The adoptive family profile is a snapshot into your lives as a couple, the life you will provide to your newly adopted child and an opportunity for you to reach out to potential birth parents and tell your story.
Before completing your profile, please read Chapter 5 (Completing the Adoptive Family Profile/Exposure to Potential Birth Parents) of your Preparing for a Successful Adoption manual in its entirety.

Please complete the form on back and submit it with your profile items.

Photo Checklist
q q 8-10 Holiday, vacation, fun photos
q q 8-10 Photos of you doing activities/hobbies
q 2-5 Photos of your home (be sure to include outside and inside photos)

q 2-5 Photos of a neighborhood park, pool, elementary school or other community space
q 4-10 Hobby photos without people in them
q 2-4 Photos of extended family and/or friends
q Additional miscellaneous photos
CD or flash drive in jpeg (.jpg) or tiff (.tif) format is required.Captions should be included as file names or as a list in Microsoft Word (.doc) or simple text format (.txt).
A printed copy of your captions may also be submitted. We do not accept photos via email.
Actual photos may be submitted for scanning for an additional fee of $25/every three photos. CD or flash drive will not be returned to you unless requested.
