MINUTES OF ANNUAL GENERAL BODY MEETING OF IMA KSSS FOR THE YEAR 2016-17 HELD ON

27.10.2017 AT 3.00 PM AT BLOCK-D, GADAG INSTITUTE OF MEDICAL SCIENCES, BENGALURU

ROAD, MALLASAMUDRA,GADAG-582101.

Agenda-1:Welcome by the Chairman and IMA Prayer Meeting was called for order by the Chairman Dr Yogananda Reddy Y.C.

The Chairman welcomed all the esteemed members of IMA-KSSS and President IMA-KSB Dr.

Rajashekar S Bellary, President Elect IMA-KSB Dr.H.N. Raveendra and Hon. Secretary IMA-KSB DR. B

Veeranna. Chairman also welcomed the past Presidents of IMA-KSB, past Chairmen of IMA-KSSS

who were present in the meeting. Chairman expressed his appreciation to all the managing

committee members of IMA-KSSS for their contribution towards growth and development of IMA-

KSSS and welcomed all the MC members to the meeting. He specially welcomed all the Office

bearers of IMA-KSSS.

Then the Chairman presented his report giving a total record of activities undertaken by him during

the year. The Chairman expressed satisfaction over membership drive in both the Schemes and

thanked whole-heartedly all the members who contributed to the success of the membership

drive.The Chairman said that two members of IMA-KSSS from Belgaum branch are not the life

members of IMA-KSSS despite repeated reminders and also said that same would be put up in next

MC meeting and would decide about their membership status.

CHAIRMAN’S DESK

Dear esteemed members of IMA-KSSS,

On behalf of the managing committee I extend my warm greetings from IMA-Karnataka Social Security Scheme ,

Hubballi to one and all . It is my honor to extend warm greetings from I MA-KSB also.

At the outset I on behalf of IMA-KSSS extend my heartfelt condolences to the bereaved families of all the demise

members of IMA-KSSS since the last general body meeting.

I welcome you all to Special General Body meeting and Annual General Body meeting of IMA-KSSS for the year 2016-

17.

I am privileged and Honored to share my experiences as Chairman of IMA-KSSS during the 1st year of my tenure. At

the outset I must say that it was a pleasant experience to work with the MC members and office staff of IMA-KSSS .

The constant support which managing committee extended to me helped me a lot in stream lining the activities of

IMA-KSSS. Due to constant effort of each of the MC members, IMA-KSSS is marching further gloriously. During the

year we had 4 MC meetings with meaningful deliberations and actionable decisions. The complete digitalization of

members data has helped us to minimize errors. The process of collection of DFC-6 and DFC -1 SJ sailed through

smoothly with members co-operation. The process of enrollment of new members to the both the scheme is

steadily progressive and motivational activities by MC members are exemplary.

This year we gave death claim benefit to nominees of 10 demise members in Old existing scheme amounting to

Rs.1,09,64,250/- and to nominees of single demise members in New Silver jubilee scheme amounting to

Rs.12,93,750/-.

The recent death claim benefit in Old existing scheme is Rs.1113000/- and in New Silver jubilee scheme is

Rs.1293750/-. We are proud to say that the nominees of late Dr.Prema V Patil , Jamkhandi who was member of both

the scheme received total of Rs.2391750/- .

This year managing committee took a decision to invest the funds of IMA-KSSS in mutual funds with

maximum portfolio investment in Govt. undertakings as per the Bye-law provisions and decision of the MC meeting .

The fund was invested in Govt. Securities through mutual fund under the guidance of official financial advisor of

IMA-KSSS and we expect better returns with tax indexation benefit.

With respect to fraudulent withdrawal of money which happened during previous year, the court cases in

High court of Karnataka ,Dharwad Bench, and State Consumer redressal forum are progressing positively.

The proposed amendments to bye-laws are attached herewith in the annual report for deliberations in Special

General Body meetings.

I extend my thanks to all the esteemed members of IMA-KSSS for electing me to serve and to extending their

faith and support during this period.

I must extend my sincere thanks to President IMA-KSB Dr. Rajashekhar S Bellary, President Elect-

Dr.Raveendra H.N., Immediate Past President DR.G.N.Prabhakara and members of State Working Committee and

State Council of IMA-KSB for their continuous support during the year.

I must thank my predecessor Chairman Dr.Nanjundaiah , Dr.K.C.Raju Reddy, Dr.A.S.Mantaganiker , Dr.P.V.Kanchi

and Dr.V.D.Sapare for their contribution to the growth of IMA-KSSS over the years.

I must extend my special thanks to the founder Chairman and Ex-Officio Dr.V.D.Sapare for his continuous efforts for

the growth of IMA-KSSS. At this juncture I cannot forget late Dr.B G Mudhol and Dr.Vyasa Uchila, who have

hugely contributed for enrollment of members during earlier days.

