DEADLINE FOR EVALUATIONS 7/17/2015

1. ASE Members: Go to www.asecho.org and log into the Member Portal with your ASE member e-mail address and password.

a. Click on ASEUniversity and CME Credits (This will take you directly to your ASEUniversity profile)

2. Non-Members: Go to www.ASEUniversity.org, create a new account or log into your existing ASEUniversity account.

3. Once logged into ASEUniversity, the top of the page should say, “Hello, ___.” Click Your Account.

4. Under Your Account, click on Add Access Code and enter the code, ASE20155. Verify your registration information. Please be sure to use the same information that you used to register for the event.

6. Once verified, click Take Evaluation to claim your CME credit. CME credits are not earned until the entire evaluation has been completed.

7. Your certificate will be available for immediate download and can also be found under Your Account in the CE Transcript.

• Complete an evaluation (in ASE’s Online Library) following the instructions on the previous page. • Please note that in order to obtain credit hours for the sessions you attended at the 26th Annual ASE Scientific Sessions you must complete the online evaluation process by July 17, 2015.

Obtain Your CME Certificate

Your CME certificate/transcript for this Conference can be accessed by clicking on “CE Transcript” under “Your Account.”Please note that you will be able to access and print your certificate at any time after you have completed the evaluation process.

Your continuing medical education (CME) records for the conference may be printed as needed from ASEUniversity (www.ASEUniversity.org) in the “CE Transcript” section.
