15. COURT ORDER:

Authorizing the Purchasing Department to solicit Statement of Qualifications (SOQ) for Professional Construction Project Management and On-Site Project Representation in accordance with Government Code, Chapter and Dallas County's established policy and procedures.

Authorizing Sigma Mechanical Contractors to proceed with the work necessary to install spring isolation systems on the exhaust systems located on the roofs of the laboratory and morgue buildings of the new Institute of Forensic Sciences building through a discretionary exemption as allowed by Local Government Code Chapter 262.024(a) (1); and authorizing Amendment No. 1 to the professional services agreement with HHS Applications, LLC for construction management services for the project.

Approving submission of a grant application to the Office of the Governor, Criminal Justice Division (CJD), for the operation of the Dallas County OWl Misdemeanor DIVERT Court project for 2010-2011 year; and designating the County Judge as the grantee's authorized official given the power to apply for, accept, reject, alter or terminate the grant on behalf of the applicant agency.

18. COURT ORDER:

Approving submission of a grant application to the Office of the Governor, Criminal Justice Division (CJD), for the operation of the Dallas County OWl Felony DIVERT Court project for 2010-2011 year; and designating the County Judge as the grantee's authorized official given the power to apply for, accept, reject, alter or terminate the grant on behalf of the applicant agency.

Approving submission of a grant application to the Office of the Governor, Criminal Justice Division (CJD) f for the operation of the Dallas County Felony Female Offender DIVERT Court project for 2010-2011 year; and designating the County Judge as the grantee's authorized official given the power to apply for, accept, reject, alter or terminate the grant on behalf of the applicant agency.

20. COURT ORDER:

Approving the outlined FY 2011 agreement with the Dallas County Hospital District Board of Managers for Dallas County to perform specific Commun Health Services for the District and to provide Ambulance Service, and County Judge to sign the agreement on behalf of Dallas County.

21 .. COURT ORDER:

Approving the outlined renewal contract for services with Dallas County Community Supervision and Corrections Department (CSCD) for a one (1) month extension for the current Post-Trial Interlock Program, funding for the services during the one (1) month extension shall not exceed $1 6,842.75, and authorizing the County Judge to sign such contract on behalf of Dallas County.

22. COURT ORDER:

Authorizing $90,305 of the County's FY 2010 Emergency Shelter Grant (ESG) award be allocated to The Bridge as part of the County's FY 2011 $790,000 contribution, that Arnendment #2 to the existing City-County Agreement for The Bridge (Court Order 2009-1785) be executed by the County Judge to reflect this $90,305 allocation, and the remaining $2,793 from the County's FY 2010 ESG avvard be allocated for the administration of the grant.

23. COURT ORDER:

Authorizing the prOVISion of a five-year 50% tax abatement on real and business personal property for the expansion and modernization of Maxim Integrated Products' Farmers Branch facilities, effective no later than January 1, 2013, provided that the project retains 800 full-time existing jobs during each of the abatement's five years and increases, by the abatement's effective date, the County's tax base by at lease $15 million, and authorizing the County Judge to sign the related tax abatement agreement for this project.


24. COURT ORDER:

Approving the reallocation of $1 6 81 6 in funds from Purchase Order Number 222814, and authorizing the purchase of the customized in-house training course by Hyland Software.

25. COURT ORDER:

Approving the reVISions to the Nepotism Policy, Sections 86-241 - 86-245 f for immediate inclusion in the Dallas County Code.

26. COURT ORDER:

Authorizing the County Auditor to process the necessary payments and transactions to clear unresolved losses totaling $36,349.18 from County Wide Self Insurance (120.9910.07560), $1,590 from District Clerk Errors and Omissions (532.0000.21437) and $26,612 from County Clerk Errors and Omissions (532.0000.21438) .

27. COURT ORDER:

Approving the funds received under the 2010 Reliant Energy CARE Program, in the amount of $2,500, for energy assistance payments to eligible Reliant Energy customers.

28. COURT ORDER:

Approving The Centers for Disease Control and Prevention Grant #1 U58CD001278-01 (CD1 0-1 011 Strengthening Public Health Infrastructure for Improved Health Outcomes); the effective budget period for said grant is September 30 f 2010 through September 29, 2011 .

Approving the Amendment to the Texas Department of State Health Services (DSHS) Contract No. 2011-036727-001 A, Immunization Branch - Locals, for the period of September 1 I 2010 through August 31 I 2011! and authorizing the County Judge to sign all related documents on behalf of Dallas County.

31. COURT ORDER:

Approving the Amendment to the Department of State Health Services (DSHS) Contract No. 2011-037262-001 I Tuberculosis Epidemiologic Studies Consortiums Tuberculosis Task Order #23, for the period of October 1 f 2010 through September 30, 2011, and authorizing the County Judge to sign the contract on behalf of Dallas County.

