1. UTANGULIZI Asasi ya Adventures in Health, Education and Agricultural Development-Tanzania

(AHEAD-Tanzania) ilianzishwa hapanchini na kusajiliwa na Registrar of Societies,

Wizara ya Mambo ya Ndani ya Nchi tarehe 29 Juni, 1989 chini ya sheria Societies

Ordinance, 1954. Cheti chake cha Usajili ni SO. No. 7055. (Tazama Kiambatisho

‘A’). Aidha tarehe 12 Mei, 2008, AHEAD-Tanzania ilipata Certificate of Compliance

chini ya Sheria ya Mashirika yasiyo ya Kiserikali, Na. 24 ya mwaka 2002. Namba ya

Cheti (Certificate of Compliance au Cheti cha Ukubalifu) hicho ni 1996. (Tazama

Kiambatisho ‘B’). Kama jina la Asasi linavyojieleza, AHEAD-Tanzania imejikita katika

kuhamasisha na kushirikiana na wananchi walengwa na wanufaika katika kuibua na

kutekeleza miradi katika Nyanja za Afya, Elimu na maendeleo endelevu ya Kilimo.

Katika maeneo hayo, wanananchi Watanzania wanaweza kukabili maadui wa

maendeleo, yaani maradhi, ujinga na umasikini.

Tangu kuanzishwa kwake AHEAD-Tanzania imetekeleza miradi mbalimbali hapa

nchini hususan katika mikoa ya Pwani, Shinyanga na Kagera. Mwaka 1995, AHEAD-

Tanzania ilitekeleza kwa ufanisi programu za Afya zilizolenga kuboresha afya ya

mtoto, huduma kawa akina mama, na kupeleka umeme wa jua. Katika Mkoa wa

Pwani, mwaka 2001 AHEAD-Tanzania ilielekeza jitihada zake katika elimu ya ufundi

katika wilaya ya Kisarawe. Mwaka 2016, AHEAD-Tanzania ilielekeza jitihada zake za

kushirikiana na wananchi kujiletea maendeleo yao katika mkoa wa Kagera,

Halmashauri ya Wilaya Bukoba. Maeneo yaliyolengwa ni uboreshaji wa mazingira ya

kufundishia na kujifunza pamoja na huduma za afya. AHEAD-Tanzania kwa msaada

kutoka Asasi dada ya Marekani, yaani AHEAD, Inc. (USA), iliweka umeme katika

majengo yote ya shule ya Sekondari Bukara. Kuanzia Mwezi Agosti, 2017 AHEAD-

Tanzania ilipanua shughuli zake kwa: (i) kujenga choo ya kisasa kwa ajili ya watoto

wa kike; (ii) kujenga miundombinu ya kuvuna maji ya mvua na (iii) kujenga Tanki la

kuhifadhi maji kiasi cha lita za ujazo 65,000. Kuanzia mwezi Oktoba 2017, AHEAD-

Tanzania ilielekeza nguvu zake katika kutekeleza programu au mradi wa ujenzi wa

Wodi ya Watoto na akina mama (Martenal and Pediatric Ward) kwa kushirikiana na

wanajamii ya Kata za Maruku na Kanyangereko katika Halmashauri ya Wilaya

Bukoba.

AHEAD-Tanzania inaihakikishia Serikali ya Jamhuri ya Muungano kupitia Wizara ya

Afya, Maendeleo ya Jamii, Jinsia, Wazee na Watoto kwamba katika utekelezaji wa

shughuli zake itaendelea kuzingatia sheria, kanuni, maelekezo na maagizo ya Serikali.

Aidha, AHEAD-Tanzania itaendelea kubuni na kutekeleza miradi inayozingatia

vipaumbele, mipango na mikakati ya Serikali katika ngazi za wilaya, mikoa na taifa ili

kutoa mchango katika kufikia maendeleo endelevu kwa manufaa ya jamii na taifa.

