Getting started
• Accountability Plan accounts for 15% of total score
• Provides a valuable summary for reviewers of what
applicants plan to do with F5AC funding
• You will create a Plan for each year of funding
Step by step instructions for completing your Accountability Plan are
available on pages 3-5 of the Application Instructions for Targeted
Grants
First 5 Alameda County 2013-2017 Strategic Plan Results and Outcomes
From: 2013 ̶ 2017 F5AC Strategic Plan
Text will be

What do you hope will change for children and families?
• no simple to this question
“
TIP: Review the required reporting measures
for each outcome
(Appendix A of Application Guidelines)
Step 1: Project Specific Outcome(s)
Add your project-specific outcomes (not more than 2)
Should align well with the First 5 Alameda County outcome you selected
• no simple answer to this question
“
First 5 Alameda County Outcome:
Enhanced parent knowledge of child development/school
readiness and parent capacity to support their child’s
development/school readiness
Your Project-Specific Outcome (examples):
Increased parental knowledge of developmentally appropriate
expectations
Increased parental knowledge of school readiness and use of
in-home strategies to support school readiness
Step 2: Strategies
Add strategies for each project-specific outcome
What strategies will you use to achieve your outcomes (e.g., provide a service, conduct a training, conduct outreach)?
Begin with a verb (e.g., “Provide,” “Purchase,” “Offer”)
Include a strategy for each major area of requested funding
• no simple answer to this question
“
Strategies (examples)
Provide weekly 2-hour parenting education groups
Offer monthly one-on-one parenting support visits to parents
attending program
Provide 1.5 hour parent-child playgroups three times a week
Distribute Parenting Kits in 3 languages
Step 3: Performance Targets
Add performance targets for each strategy
Targets should be specific, realistic, measurable, and time-limited
Be sure to include:
Units of service
Number of clients to be served
Note: If funded, performance targets will be used to track progress (i.e., “met,” “in progress,” or “not met”)
Performance Targets
Separate out units of service and numbers of clients
10 parenting classes
Minimum of 35 parents participate
Not: 10 parenting classes for a total of 35 parents
Consider including targets that go beyond units of service and number of clients
100% of children who score “of concern” on the ASQ will be referred for assessment or services
75% of parents will attend a minimum of 9 classes
Sample Accountability Plan
F5AC Outcome: Enhanced parent knowledge of child development/ school readiness and parent capacity to support their child’s development/school readiness

curriculum

• 80% of parents attend at least
7 of 10 sessions
Step 4: Reporting Measures Documenting your progress and outcomes
Add reporting measures (in addition to the required
measures already selected)
Select reporting measures that describe:
The number and type of services provided (effort)
The impact services have on clients (effect)
Try to include:
Measures of quality and quantity
How well did we deliver
service?
Quantity Quality
Effort
Effect What quality of change

How much change
did we produce?
How much service
