SUMMARY! DISCUSSIONIFINDINGS: On October 13, 2009 your Board approved the Ryan White Part A Fiscal Agent agreement amendment between Contra Costa County and Alameda County Public Health Department in the amount of $1 ,294,179 for the period March I, 2009 through February 28, 2010. The Public Health Department is requesting a second amendment to the contract. The total federal award has not changed from the first amendment to the second and continues to be for $5,833,009. The percent allocated to Contra Costa also has not changed and remains at 25.8%. The total amount identified to be allocated in amendment I was $1,294,179 and is now stated to be $1,302,010; a total amount difference of $7,831 - all for which can be accounted for in the increase to quality management, page 2, section III of Exhibit A Quality management (QM) activities are mandated by the funding source to ensure that the services provided are consistent with current HIV Standards. Alameda County, as the grantee, receives 5% of the total grant amount for its QM activities, of which $ 15,000 was initially given to Contra Costa County. For this year's allocation, $ 30,352 of the QM dollars was not spent, which represents QM savings. Contra Costa County is to receive 25.8% of this amount which is $ 7,831. Their budget in

FINANCING:

The funding for this contract is included in the FY 2009-10 adjusted budget. There is no impact on the net County cost.

Services are provided to Contra Costa residents in accordance with Ryan White HIV Treatment and

Modernization Act requirements for HIV services.

Budget and Service Priorities

I. Core Services

Early Intervention Services

To provide HIV early intervention services including HIV counseling and testing at high risk

clinics and/or PCRS to high risk residents of Contra Costa. $ 38.900

Case Management

To provide medical case management services, including: coordinating $ 772.522 access to medically appropriate levels of health care, treatment adherence,

partner counseling and referral, case conferencing, etc., to at least

500 people with HIV/AIDS in Contra Costa

Mental Health

To coordinate and provide mental health assessments, counseling and referral

follow up services to at least 50 people with HIV in Contra Costa. $ 54,298

Substance Abuse

To coordinate access to care and provide assessments, substance abuse

counseling and treatment for at least 100 people with HIV in Contra Costa. $ 134,717

Oral Health Care

To prOVide dental services to at least 15 eligible low income residents

Ambulatory Outpatient Primary Medicol Care

To prOVide ambulatory medical core to eligible undocumented and indigent

RECOMMENDATION(S):

Approve and authorize the Health Services Director, or his designee (Wendel Brunner, MD), to execute, on behalf of the County, Amendment Agreement #28-528-39 with the County of Alameda, effective March I, 2009, to amend Agreement #28-528-37 (as amended by Amendment Agreement #28-528-38), to increase the total payment limit by $7,831 from $1,294,179 to a new total of $1,302,010 and to make technical adjustments to the agreement, with no change in the original tenn of March 1,2009 through February 28, 2010.

FISCAL IMPACT:

This agreement will result in an increase in the amount of funding by $7,831 to a new payment limit of $1,302,010, from the County of Alameda, as the Grantee of federal funds under the Ryan White J-IIVIAIDS Treatment Modernization Act of 2006, Part A. No County match is required.

BACKGROUND:

The U.S. Department of Health and Human Services has designated the County of Alameda as "Grantee" for the purpose of administering the Ryan White HIV/AIDS Treatment Modernization Act of2006, Part A funds for coordination of essential services to COWlty Costa County residence with H1V disease and their families.