Lengo la andiko hili ni kutoa taarifa zinazohusu AHEAD-Tanzania kwa mujibu wa agizo

la Serikali kupitia Wizara ya Afya, Maendeleo ya Jamii, Jinsia, Wazee na Watoto

lililotolewa tarehe 28 Septemba, 2018. Taarifa hii imegawanyika katika sehemu kuu

nne. Sehemu ya kwanza ni Taarifa za za fedha zilizokaguliwa (audited financial

statements) za miaka miwili iliyopita, 2016 na 2017 maandalizi ya kutoa taarifa ya

mwaka 2018 yanaendelea. Sehemu ya pili ni taarifa ya vyanzo vya fedha, matumizi

yake na miradi iliyotekelezwa na inayotarajia kutekelezwa. Tatu, ni hati ya

makubaliano (Memorundum of Understanding) kwa kipindi cha mwaka 2017 hadi

The Annual Report and Audited Financial Statements of AHEAD -Tanzania for the year ended 31

December 2016 is presented by the Advisory Committee in accordance with laid down procedures.


1. PREAMBLE

The Advisory Committee Members submit their report together with the Audited financial statements

for the year ended 31 December 2016 which discloses the state of affairs of AHEAD -Tanzania.

2. INCORPORATION AHEAD-Tanzania is a Non-Profit, Non-government organization whose purpose is to work in

underserved communities in Tanzania to improve the quality of life for residents by implementing

programs that lead to self-sufficiency and self-reliance. Using a participatory, problem–solving

approach to development, AHEAD-Tanzania works with beneficiaries to develop self-help programs.

Situated in Kagera Region, Bukoba District – Tanzania. Postal address: P.O. Box 1649 Bukoba, Tanzania.

Registration:

(i) Registered with United Republic of Tanzania Registrar of Societies, Ministry of Home Affairs, 29 June, 1989, Reg. So. No. 7055

(ii) Complied with the terms and conditions of NGOs under the Non-Governmental Organizations Act, 2002 on 12th May 2008.Certificate of compliance No.1996.

3. THE GOAL OF AHEAD – TANZANIA ARE AS FOLLOWS:- (i) To reduce maternal and childhood mortality (ii) To improve the quality of life for girls and women. (iii) To prevent the spread of HIV/AIDS and others STIs (iv) To support education and intervention campaigns aimed at changing harmful

attitudes and practices (v) To improve the support of women in their roles as care –providers for their families (vi) To help reduce socio-economic inequalities in families and communities that impinge

upon good health. (vii) To solicit funds to maintain and expand the operation and communities that impinge.

Governmental Organizations Act, 2002 and the entity’s constitution to prepare financial

statements for each financial year, that present a true and fair view of the state of affairs of

the AHEAD Tanzania as at the end of the financial year.

The preparation of these Financial Statements was based on appropriate accounting policies

which have been used and applied consistently. Reasonable, prudent judgment and estimates

have been made in the preparation of these financial statements for the year ended

31 December 2016.

The annual financial statements for the year were prepared on the basis of Cash received and

expenses paid.

Adequate accounting records have been kept which at any time disclose with reasonable

accuracy the financial position of the Organization.

7. GOVERNANCE

The Constitution of AHEAD-Tanzania provides a framework for ensuring application of good

governance principles and best practices by the Advisory Committee Members and the

Management in the course of managing the day to day affairs of the Organization.

8.0 FINANCIAL PERFORMANCE OF THE COMPANY

The Financial Statements of AHEAD-Tanzania for the year ended 31 December, 2016 have

been prepared in accordance with Cash received and expensed paid. The Financial

Statements are enclosed from page 6 as part of this report.

8.1 Financial Position

During the year under review, the Organization’s financial position was satisfactory.

8.2 Operational Results

The operational results of the Organization shows the total revenue collected during the year

amounted to TZS 10,300,698

The expenditure on the other hand, amounted to TZS. 2,091,900 Therefore, the Organization

realized a surplus amounting to TZS 8,208,798 during the year ended 31 December, 2016.

9.0 STATUTORY AUDITORS

The Advisory Committee Members appointed SYMMETRIC AUDITING AND MANAGEMENT

CONSULTANCY as statutory/external auditors of the Organization for the financial years 2016,

2017 and 2018.

The Advisory Committee Members are required by the NGOs under the Non-Governmental

Organizations Act, 2002 to prepare financial statements for each financial year that give a true and

fair view of the state of affairs of the Organization as at the financial year end and of its surplus or

deficit.

