From submenu under “Information for Students”, navigate to the link “Request Alt Media”.

Then, navigate to the button that states “Submit a Request”.

You will be prompted to log in through Shibboleth

After you log in, you will be redirected to the online form.

Your identifying information will automatically be filled out according to your login. Please review to make sure it is correct.

Please fill out the form to the best of your ability. You will have the choice of which type of request you are submitting. The form fields will adjust for every submission type.

If you wish to have a specific file format, please indicate in the “format requested” field. The default is always PDF. If you use Read&Write software given to you during your alternative media orientation, PDF or Word files (DOC) are both compatible.

If your course materials are anything other than a textbook, you will need to provide a copy to the alt media specialist. Please indicate how you will be delivering the material. You will have the option to attach files later in the request process. If for some reason you do not have a copy of your materials, please let us know in the comments section.

You may choose to put a delivery date. Please put the date you need the text by, not the date your reading assignments are due! Consider how much time it will take to read the material once it is given to you. **Note that submission of a delivery date does not guarantee you will receive the text by that date.

The “Comments” section is for you to put any extra information you think will be useful for the alternative media specialist. For example, if you require a specific chapter first, or if you are only planning to use certain parts of the textbook during your course.

When you are finished, please hit the submit button. At this point, you will be given the opportunity to attach any important documentation. This includes receipts, syllabi, documents (if you are submitting course materials electronically), etc.

When you are satisfied with your attachments, you may select “Continue”.

Review your order a final time. This is a DRAFT of your request and has not actually been submitted yet. This is your LAST CHANCE to edit any information before submitting your request. If you fail to select “submit” on this page, the alternative media specialist will not receive notification of your request.

And that’s it! You’re done!

View Request StatusTo view the status of your requests, enter the form in the same manner you would to submit a request.

When you arrive on the form page, navigate to the menu bar in the top, right-hand corner. With the “menu” option selected, a dropdown menu will give you additional options. Select “Requests”.

You will be taken to a window that will show all of your requests submitted to Disability Resource Services. Each request will have an order status accompanying it.

This allows you to keep track of your orders, as well as clarify with the alternative media specialist any questions, comments, or concerns you might have.
