The post requires the ability to work without direct supervision' experience of working within palliative care is desirable. St. David's Hospice works in partnership with Bangor University and can offer an accredited Palliative Care Module in-house if necessary. The successful candidate will spend time in the main Hospice in Llandudno working with experienced ANP/NMP spending time with the nursing and medical team before starting at our new four bedded nurse led Hospice Satellite Unit in Holyhead on Anglesey.

About St David’s Hospice

Brief History

St David’s Hospice was opened in 1999 following a local fundraising effort by a group of dedicated volunteers.

The land on which the Hospice stands was acquired from the council for the sum of £1 and the work develop to Hospice building got underway. Over the following years services developed, fundraising increased and the building expanded. In 2013 a £1.2m extension was completed to provide a total of 14 single inpatient rooms, and education centre and improved facilities.

St. David's Hospice is a local charity, caring for local people, delivering specialist care to adult patients with advanced illnesses or those in need of end of life care and their families. Every year, this touches the lives of over 1000 people within our local communities from across the counties of Conwy, Gwynedd and Anglesey.

We offer 24 hour care, 365 days a year providing a clinical service alongside emotional, physical and spiritual support. In our Day Care Unit we provide patient care, bereavement and counselling service, whilst our Inpatient Unit offers symptom control, respite and end of life care.

It costs over £3 million each year to deliver the services - that's over £8,000 a day; £351 an hour. The majority of this comes from community fundraising. None of these costs are passed to the patients, their families or carers, because St. David's Hospice care is free to all.

Since 1st April, 2018, Hospice at Home Gwynedd and Anglesey has merged with St. David's Hospice; this allows us to work together more effectively to provide excellent end of life care to the people of North West Wales.

St. David's Hospice believes that everyone deserves high quality end of life care. St. David's Hospice Holyhead Project is an ambitious plan to convert an eight bay ward at Penrhos Stanley Hospital into four single-bedroom units for Inpatient care. The nursing standards will replicate the high standards of specialist care provided at St. David's Hospice, Llandudno and complement the existing Hospice at Home service on Anglesey.

Why is this development taking place? According to Hospice UK, 1 in 4 families who need hospice care, can't get it. Too many people are unable to die in the place of their choice. Feedback received from local patients, carers, families, GP's and healthcare professionals have told us that there is a clear need for these facilities on Anglesey. We believe that our exciting plans will offer choice and end the postcode lottery that exists.

It is hoped that internal building work at Penrhos Stanley Hospital will commence in the summer and that the 4 bedded unit will open early in 2019.

It is anticipated that the project will create 15 new jobs for local people.

All nursing staff will be trained at St. David's Hospice in Llandudno, Conwy initially before transferring to Penrhos Stanley in Holyhead, Anglesey.

How will it be funded? The project will cost in the region of £500,000 to establish the unit in Holyhead then annual running costs thereafter. Betsi Cadwaladr University Health Board and Isle of Anglesey County Council will be providing short-term funding for the first two years towards nursing salary costs (30% in year 1 and 15% in year 2).

As well as the main site situated on Abbey Road, Llandudno the Hospice has a number of retail operations. There are a total of 25 Hospice shops across North West Wales, three warehouses and a refuse centre.

Governance

St David’s Hospice is a registered charity. It is governed by a Board of Trustees under the leadership of the Chairman – Dr. Lyndon Miles. The Trustees are all volunteers coming from a range of backgrounds. The Board meets regularly and has three sub committees, namely, Finance and HR, Income Generation and Clinical Governance.

Our People

The Hospice employs 156 staff (full and part time, including bank staff) in a wide range of roles. We also have a loyal and hardworking team of over 550 volunteers working in all areas of the Hospice including our shops.

Medics, Nurses, Care Assistants, Social Workers, Spiritual Care, Bereavement, Complementary Therapies, Physiotherapy and Occupation Therapy, Education and Housekeeping and Activity Leader.

Fundraising

We have an energetic, busy and committed fundraising team who run a number of events and activities throughout the year as well as maintaining other streams of income.

Our Hospice lottery has around 11,000 members and our community fundraising team is responsible for major events, generating community-led support and developing new ideas and activities.

