In Florida - FHM submitted 14 suspected fraud cases to the State of Florida Division of Insurance Fraud from 7/13 - 5/15.

• Three arrests were made • Nine cases are being actively investigated by the state

In Georgia - FHM submitted 3 suspected fraud cases to the Georgia State

Board of Workers’ Compensation from 7/13 - 5/15.

• Two arrests made, one additional arrest warrant issued • One case being actively investigated by the state

Since July 2013, the courts have ordered restitution to FHM Insurance in the amount of $31,887

Since January 2007, investigations conducted by FHM Insurance Company have resulted in:

• The arrest of 27 claimants; issuance of arrest warrants in 4 additional cases, and restitution awarded to FHM in the amount of $275,923

• One claimant arrested and prosecuted by the United States District court for postal fraud

• Investigations into two medical providers resulted in the recovery of $9,976 when it was discovered services billed for were not provided
