The West Virginia Primary Care Association (WVPCA) is excited to announce that we will again be partnering with Jennifer Calohan, of CURIS Consulting, to bring your health center an innovative, fresh, and pertinent approach to quality improvement.

Background

LEAN principles are used to create the most value for the customer while minimizing waste. The LEAN approach is about understanding what’s really going on, and improving the processes by which services are created and delivered. Six Sigma is shorthand for a discipline that allows any business (health care entity) to design, improve and manage its processes so that they perform at their highest possible levels. Effective and efficient processes also help reduces staff turnover and increase retention by eliminating one of the main causes of high turnover- cumbersome and complex pro-cedures and routines. Combining the principles and practices of LEAN and Six Sigma and tailoring them to the health care environment will increase efficiency and reduce costs.

With trends in healthcare continuing to shift toward value-based reimbursement models and performance-based pay-ment methodologies, there is an increased level of importance placed on quality and performance data. In order to thrive in the value-based care environment, Community Health Centers must have a strategy and a commitment to improving performance across the domains of clinical, operations, finance, and quality. Establishing and driving a cul-ture of continuous improvement supported by the principles and concepts of LEAN/Six Sigma is an essential piece of the necessary strategy for health centers to optimize their resources and maximize their reimbursement opportuni-ties. Improving diabetes outcomes will be used as the case study for the learning collaborative; however, the princi-ples learned can be applied to improve processes and outcomes throughout your health center.

The Program

The WVPCA Learning Collaborative is focused on leveraging LEAN/Six Sigma principles and practices and applying them to improve outcomes in Community Health Centers.

The program will provide multi-modal training on the foundational elements, concepts, and principles of LEAN/Six Sigma as it relates to healthcare with focus on real world scenarios specifically applicable to the Community Health Center workplace(s).

Participants will receive a certificate of completion. The program will provide participants with core components of LEAN/Six Sigma to assist with preparation for yellow and/or green belt professional certification.

During the 10-month collaborative timeframe, teams will receive training from subject matter experts, collaborate with one another in peer-to-peer learning activities, and develop strategies to enhance the programs and ser-vices available to the patients/communities they serve.

Participants will develop new strategies and strengthen existing organizational infrastructure, leadership structure, quality management programs, and front-line care delivery models while being supported by an expert faculty/facilitator.

Each participating team/member will receive a comprehensive compilation of resources and materials to support ongoing improvements at their Community Health Center. Support will be provided by the WVPCA Health Center Controlled Network (HCCN) as sponsors of this work and by the CURIS team.

Learning Sessions will be provided in a combination of onsite/in-person workshops, remote webinar sessions,

and virtual 1:1 office hours for each participating health center.

Commitment

Participating health centers should be prepared to commit dedicated time to participation, collaboration, learning, and improvement.

Each participant should anticipate 4-6 hours each month on average to optimize results and success related to the learning collaborative opportunity.

Time commitment and level of engagement will have direct correlation and impact on optimization and level of benefit to the participating organizations.

Principles

Participating health centers are committed to improvement. Investment of time and resources will result in strategic and sustained change. Active participation, including sharing of lessons learned, is essential to collaborative success. When indicated, preparation in advance of learning sessions may be necessary. Faculty/facilitators will commit to expertise, quality, responsiveness and engagement with participants.

Target Audience for Learning Sessions

Ideal team composition will be determined based on the topic of focus and curriculum. This collaborative will be structured into 2 primary teams–

1. Health Center Leadership Team (Administrators, CEO, COO, CMO, Medical Directors)a. Focus will be placed on:

i. Strategic Organizational Leadership Strategiesii. Leadership Role in Driving Positive Organizational Culture

iii. The Role of Leadership Engagement & Support in a Kaizen Culture ofContinuous Improvement

2. Cross Functional Quality & Operations Team (Provider Champion, Quality Improvement Director/Manager/Coordinator, Director of Nursing, Nursing Manager, Data/Information Systems Manager,Clinical Operations Manager)

a. Focus will be placed on:i. Etiology, Risk, Prevalence, and Impact of Diabetes at State and National Level

ii. LEAN/Six Sigma Concepts and Principles - including Change Managementiii. Contributing to a Kaizen Culture of Continuous Improvementiv. Integrated Team-Based Care Delivery Modelsv. Care Coordination and Care Management

Rationale

A learning collaborative is an initiative in which teams of leaders and peers come together to study and apply quali-

ty improvement methodology to a focused topic area. First conceived by the Institute for Healthcare Improve-

ment (IHI) in 1994, learning collaboratives help organizations apply known improvement principles to current

health care practices with the goal of achieving “breakthrough improvements in quality while reducing costs.”

Pricing

This 10-month collaborative program is available for $600* per participant. Participants will receive a

certificate of completion. The program will provide participants with core components of LEAN/Six

Sigma to assist with preparation for yellow and/or green belt professional certification.

* Please note while the cost is per participant, teams of 3 are highly encouraged to reap the full

benefits of the collaborative.

Jen leads the CURIS Consulting team as Principal Consultant. She has more than 20 years of experience in the

healthcare field. A Registered Nurse by background, Jen began her career in the clinical arena, expanded into leader-

ship, administration, and finally - consulting. Jen’s extensive clinical and operational expertise, combined with her pas-

sion for quality, performance improvement and change management make her a valuable asset to the CURIS team and

to her clients. Jen is deeply committed to advancing clients’ transformative efforts toward high quality patient-

centered care delivery.

Prior to jointly launching CURIS Consulting, Jen’s 20+ years in healthcare has included direct patient care, community

hospital leadership, Quality Improvement Council, health center and network leadership, HIT/software leadership and

consulting. Jen served as Executive Administrator for a Rural Health Clinic where she led her health center team

through HIT and PCMH transformation. She then transitioned into consulting, where she has spent the last 10 years

working with hospitals, IPA’s, Health Centers, PCA’s and HCCN’s and HIT Software vendors to promote clinical integra-

tion of technology, develop patient centered care delivery models, quality and process improvement, population

health management, and the development of clinical and operational programs to enhance outcomes.