The Advisory Committee Members confirm that suitable accounting policies have been used and

applied consistently and reasonable and prudent judgment and estimates have been made in the

preparation of the financial statements for the year ended 31 December 2016.

The Advisory Committee is responsible for keeping proper accounting records which disclose with

reasonable accuracy at any time the financial position of the company and which enable assurance

that the financial statements comply with the Non-Governmental Organizations Act, 2002. The

Advisory Committee Members are also responsible for exercising and maintaining an internal control

system for safeguarding the assets of the Organization, preventing and detecting fraud, errors and

other irregularities.

The expenditure on the other hand, amounted to TZS. 47,027,360 therefore the Organization

realized a surplus amounting to TZS 34,337,128 during the year ended 31 December, 2017.

2. VYANZO VYA FEDHA MATUMIZI NA MIRADI ILIYOTEKELEZWA

Vyanzo vya fedha kwa ajili ya utekelezaji wa miradi ya AHEAD-Tanzania ni viwili tu.

Kwanza ni Michango kutoka wananchi, hususan wanajamii ambao ni wanufaika wa

miradi. Wananchi wanachangia fedha taslim, vifaa na wakati mwingine nguvukazi kwa

kufanya kazi za mradi kwa kujitolea. Pili, ni Asasi dada ya AHEAD-Tanzania, inayoitwa

AHEAD, Inc. ya Marekani (http://www.aheadinc.org/). Na hivi Karibuni Asasi inayoitwa

1% for Development Fund (http://www.one-percent-fund.net) ya Roma-Italia imesaidia

AHEAD-Tanzania katika kugharamia ununuzi wa madirisha ya mradi wa ujenzi wa Wodi

ya akina mama na watoto katika zahanati Maruku, ikiwa ni pamoja na ujenzi wa mfumo

wa maji taka wa jengo la wodi hiyo.

Kuanzia mwaka 2016 hadi 2018, AHEAD Tanzania imetekeleza miradi miwili ambayo ni:

(i) Maboresho ya Miundombinu ya kufundisha na kufundishia Shule ya Sekondari Bukara;

na (ii) Ujenzi wa Wodi ya Mama na Watoto (Maternal and Pediatric Ward).

Miundombinu ya Kufundishia na Kujifunzia-Shule ya Sekondari Bukara

Mradi huu ulihusishashughuli nne zinazoshabihiana: (i) Kuweka umeme katika majengo

yote ya shule; (ii) kujenga choo ya kisasa kwa ajili ya watoto wa kike; (iii) kujenga

miundombinu ya kuvuna maji ya mvua; na (iv) kujenga Tanki la kuhifadhi maji ya mvua

kiasi cha lita za ujazo 65,000.

Miradi yote hii ilikamilika Mwaka 2017

Ujenzi wa Wodi ya Mama na Watoto (Maternal and Pediatric Ward)

Ujenzi wa wodi ya mama na watoto unaendela katika zahanati ya Maruku inayomilikiwa

na Halmashauri ya Wilaya ya Bukoba. Ujenzi huu unasimamiwa na AHEAD-Tanzania.

Aidha wanaochangia ujenzi ni wananchi wenyewe wa Kata za Marku na Kanyangereko

katika Wilaya ya Bukoba. Wananchi wanachangia fedha taslim, vifaa na nguvukazi kwa

kujitolea. Shirika dada AHEAD, Inc. (Marekani), linaendelea kuunga mkono juhudi za

wananchi kwa kuchangia fedha taslim. Na hivi karibuni shirika lisilo la Kiserikali linaloitwa

“1% for Development Fund (http://www.one-percent-fund.net/) limetoa msaada wa fedha

kuwezesha ununuzi wa madirisha ya Aluminium na ujenzi wa miundombinu ya maji taka

kwa ajili ya jengo la wodi ya Mama na watoto inayoendelea kujengwa katika zahanati

Maruku, ambayo kwa sasa inapanuliwa na Serikali ili iweze kuwa Kituo cha Afya.

