A m a r r i e d woman, a g e d 3 2 . was a d m i t t e d t o a m e n t a lh o s p i t a l w i t h b i z a r r e symptoms o f t w o y e a r s ' d u r a t i o n .S h e c o m p l a i n e d o f a r e c u r r e n t m o r b i d urge t o s t r a n g l eh e r s e l f , o r a n y o n e e l s e wh o c h a n c e d t o b e a t hand d u r i n g

t h e a t t a c k s . I n a d d i t i o n , s h e was p e r i o d i c a l l y c o n s c i o u s

o fi l l u s o r y

d i s t o r t i o n s o f h e r b o d y . On t h e s e occasions h e r

h e a d w o u l d f e e l t h r e e t i m e s i t s n o r m a l s i z e , or h e r l e g sso s h o r t e n e d t h a t h e r f e e t s e e m e d t o be a t t a c h e d j u s t

b e l o w h e r k n e e s . W i t h o u t warning, s h e woul d b e over-

w h e l m e d b y f e e l i n g s o f u n r e a l i t y o f such i n t e n s i t y t h a t

s h e was c o m p e l l e d t o l o o k i n a m i r r o r t o c o n f i r m h e r

p r e s e n c e i n t h e room. S o m e t i m e s h e r l e f t arm a nd

r e a s t w o u l d s u d d e n l y l o s e t h e i r p e r s o n a l s i g n i f i c a n c e ;

t h e y no l o n g e r s e e m e d t o b e l o n g to h e r . No t i n f r e q u e n t l y

s h e n o t i c e d a n i l l u s o r y d i m i n u t i o n i n t h e s i z e o f o b j e c t sor persons, w h i c h a t t i m e s a s s u m e d L i l l i p u t i a n propor-

t i o n s ( m i c r o p s i a ) . S h e was a l s o t r o u b l e d b y r e c u r r e n t

s e n s a t i o n s t h a t s h e was a b o u t to d i e ; t h e s e a t t a c k s o f

a n g o r a n i m i , w h i c h l a s t e d a b o u t h a l f an hour, were

a c c o m p a n i e d b y an i l l u s o r y s l o w i n g i n t h e passage o f

time.

F o u r t e e n y e a r s b e f o r e t h e s e symptoms appeared, she

b e g a n t o s u f f e r f r o m t h r o b b i n g , l e f t - s i d e d h e a d a c h e sa c c o m p a n i e d b y p h o t o p h o b i a and p a r e s t h e s i a e of her

l e f t f a c e a n d arm. T h e s e a t t a c k s c e a s e d a f t e r two years

a n d d i d n o t r e t u r n u n t i l a year b e f o r e h e r a d m i s s i o n .

H o w e v e r , t h e were now r e i n f o r c e d b y a t t a c k s o f ver-

t i g o a n d t e i c h o p s i a . D e t a i l e d i n v e s t i g a t i o n s ( i n c l u d i n g

a n g i o g r a p h y a n d a i r e n c e p h a l o g r a p h y ) h a d b e e n p e r -f o r m e d a t a n o t h e r h o s p i t a l b e f o r e h e r a d m i s s i o n .
