• Must be duly registered with the Cooperative
Development Authority (CDA);
• Must be in Good Standing for the past three (3) preceding years;
• Must be existing for at least 5 years and must be operationally gainful for the last three (3) years– for Medium/Large Cooperative;
Eligibility and Qualifying Criteria
Must have a NETWORTH of: Micro Scale Category - Below 10 Million Small Scale Category - 10 Million to
Below 50 Million Medium Scale Category - 50 Million to
Below 100 Million
Large Scale Category - 100 Million and above

• Must be existing for at least three (3) years and operationally gainful for the last two (2) years -- for micro and small cooperative;
• Compliant with the legal and administrative
requirements of CDA and other government agencies; and
• Must be nominated by any one of the following: cooperative federation or union, government organizations, local government units (LGUs) and other non-government organizations.

Most Outstanding Cooperative Federation
• Must be duly registered federation with the
Cooperative Development Authority (CDA);
• Must be in Good Standing for the past three (3) preceding years;
• Must be existing for at least 5 years and must be operationally gainful for the last three (3) years
• Compliant with the legal and administrative requirements of CDA and other government agencies
Eligibility and Qualifying Criteria
• Must be nominated by any two of the following: cooperative affiliate, government organizations, local government units (LGUs) and other non-government organizations;
• In case of federation with national area of operation, nomination of at least 1 member affiliate from region where it has member is required.

CRITERIA FOR EVALUATION
• Must be duly registered union with the Cooperative
Development Authority (CDA);
• Must be in Good Standing for the past three (3) preceding years;
• Must be existing for at least 5 years and must be continuously operating for the last three (3) years
• Compliant with the legal and administrative requirements of CDA and other government agencies
Eligibility and Qualifying Criteria
• Must be nominated by any two of the following:
cooperative affiliate, government organizations, local government units (LGUs) and other non-government organizations; .
• Must have an office in the designated official address as indicated in the articles of cooperation

Local Government Units CRITERIA FOR EVALUATION
• Must come from a province/chartered or independent city or components cities and municipalities
• Formally organized/established/created office
• With Cooperative Development Officer duly appointed
• With staff and budget
• Must have relevant cooperative development program/s.
• Must be endorsed by CDS and at least one (1) cooperative in the area

Most Outstanding Cooperative Leaders CRITERIA FOR EVALUATION
• Must be a cooperative member in good standing for the past ten (10) years immediately preceding the nomination – With share capital not exceeding 10% of the
total subscribed capital – Actively participating in the SMCBU program of
their cooperative – Regularly attending General Assembly Meeting – Patronizing the services of cooperatives
• Presently or had been an officer of cooperatives for
at least five (5) years whether elective or appointive

• With no derogatory record / No administrative or criminal record
• With exemplary contributions to cooperative
development
• Must be nominated/endorsed by at least four (4) different type of organizations from the following : cooperative, government organizations, local government units, private sector, other institutions

One (1) winner per category will be selected from among the entries from the regions.

Category of Special Awards
Criteria
The award will be based on the programs/projects/ activities undertaken/ implemented by the cooperative for the year 2012.
• BEST in Environment Protection and Conservation The cooperative must have remarkable programs/projects that promote environment awareness and advance the community in terms of environment protection and conservation.

• BEST in Community Involvement and Development
The cooperative must have remarkable community development and social welfare programs/projects/ activities initiated and/or participated that contributes great impact to the members and the community and nation as a whole.
Criteria
• BEST in Cooperative Solidarity and Partnership
The cooperative must have exemplary performance in terms of building cooperation among cooperatives and alliances/network linkages with the other cooperatives. Implement activities that promote cooperation among cooperatives.
Criteria
• BEST in Membership Expansion
The cooperative must have exemplary performance in terms of increasing membership. Unique strategy to promote regular members will be given more weight.