Pamoja na wanananchi kuchangia fedha, vifaa na nguvukazi, wadau mbalimbali

wameshiriki kuchangia Ujenzi wa Wodi, ikiwa ni pamoja na Mfuko wa Maendeleo ya

Jimbo chini ya uongozi wa Mheshimiwa Mbunge wa Jimbo Bukoba Vijijini- Milioni kumi

(Tsh 10,000,000); Wakala wa Misitu Tanzania – Million Tano (Tsh 5,000,000) na Kituo

cha Utafiti wa Kilimo Maruku- Mbao 390 zenye thamani ya Tsh 3, 647,000; Serikali za

Vijiji vitatu vya kata ya Maruku- Tsh 4,020,000. Aidha shule na kanisa la kilutheri Tanzania

(Usharika Maruku) wamechangia mradi mbao za kuwezeka jengo la wodi.

MEMORANDUM OF UNDERSTANDING (MoU) This MEMORANDUM OF UNDERSTANDING (Here in after referred to as the agreement) is made this First day of October, 2017

BETWEEN

Adventures in Health, Education, and Agricultural Development hereinafter referred to as AHEAD, Inc.-USA of P. O Box 2049 Rockville, MD 20847 registered with US government on 14th June, 1986 and registered with US Department of the Treasury,

Internal Revenue Services (IRS) on 19th March 1993 as a 501(c) (3) qualified non-profit, tax-exempt and non-governmental organization based in Rockville, Maryland

with affiliates in Tanzania, The Gambia and Bermuda. AHEAD Inc., USA is an independent and a non-partisan organization whose mission is to work with under-served communities in developing countries to improve the quality of life for orphans

and other vulnerable children, women and youth by implementing sustainable programs that lead to self-sufficiency and self-reliance.

AND

Adventures in Health, Education, and Agricultural Development, Tanzania hereinafter

referred to as AHEAD-Tanzania of P. O. Box 1649, Bukoba, Tanzania, a non-governmental Organization which is non-partisan and not-for profit entity registered

with the United Republic of Tanzania’s Registrar of Societies, Ministry of Home Affairs under the Societies Ordinance, 1954 on 29th June, 1989 (Registration Number 7055) and certified on 12th May, 2008 as a duly compliant of the terms and conditions of

NGOs under the Non-Governmental Organizations Act No. 24 of 2002 (Certificate No. 1996), mandating AHEAD-Tanzania to carry out its duties and responsibilities on

Tanzania mainland. AHEAD-Tanzania works to reduce maternal and childhood mortality, improve the quality of life for girls and women, enhance access to quality education and healthcare services, promote agricultural development and solicit

funds from internal and external sources in order to maintain and expand AHEAD programs and projects.

WHEREAS, AHEAD Inc.-USA has been successful raising funds to support sustainable programs in Tanzania during the past 29 years, including programs addressing

Primary Health Care, HIV/AIDS, education, youth development and Environmental Health.

WHEREAS, AHEAD-Tanzania has overtime has significantly benefitted from the

support of AHEAD, Inc. –USA, making it possible to enhance the quality of life to many Tanzanians, including women (mothers), infants and youth in Shinyanga Rural and Meatu Districts as well as Kisarawe District, Coast Region.

WHEREAS with effect from 2016 AHEAD, Inc.-USA has supported and continues to

provide financial and volunteers’ support to AHEAD-Tanzania to improve the teaching and learning infrastructure and environment at Bukara Secondary School in Kanyangereko ward, Bukoba district, including the construction of girls’ toilets and

rain water harvesting and storage systems as well as electrification of the school

buildings.

WHEREAS, AHEAD, INC.-USA has supported the improvement of dental care services at Kagera regional referral hospital and Maruku dispensary by providing hi-

tech equipment and technical expertise/advice to local staff. AND WHEREAS, with effect from October 2017 todate, AHEAD, Inc.-USA continues

and will continue to provide significant financial resources to AHEAD-Tanzania to support, inter alia, the efforts of the people of Maruku and Kanyangereko wards in

the construction of the maternal and pediatric ward at Maruku Dispensary (in the process of being elevated to health centre).

