Prior to drafting any contract for goods and services, the responsible party or the decedent’s survivor who is handling the funeral arrangements is entitled to receive a copy of any pre-need arrangement in the possession of the funeral establishment that has been signed and paid for, in full or part, by or on behalf of the decedent.

PROFESSIONAL SERVICES

1. Basic Services of Funeral Director/sta� and Overhead $720.50

This fee includes: conducting arrangements conference, planning services, consulting with family and clergy, shelter of remains, preparation and filing of necessary authorizations and permits, recording vital statistics, coordination with the crematory, cemetery, or other third parties. Also included in this charge are overhead expenses relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative costs, equipment and inventory cost.This fee for our Basic Services and overhead will be added to the total of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.)

2. Cremation (the process) $188

3. Transportation $224.50This includes removal of the deceased from Hospital/Nursing Facility within 30 mile radius, (additional mileage is $3/mile). (Additional charges for residential, after hours and holiday removals.)

4. Forwarding of Remains $2,395This includes basic services of funeral director and sta�, overhead costs, preparation of remains (including embalming), and utility vehicle to local airport 30 mile radius (additional mileage is $3/mile.). (Additional charges for residential, after hours and holiday removals.)

5. Receiving of Remains $1,420This includes basic services of funeral director and sta�, overhead costs, shelter of remains, and utility vehicle to local cemetery (30 mile radius; additional mileage is $3/mile) or transport to our crematory. (Additional charges for after hours and holiday removals may apply.)

Embalming $800 Embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangment that does not require you to pay for it, such as simple cremation or immediate burial.

SIMPLE CREMATION (STARTING AT)$639.50Our charge for a simple cremation (without ceremony) includes: basic services of funeral director and sta�; a proportionate share of overhead costs, minimum corrugated box, transfer of remains to funeral home, filing of necessary authorizations and permits. Additional charges for residential, after hours and holiday removals.

IMMEDIATE BURIAL (STARTING AT)

$1,295This option includes: basic professional services of the funeral director and staff, death certificate and permit processing, overhead costs, local transport of the deceased, local transfer of remains to the funeral home (one attendant included), and utility vehicle for

transfer to local cemetery.

Additional charges for residential, after hours and holiday removals.

Merchandise, cash advances and additional services are not included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.

GENERAL PRICE LIST

These prices are effective as of February 3, 2020 but are

subject to change without notice. The goods and services shown in this brochure are those we can provide to our clientele. You may choose only those items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead.If legal or other requirements mean you must buy any items you did not specifically ask for within this contract, we will explain in writing, on the statement we provide, describing the cremation/ funeral goods and services you selected at time of need. If you want to arrange a direct cremation, you can use an alternative container. Alterna-tive containers encase the body and can be made of materials like fiberboard or composition materials with or without an outside covering (the containers we provide are fiberboard containers.)

To continue to o�er our low cost cremation services, we require payment in full at the time of arrangements.