32. COURT ORDER:

Approving the request of the Dallas County Juvenile Department to renew the nonresidential contracts for the continuation of the listed programs for FY 2011 with continuation of the previously developed Corrective Action Plans; the effective date for reimbursement for services rendered under these contracts is retro-active to coincide with the FY 2011 contract year which began October 1, 2010.

33. COURT ORDER:

Approving the request of the Dallas County Juvenile Department and Youth Services Advisory Board to renew the non-residential services contract with Phoenix House of Texas, Inc., and the allocation of $156,589 in juror funds for continuation of the Empower Dallas Program for FY 2011; and approving the allocation of $5,000 in juror funds to be expended on GED preparation and testing for youth served by Dallas County Juvenile Department.

34. COURT ORDER:

Authorizing and recognizing the outlined core list as associated with (RFP No. 2007 -11 8-3034 - Request for Proposals for Cleaning Chemicals and Supplies) as awarded to Acuity Specialty Products, Inc. dba Zep Manufacturing Company, for the period of January 1, 2011 through December 31, 2011, and authorizing all County documents/payments to reflect accordingly.

Authorizing the extension of (RFP No. 2008-016-3250 - Annual Contract for Psychological Screening and Counseling Services for Law Enforcement Personnel) as awarded to John A. Lehman & Associates, for the period of February 1! 2011 through January 31 f 2012, and authorizing all County documents/payments to reflect accordingly.

36. COURT ORDER:

Authorizing the extension of (Bid No. 2009-009-3963 - Annual Contract for Purchase of Laboratory Supplies) as awarded to ChromTech, Inc. Scientific, Inc., for the period of December 1 r 2010 through November ! 2011/ and authorizing all County documents/payments to reflect accordingly.

37. COURT ORDER:

Authorizing (Bid No. 2009-061-4311 - Annual Contract for the Purchase of Paint and Related Products) as awarded to ICI Paints for Groups 3,6,11,15,16, 22 and 24 to reflect the company's name as Akzo Nobel Paints LLC - DBA Glidden Professional Paint Center, and authorizing all County documents/payments to reflect accordingly, effective August 26, 2010.

38. COURT ORDER:

Authorizing the first twelve (1 2) month extension of (Bid No. 2010-003-4661 -Annual Contract for Newspaper Advertising) as awarded to Daily Commercial Record, for the period of November 1 r 2010 through October 31 f 2011, and authorizing all County documents/payments to reflect accordingly.

39. COURT ORDER:

Authorizing the cancellation of (Bid No. 2010-050-5034 - Annual Contract for the Purchase of Custom Heavy Duty Storage Boxes) with Berndt Business Forms and the disqualification of said company from participating in future Dallas County solicitations for a period of thirteen (1 3) months (October 1 2, 2010 through November 11, 2011), and authorizing all County documents/payments to reflect accordingly.

Rendering Buchanan Technologies, DFW Technology, Komia and Unisys as susceptible firms to proceed to Step Two of the approved High Technology procedures established in RFQ No. 2010-070-5138 - Request for Qualifications for Level 1 Heip Desk Service Provider.

Approving a project in District No. 1 to provide partial funding for constructing the Kelly Boulevard north of Country Club DART RR Crossing Project, located within the City of Carrollton, and authorizing the County Judge to sign the Approval of Interlocal Cooperation Project; and approving an Interlocal Agreement with the City of Carrollton for said project for a total participation cost of $ 37,500.00 I and authorizing the County Judge to sign said agreement for this Type liB" road work.

42. COURT ORDER:

Approving a project in District No. 2 to participate in 50% of the funding for the hot-in-place asphalt recycling of Pleasant Valley Road from Merritt Road to Elm Grove Road, and Elm Grove Road from Pleasant Valley Road to Hinton Landfill, authorizing the County Judge to sign the Approval of Interlocal Cooperation Project; and approving an Interlocal Agreement with the City of Garland for said project for a total estimated project cost of $349,200.00, and authorizing the County Judge to sign said agreement for this Type JIB" road work.

43. COURT ORDER:

Approving a project in District No.2 to participate by reimbursing City in a not-toexceed amount of $200,000 for removing and replacing concrete pavenlent on Town East Boulevard from Motley Drive to LBJ (lH635), authorizing the County Judge to sign the Approval of Interlocal Cooperation Project; and approving an Interlocal Agreement with the City of Mesquite for said project for a total estimated project cost of $1,964,142.60, and authorizing the County Judge to sign said agreement for this Type "B" road work.

Authorizing and directing the County Judge to execute Work Order No. 7 for Laboratory Services (geotechnical work) with Rone Engineering Services, Ltd., via a current Indefinite Delivery Indefinite Ouantity (lDIO) Contact, in an amount notto-exceed $2,000.00, to be paid from the CDBG FY 2010 Cockrell Hill Water Main Improvements, Phase VI Project, Program Funds in Oracle No. 09512 Year 2011 on behalf of Dallas County.