I extend my sincere thanks to 1st vice –Chairman Dr.N.Veeranna and 2nd vice-Chairman Dr.N.N.Mahajan for their

support and guidance.

I appreciate the participation of former Chairmen Dr.C.K.Raju Reddy , Dr.A.S.Mantganiker and Dr.P.V.Kanchi during

the deliberations of MC meetings.

I extend my profound thanks to dynamic Secretary Dr.Madhusudhan K. , watchful treasurer Dr. Mahesh Kumar. S.

and ever active joint Secretaries Dr.V.B.Nitali and Dr.S.B.Lakkol.

I extend my sincere thanks to Dr. Sharanappa , senior vice-President IMA-KSB and Dr.Shivakumar Kumbar, vice-

President IMA-KSB for their effective participation, membership drive, promotional activities and legal help.

It is my duty to extend sincere thanks to Dr.S.K.Nara and Dr.K.T. Prakash Reddy for the promotional activities and

administrative guidance. My special thanks to Dr.K.T.Prakash Reddy, Dr.Jayanth M.S., Dr.Yuvaraj Patuvardhan,

Dr.Narayana Murthy and Dr.Sharanappa for motivating maximum number of new membership.

I extend my sincere thanks to Dr.G B Bidinahal , Dr.G.N.Prabhakar , Dr.N.S.Hiramath, Dr.Jayappa , Dr.Azhar Kittur,

Dr.Suresh Rudrappa, Dr.Sachin Revankar for their guidance and effective inputs during MC meetings.

I extend my sincere thanks to the office staff Miss.Swathi, Mr.Basavaraj Nyamathi and Smt.Geetha Desalli for their

SECRETARY’S ANNUAL REPORT-2016-17

Dear esteemed members of IMA-KSSS a warm regards and greetings from IMA’s Karnataka Social Security

Scheme, Hubbali to one and all.

I am honoured and privileged to present the Secretary’s Report for the year 2016-17. I congratulate all

the new office bearers of IMA’s KSSS for 2016-2019. The new team of enthusiastic Managing Committee

members is very much encouraging to the growth and financial stability of our scheme.

The enrollment of new membership to old existing Scheme and New Silver Jubilee Scheme is as follows:

The DFC-6 and DFC-1 –SJ was collected from the month of Aril to May 2017 for maximum of 10 Deaths

accrued after the previous DFC -5 collections of old existing scheme and one death in new Silver jubilee

scheme (Details enclosed).

The defaulter list in DFC -5 collections has come to all time low of five members and only those five

members have been terminated from IMA-KSSS membership.

Only one member from old existing scheme has taken retirement under voluntary retirement scheme of

IMA-KSSS. We have refunded Rs.14,443/-(Rs fourteen thousand and four hundred forty three only) to the

concerned member as per the bye-law of IMA-KSSS.

We were able to successfully revive 11 members from the terminated list during this association year. This

year 235 senior members of IMa-KSSS are completing 25 years of their of membership.

IMA-KSSS has dishonored one death claim due to non availability of IMA-KSB membership of the

concerned member. The decision of rejection of claim was taken after seeking the legal opinion by

managing committee of IMA-KSSS.

As there were few member in IMA-KSSS without life membership of IMA-KSB, IMA-KSSS successfully

motivated many of them to become life members of IMA-KSB during the recent membership drive of IMA-

KSB (July-2017). However as ten members did not become life members on their own, IMA-KSSS fecilitated

their life membership of IMA-KSB with member’s consent by paying Rs.66,500/-( Rs sixty six thousand and

five hundred only) from IMA-KSSS accounts. This was done as per the decision of the MC meeting to avoid

further legal issues and IMA-KSSS would be deducting this amount without interest in due course from

their final claims.

The IMA-KSSS has disbursed death claim to nominees of ten demise members in Old existing scheme and

one demise member in New silver jubilee scheme since the previous annual general body meeting. The

total amount disbursed to the bereaved family is Rs.1,09,64,250/- ( Rs one crore nine lakhs and sixty four

thousand and two fifty only) in old existing scheme and Rs.12,93,750/- (Rs twelve lakhs and ninty three

thousand and seven fifty only) in new silver jubilee scheme ( list attached).

The recent death claim benefit to single member in Old existing scheme is Rs.11,13,000/- ( Rs eleven lakhs

and thirteen thousand only) and in New Silver jubilee scheme is Rs.12,93,750/-( Rs twelve lakhs and ninty

three thousand and seven fifty only).

The most significant development is the Death benefit given to the nominees of late Dr.Prema

V.Patil of Jamkhandi of Rs.23,91,750/- ( Rs twenty three laksh and ninty one thousand seven fifty only)

from both the schemes.

IMA-KSSS has disbursed pending amount of Rs.3,60,250/- ( Rs three lakh sixty thousand and two hundred

and fifty only) to one of the nominees of late Dr.B K Kurbet of Sankeshwar branch.

