AFFIDAVIT

WE, RIZA LAGMAY and JAYSON ALCANTARA, Filipinos, of legal ages, and residents of San Fermin, Cauayan City, Isabela, having been duly sworn in accordance with law, hereby depose and state:

That we are the biological parents of a baby girl whom we named Princes Angel, born on July 04, 2014 at Cauayan City, Isabela;

That we are no longer capable of providing the basic needs of the said child because we are financially depressed;

That because of the above, we are relinquishing custody and care over the said child in favor of the Spouses Reynaldo Tejo and Elvie Tejo, residents of Bunnay, Angadanan, Isabela, and whom we know to be better capable of providing for the needs of our child Princes Angel;

As such, we are executing this Affidavit to attest to the truth of the foregoing and for other legal purposes it may serve.