AND WHEREAS, AHEAD, Inc.-USA and AHEAD-Tanzania are committed to redouble their efforts in order to ensure that Maruku health care facility gets a furnished,

equipped and functioning maternal and pediatric ward; AND WHEREAS, both sister organizations, AHEAD-Inc.-USA and AHEAD-Tanzania

do commonly share the ideals embodied in their twin operational philosophies, respectively “Helping People Help Themselves” and “Development of the

People by the People for the People” and recognizing that cooperation between these like-minded organizations is necessary and it is a preconditions for arresting the three development enemies, namely disease, ignorance and poverty.

NOW THEREFORE, the parties to this memorandum in their free will, agree to jointly

work together and cooperate to enhance the quality of life in different Tanzania communities in the areas of health care, education and agricultural development programmes and projects under consideration of the promises and mutual covenants

set forth herein, thus the two sister organizations hereto agree as follows:-

ARTICLE I PURPOSE

The Memorandum of Understanding between AHEAD-Inc.-USA and AHEAD-Tanzania

is intended to establish and formalize the relationship between the two entities in order to maximize cooperation required for the smooth implementation of the

programs and projects that are under-taken jointly by the two parties to the agreement.

ARTICLE II OBJECTIVES, ROLES AND RESPONSIBILITIES

The Memorandum of understanding (MOU) is designed to indicate a desire to partner on various programs and projects to achieve various aims and objectives relating to

the goals of the parties to the agreement. The main specific objectives, roles and responsibilities of the two sister organizations are to: 2.1 Carry out joint activities as a partnership in reducing and eliminating disease

and premature death; 2.2 Cultivate and advance healthy living;

2.3 Forster sustainable environmental activity;

2.4 Work with under-served communities in developing countries to improve the quality of life for orphans and other vulnerable children, women and youth by

implementing sustainable programs that lead to self-sufficiency and self-reliance.

2.5 Improve the quality of life for mankind, particularly girls and women; 2.6 Eliminate killer diseases, particularly Malaria; 2.7 Prevent the spread of HIV/AIDS and other STIs

2.8 Promote access to quality education through improvement of the teaching and learning infrastructure;

2.9 Address agricultural development constraints and challenges; 2.10 Help reduce socio-economic inequalities in families and communities that

undermine people-centred development;

2.11 Support women in their roles as mothers and care-providers for their families; and

2.12 Mobilize financial and technical support through different fundraising initiatives;

ARTICLE III LIMITATIONS

3.1 Without prejudice to what is stated in this MOU, the parties to this MOU are free to engage in partnership for the purpose of fulfilling mutually agreed upon objectives;

3.2 Parties undertake to promote the objectives of this MOU in good faith and avoid conflict of interest;

3.3 Parties shall work harmoniously and transparently and share relevant information consonant with the spirit of cooperation;

3.4 Parties shall be free to solicit resources from local and international sources

for the purpose of implementing mutually agreed upon projects and programmes;

3.5 There is no restriction of partners from participating in agreements with other entities;

3.6 The partners’ participation levels and commitments will depend on the

abilities and resources of each party to the relationship;

ARTICLE IV TIME FRAME

4.1 This MoU shall come into force immediately after its adoption or execution by

the parties to the agreement;

ARTICLE V JOINT RESPONSIBILITIES OF PARTIES

5.1 All parties shall work together in harmony to the fulfillment of this MoU. 5.2 All parties shall cooperate and collaborate through mutual assistance in any

technical matter within the ambit of this MoU.

5.4 All parties shall be willing to exchange relevant information, reports and

documents which will facilitate the implementation of this MoU and subsequent programs and projects.

ARTICLE VI

TERMINATION OF THE MOU 6.1 Any party to this Memorandum of Understanding may terminate this MoU

by giving a notice to the other party of its intention to terminate the MoU

and giving out reasons for the termination. 6.2 Each party therefore upon such termination shall fulfill all their obligations

and responsibilities as stipulated into this MoU till the stipulated date of the intended termination.

ARTICLE VII DISPUTE RESOLUTION

7.2 In the event a dispute arises, the parties shall try to amicably resolve the matter, in the event this fails the management of each entity in the collaboration shall have final decision on resolution of the matter.

ARTICLE VIII

INTERPRETATION

8.1 If any dispute arises with regard to the interpretation on any matter in

this MoU the parties shall have the power to resolve it. WE parties, whose names and addresses are subscribed, are desirous of entering
