About My Way My Way is the next step towards more personalised, individualised services. Through My Way, people with disability, their families and carers are able to exercise genuine choice and control over the supports and services they need to live a good life in their local community. My Way builds on the Commission’s Local Area Coordination program which has been supporting West Australians with disability and their families and carers since 1988. The project has been developed in consultation with more than 1,000 people and includes feedback and knowledge drawn from people with disability, their families and carers and disability organisations. Project areas Beginning in November 2012, the project will run for three years in four areas across the state: Goldfields Lower South West Cockburn/Kwinana Hills These project areas will ensure representation from a broad cross-section of the population. My Way objectives Individualised, self-directed supports and services: to increase choice and control for people with disability, their families and carers over their supports and services to ensure their identified needs are met. Good life planning: to increase engagement in planning with people with disability, their families and carers with a focus on a good life rather than funding. Stronger connections with community and services: to enhance community belonging for people with disability in their local area. Eligibility All people with disability who live in the identified project areas and are eligible for Commission provided or funded services will be invited to participate in the project. Participation is voluntary and people can choose to join in or continue with their current arrangements.

My Way Coordinator roleCoordinators provide ongoing support to help individuals, their families and carers to access information, plan for the future, participate in their community and identify supports and services.

My Way Coordinators will be based in both the government and non-government sectors.

Monitoring and evaluation is an important part of the My Way project. Lessons learnt will inform how disability supports and services are provided in the future.

My Way Coordinators: • buildandmaintaineffectiveworking

relationships with people with disability, families and carers in the local area

• provide accurate and timely information and assist people with disability to participate in their local community

• engage with people with disability to identify their goals, strengths and needs, and plan for the future

• support people to gain the expertise andconfidencetoexerciserealchoiceand control

• support people to use personal and local community networks and develop practical, local solutions to meet their goals and needs

• assist people with disability, families and carers to access supports and services, including access to funding as appropriate.