We are also successful in seeking a range of annual grants and trust donations and are keen to develop more meaningful corporate partnerships with mutual benefits.

Finance

It costs over £3 million pounds a year for the Hospice to continue to provide its services. Over 86% of our income is generated through community fundraising.

Hospice finances and resources are managed carefully and prudently. Due to good public support and efficient housekeeping since the Hospice opened, we are in a position to hold reserves.

All our services are provided without charge to our patients, their families and carers.

Geographical Area

The Hospice catchment area is North West Wales which includes the counties of Conwy, Gwynedd and Anglesey. This is a population of approximately 330,000.

JOB SUMMARY

The post holder is an experienced nurse, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the satellite unit. They will work collaboratively with the multi-disciplinary team in the main Hospice to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.

1. SCOPE AND PURPOSE OF THE ROLE

To deliver a high standard of patient care as an Advanced Nurse Practitioner (ANP) in the satellite unit, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.

To manage a clinical caseload, dealing with presenting patient’s needs in a nurse led inpatient setting

To participate in developing a nurse-led service

4. PRIMARY DUTIES & AREAS OF RESPONSIBILITY

A. CLINICAL ROLE:

The post-holder will:

Make professionally autonomous decisions for which he/she is accountable Provide a first point of contact within the satellite unit for patients presenting with

undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan

Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports

To prescribe safe, effective and appropriate medication as defined by current legislative framework

Provide safe, evidence-based, cost-effective, individualised patient care Offer a holistic service to patients and their families, developing where appropriate

an on-going plan of care/support. Refer patients directly to other services/agencies as appropriate

Work with St David’s clinical team in developing and delivering specialist palliative care services within the environment of a satellite service using St David’s medical team to support and advise ongoing care

Work with nursing, medical and health care assistant and other Allied Health care Professionals (AHP’s) to ensure National Palliative Care Standards are met.

Work with multi-disciplinary teams across the community, to promote integrated and seamless care

Contribute to St David’s Hospice achieving its quality targets to sustain the high standards of patient care and service delivery

Participate in identification of community local needs and develop patient/family-centred strategies to address them

Help develop and set up new patient services and participate in initiatives to improve existing patient services

B. TEACHING AND MENTORING ROLE:

The post-holder will:

Promote a learning environment for patients, nurses and other health professionals.

Contribute to the planning and implementation of the teaching for staff

C. PROFESSIONAL ROLE:

The post-holder will:

Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.

Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review

Maintain their professional registration Participate in continuing professional development opportunities to ensure that up-

to-date evidence-based knowledge and competence in all aspects of the role is maintained

Work within the latest NMC Code of Professional Conduct Record accurate consultation data in patients records in accordance with the latest

D. MANAGERIAL ROLE:

The post-holder will:

Encourage and develop teamwork within the Hospice. Help the Hospice operate in a cost-effective manner. Identify and manage nursing care risks on a continuing basis.

Participate in Hospice meetings and Hospice management meetings, reporting progress as required.

Participate in audits and inspections as appropriate. Work closely with the doctors and administrative managers in the setting up

and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets.

Ensure that all Hospice Policy’s are fully implemented

E. AREAS OF PRACTICE EXCLUDED FROM THE POST-HOLDERS REMIT:

6. HEALTH AND SAFETY/RISK MANAGEMENT

The post-holder must comply at all times with the Hospice’s Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System.

The post-holder will comply with the General Data Protection Act and the Access to Health Records Act (1990).

10. SPECIAL WORKING CONDITIONS

The post-holder is required to travel independently between practice sites (mileage allowance applies according to the Travel Policy and terms and conditions of

employment)

The post-holder will have contact with body fluids (i.e. wound exudates; urine etc.) while in clinical practice.

11. REHABILITATION OF OFFENDERS ACT 1994

Because of the nature of the work, this post is exempt from the provisions of Section 4(2) of the Rehabilitation of Offenders Act 1994 (Exceptions) Order 1995. Applicants for posts are not entitled to withhold information about convictions which for other purposes are “spent” under the provisions of the Act and in the event of employment any failure to disclose such convictions could result in disciplinary action or dismissal by the Hospice.
