• HHS has asked the National Quality Forum (NQF) to begin work on nationally endorsed quality measures for home and community-based services (HCBS), which impact people with ADRD and others who may need support to live at home.

Instructions for requesting CE

For best functioning, please use a browser other than Internet Explorer (e.g., Firefox, Chrome)• Go to http://www.cdc.gov/TCEOnline, and log in (you may need to register

as a new participant). • After successful log in, the “Participant Services” menu displays. Select

“Search and Register”. Select option 2, “Keyword Search”, and enter the course WC2463 and select “View”.

• Select the course “092215”to open the Course Description page. Scroll down to the box labeled “Register Here”, choose the appropriate credit type, and select “Submit”.

• The next page requests demographic information. Answer or update the demographic questions. Scroll down to the bottom of the page and click “Submit”.

• You will be asked to enter the Verification Code (ADWeb14) prior to completing the evaluation.

• The post-test is required and will follow the evaluation. 59
