The individual consultations are with a dietician and group sessions are offered by the obesity clinic. The obesity clinic is a 10 week group session program that is offered at the YMCA locations in Boston, Brockton, Lowell, MetroWest, and Woburn. Collaboration between a medical doctor, nutritionist, psychologist, and health coaches offers a holistic approach for each two hour visit, consisting of a nutrition/behavior change program (one hour) and a physical activity program (one hour).

The program focuses on the identification of specific, achievable goals that are implemented in a step-wise fashion over time. The concerns of asthma and obesity being highly correlated, the team believes that by targeting the issues associated with both asthma and obesity, they will work towards preventing one from arising from the other. After initial evaluation of a patient, an individualized plan is created for the patient and monitored with close follow-up for as long as necessary.

After an initial consultation that evaluates the overall necessities of each individual patient, a close follow-up plan is developed with the health coach and the rest of the team.

Nutrition and Fitness for Life Boston Medical Center

After physician evaluation, patients are provided with one-on-one nutritional/behavioral counseling as well as referrals to community-based programs that lend to a healthier lifestyle. Visits occur once a month for the first three months followed by once every three months for the remainder of the care.

Individualized treatment is determined at the initial patient visit and then monitored and modified over several months of follow-up visits every 2 months.

Pediatricians, registered dieticians, a behavioral psychologist, and a nurse educator work together to help patients and their families make healthier lifestyle choices through nutritional education, personalized diet modifications, links to community resources for physical activity, and ongoing social support. Frequency of care is variable patient to patient.

After the initial visit, patients have variable follow-up schedules consisting of one-on-one services and group activities such as fitness and cooking classes. Collaboration amongst a case manager, nutritionist, nurses, and the patient’s PCP allows for overall health improvement through the implementation of healthy habits and lifestyle changes/goals.
