Appendix III: Advisory Protocol for G6PD Deficient Patients.

1. Typically, an episode of drug-induced hemolysis in G-6-PD-deficient individuals

begins 1 to 3 days after drug administration is initiated. Heinz bodies appear in the red

cells, and the hemoglobin concentration begins to decline rapidly. In severe cases

abdominal or back pain may occur. The urine may turn dark – even black.

2. We advise our G6PD deficient patients to avoid following drugs

Acetanilide

Doxorubicin

Furazolidone (Furoxone)

Methylene Blue

Nalidixic acid (NeGram)

Naphthalene

Niridazole (Ambilhar)

Nitrofurantoin (Furadantin)

Phenazopyridine (Pyridium)

Phenylhydrazine

Primaquine

Sulfacetamide

Sulfamethoxazole (Gantanol)

Sulfanilamide

Sulfapyridine

Thiazolesulfone

Toluidine blue

Trinitrotoluene (TNT)

3. Fava beans and soya products be excluded from food.

4. Cocoa items be incorporated in diet.

5. Incorporation of Folic acid, Iron and Vitamin C in food / supplements

6. Immediate reporting to Hospital in case of jaundice, anemia, hemolysis, abdominal or

back pain, dark / black coloured urine or pallor.

7. Regular CBC analysis and Cytochemical observations of blood for Heinz bodies and

Haemoglobinopathies be got done periodically.

8. Medical advice be taken for the selection of antibiotics, in case of microbial or viral

Infections.

9. Immediate reporting of cases of jaundice, anemia, hemolysis, abdominal or back pain,

dark / black coloured urine or pallor to hospitals for investigations.

10. Careful vigilance on the stresses triggering / precipitating haemolysis / other G6PD

related episodes and identification of drugs and diets including broad beans, lentils and

peas as triggers.

11. Blood transfusions may be needed.

12. Dialysis may be required in cases of acute kidney injury.

13. Blood exchange transfusion may be required in cases of Infants.

14. Avoid transfusion of G6PD-deficient blood

15. Supplementation of Sn-mesoporphyrin (SnMP) a potent inhibitor of bilirubin may help in

G6PD deficiency

16. Supplementation of Pycnogenol dry extract of the French Maritime pine (Pinus pinaster)

bark that has high ability to scavenge free radicals and provide protective action against

chemical induced haemolysis in G6PD deficient human erythrocytes may be tried.

17. In case of recurrence, particularly in case of females, get the heterozygosity test

conducted and if the test is positive, such cases need vigilance and prompt care.

18. To avoid appearance of this enzymopathy in the family progenies, G6PD deficient

individuals should avoid marriage with a G6PD deficient partner.

19. Get all the Neonates, children and females of the family screened for G6PD enzyme

status at a reputed Test Centre.

20. The drugs, listed below, do not produce clinically significant hemolytic anemia and can

be given safelyin normal therapeutic doses to G-6-PD-deficient subjects. Most G-6-PD-

deficient patients have taken aspirin without untoward effect:
