This informal phonics assessment will help determine which series of Phonic Books is appropriate
for your child/student. It is quick, easy to administer, and easy to score!
You will be asking your child/student to identify letters/sounds and words. This assessment is
divided into four “sets”:
Set 1: Letters & Sounds
Set 2: Single syllable words with digraphs, blends, suffixes –ed, -ing and –cle.
Set 3: Words with silent e, vowel teams, r-controlled vowels.
Set 4: Words with advanced vowel teams and irregular sound/symbol correspondences.
As your child progresses through this assessment, you will be looking for two things: ACCURACY &
AUTOMATICITY (explained on the following pages).
Do I Need to Administer the Entire Assessment?
No. If your child reaches frustration (below 90% accuracy or automaticity), stop the assessment
and score completed portions.
Why Do Sets 3 & 4 Have Multiple Pages?
There are many types of sound—symbol correspondences covered in levels 3 & 4. The
assessment must include at least four items from each category to determine if it’s a strength or
weakness.
What Do I Do Once I’ve Finished Administering the Assessment?
Once all the scores are calculated, use the flow chart to determine which book series is best for
your child. The flow chart is at the end of the assessment.

What is Accuracy?Accuracy refers to the ability to read words correctly.
Why is Accuracy Important?Accuracy is a very important piece of reading fluency. Students must be accurate
before they can be automatic.
How Do I Calculate Accuracy?Follow along as your child/student reads the words. Put a check next to words that
are read correctly. For words read incorrectly, write what your child/student read
next to the correct word. If your child/student self corrects, write SC next to the
word to show it is not an error. See example on page _____.
When your child/student has finished each set of the assessment, calculate the
following:
1. Count the total number of words read correctly.
2. Divide the total number of words read correctly by the total number of words.
3. Use the table below to determine if your child/student has the accuracy
necessary to be successful at this level.

What is Automaticity?Automaticity refers to the ability to read automatically.

at every stage of reading, beginning with letter sounds and progressing through the
stages of word reading.
How Do I Calculate Automaticity?For this assessment, put an e above above letter/word your child does not identify
automatically. The e represents “effort”. The goal is automatic, not fast. You should
put an e when your child/student hesitates, sounds out, repeats a word, etc. See
example on page ______.
When your child/student has finished each set of the assessment, calculate the