We successfully renewed accidental death benefit premium of Rs.3,89,871/- ( Rs three lakh eighty nine

thousand and eight hundred and seventy one only) with Royal Sundaram General insurance company ,

Bengaluru.

Recently the IMA-KSSS has invested Rs . 1 ,92,30,000/- ( Rs one crore ninty two laksh and thirty thousand

only) from old existing scheme and Rs.1,07,46,000/- (Rs one crore seven laksh forty six thousand only )

from new silver jubilee scheme in Mutual funds with maximum portfolio investment in Government

undertakings under the advise of Mr.Saji K S ,financial advisor of IMA-KSSS. The details of investment is

attached. With this we expect better long term returns and tax indexation benefit as compared to fixed

deposits in banks.

Our office was shifted to the New well furnished and digitalized premises in the same Building of IMA

Hubballi. All the data and details of the members of IMA’s KSSS has been updated in our dynamic website:

The Audited accounts along with the Balance Sheet and the Budget proposal are submitted herewith. We

have uploaded the minutes of previous AGM and all the MC meetings on our website ( imakssshubli.org).

The details of progress of court cases and investigations in the fraudulent withdrawals of money from IMA-

KSSS accounts has been separately attached.

I extend my sincere thanks to all the esteemed members of IMA-KSSS who stood firmly with the scheme

and extended valuable advises and constructive criticism during the difficult days of IMA-KSSS.

Agenda-9: Presentation and adoption of the audited accounts for the year 01-

04-2016 to 31-03-2017 Dr Mahesh Kumar S, the Treasurer presented the audited report including Balance Sheet of IMA

KSSS for the year 2016-17.

After detailed discussions, the audited report, the balance sheet for the year 2015-16 was approved by the

Annual General Body. Proposed by Dr.H. Tosaniwal and seconded by Dr Prakash Reddy .

Agenda-10: Appointment of auditor and fixing the remuneration:

It was unanimously resolved to continue with the same auditors M/s. Adarsh seth & Associates.

It was also resolved to fix the remuneration after discussion in the MC meeting.

Agenda-11: Budget for the year 2017-18:

The Treasurer put up the Budget proposals for the year 2017-18 as per the details mentioned in

Annual Report for 2016-17 before the General Body. After discussions, this budget was approved by the

General Body unanimously.

STATUS REPORT OF FRAUDULENT WITHDRAWAL OF MONEY

FROM IMA-KSSS BANK ACCOUNTS SINCE LAST AGM

As you all know the unfortunate incidence of fraudulent withdrawal of money from KSSS accounts to the

tune of Rs. 47,16,515/- (Rs forty seven lacks sixteen thousand and five hundred and fifteen only) during

the previous financial year. ,we would like to update the members regarding various court cases and status

of complaint lodged with Sub urban police station, Hubballi.

1) As police investigations were getting delayed despite best efforts of IMA KSSS we filed a writ

petition in High Court of Karnataka Dharwad Bench seeking directions from court to expedite the

investigation process and to file a charge sheet at the earliest. Accordingly charge sheet has been

filed and forged documents are sent for hand writing verification to Govt Forensic Science

Laboratory and the results are awaited time bound by march 2018.[ Advocate report (B) below].

2) The IMA KSSS has submitted all the required documents to investigation officer in support of its

claim.

3) We have lodged complaints with state consumer redressal forum, Bengaluru against Canara bank ,

B N Nagara branch , Hubballi and Karnataka bank limited , NCM and Durgadbail branch , Hubballi

and status of both complaints is given below in advocates report (A).

4) We have lodged additional complaint to police making bank officials as accused.

5) The reports of forgery verification from Govt approved private forensic lab (Truth lab , Bengaluru

and JK Consultancy , New Delhi) have come in favour of IMA-KSSS and forged signatures are not

matching with specimen signatures.

6) We have filed applications under RTI to both the banks seeking various documents to gather

documentary evidences. We have also collected video clippings of Bank withdrawals and submitted

to investigation officer.

7) We have hired Mr Hanumareddy , High court advocate and Mr Yogesh Kumar and associates

advocates, Bengaluru as our legal counsels. Till today we have paid advocate fee of Rs.1,05,000/-

to Mr.Yogesh Kumar & Associates and Rs.40,000/- to Mr.Hanumareddy

directed the HCGP to file the charge sheet and granted time up to 15/02/2017. I state

that again the matter listed on 15/02/2017 and on that day Hon’ble Court warned the IO

to file the charge sheet as expeditious as possible. Thereafter the matter came to be listed

on 08/03/2017 and on that day also court has warned the IO and HCGP to file the charge

sheet. I state that now I told by HCGP that the charge sheet ids filed against the accused

Bo.1 and 2 and showing the accused No.2 as absconding and same has to be confirmed

after obtaining the certified copy of the charge sheet.
