence of the Cleaning Service Company’s Insurance and Hold Harmless in Place should be sent in as subrogation will most definitely be pursued. There have been no further updates on open items regarding the April 27th Boardwalk inspection, and the Beach Patrol Inspection will be addressed in early 2018 regarding training issues as the season has already ended. Mr. Gratz stated that the current Chlorine Gas method used in the water department purification system is being phased out and replaced with a liquid injection method by the end of 2018. This is a major step in procedures both to employees as well as the community in proximity to current gas operations. Supervisors Incident reports from the ACMJIF web site were reviewed as to information gathering for additional insight into all events that could develop into a claim. These forms were distributed to all by Donna Peterson. Valerie Smith and Sari Appelbaum reviewed the wellness newsletter of the month focusing on opioid abuse with some interesting statistics and valuable articles as well as web sites for further information. Mike Cahill was introduced as New Fire Chief, he presented multiple training documents to the Safety Director, the Fire Drill at City Hall is in Final Stages, so exit when alarms ring to complete this required annual drill. 17) ANNOUNCEMENTS: JIF SAFETY COMMITTEE REPORT FOR NOVEMBER 2017:

Safety on the Agenda The safety committee met on Wednesday November 8. We reviewed 2 incidents. On 10/12/17 there was an auto related accident as described a vehicle hit a bus driving it into a police vehicle which was behind the bus. 3 Claims from this were opened. Workers Comp was medical only, no lost time to the officer. A property claim for the damage to the police cruiser, and an auto liability claim, as a fatality resulted to the driver of the vehicle. There was another property claim when a garage door fell on an ambulance at the fire station due to faulty sensor and remote which are in process of being repaired. From some old business, all city wide fire extinguishers have been tagged and in some cases re-located; all alarms are now functional in all buildings where personnel numbers require periodic fire drills. Training issues were continuing per Ernie Gratz, many sessions have been scheduled to host in 2018, and further pursuit of Hepatitis inoculations for many personnel dealing with blood borne pathogens training. In the wellness department, the monthly newsletter closed with accolades to Sari Appelbaum, Donna Peterson, Toro Aboderin, and Jim Goos for setting up 2018 wellness events. Commissioner Kriebel stated that improvements to Life Guard Headquarters, railings, decking are scheduled to be done which will answer some of the outstanding boardwalk inspection open items. Loss Time Accident Frequency rate: .74 as of October 31. Transitional Duty/Return to work light duty: 90% Average reporting time, 26 claims January-November: .8 days
