2. Type of incident: Death of a Detainee

3 Date/time of incident: 231625CJULQ9

4.Additional information: 232027CJUL09, CID reported that their preliminary investigation revealed that about 2316Q0CJULQ9, several Detainees began banging on the door of their MDHU. The Guard Force immediately inquired into the disturbance, and was informed that Detainee iSNi-^iE) was not breathing. The Guard Force subsequently cleared out the MDHU and brought the Detainee to the TIF Medical Station, where CPR and life saving measures were begun. The Detainee was then evacuated to the TF Hospitai, where life saving measures continued unsuccessfully. The Detainee was pronounced dead at 2317Q3CJULQ9 by a Medical Doctor at the TIF Hospital. The Detainee was last seen alive by the Guard Force about 23150QCJUL09, during a routine head count.

An examination of the Detainee revealed no outward signs of trauma or injury. The liver and rigor mortis were consistent with the Detainee expiring about the time of guard notification. The Detainee had previously been diagnosed with hypertension, diabetes and high cholesterol and was taking the following prescription medication: Gemisbrozil, Melormin-Glucophage, Simuvastatin, Atenold and Lisinoprii, tl was unknown if the Detainee had any conflicts with olher Detainees within his MDHU.

A dealh scene examination revealed no indication of a slruggle, trauma, or other signs of foul play.

Al 2S1400C JUL09, Ihe remains were moved to TaSlil.

Al 251859CJULQ9, Ihe remains had arrived at Tap , from Ihere: the remains will be shipped to Dover, DE by the Mortuary Affairs Team. Once the autopsy is complele, the remains will be returned to morluary affairs. Mortuary Affairs, then has Ihe responsibility of noticing Ihe family and resolving Ihe final disposition of the body. No further action is required of this unit .
